Sources included the Guidelines Network. We adopted a conservative approach to selecting indicators for study, aiming to focus on processes not subject to other forms of incentive and for which guidelines remained unchanged throughout the study period. We therefore created five rejection criteria, which are listed below with examples of rejected indicators: 1) Activity does not fall under the remit of all general practices. Example: Percentage of abnormal mammograms that are followed by a biopsy within 14 days. 2) Data to assess performance is not available, or is inconsistently recorded, in practices' electronic patient record systems. Example: Percentage of patients with diabetes who have a record of retinal screening. 3) Substantial change in the evidence base between 2000 and 2007. Example: Percentage of patients with a history of myocardial infarction who are currently treated with an ACE inhibitor. 4) Substantial change in coding procedures or availability of data from electronic laboratory reports between 2000 and 2007. Example: Percentage of patients with coronary heart disease whose last measured total cholesterol is 5 mmol/l or less. 5) Indicator was incorporated into, or dropped from, the incentive scheme after 2004/05. Example: Percentage of patients with chronic kidney disease who have a record of blood pressure. Following the initial selection process we classified the remaining indicators into three types of clinical activity for the analysis: measurement, prescribing, and intermediate outcomes. As we had identified no non-incentivized intermediate outcomes indicators, we also excluded indicators from this category (for example: patients on lithium therapy with a record of lithium levels in the therapeutic range).
