However, the hand involvement is extremely rare. Diagnosing the tenosynovitis due to the tuberculosis is the main challenge for the surgeons. It is aimed to present a case of Mycobacterium tenosynovitis of the hand and outline the treatment protocol for its diagnosis. 46-year-old male patient was presented with swelling in his right hand. Physical examination revealed no pathology except for swelling in the right thenar region. Magnetic resonance imaging (MRI) revealed chronic tenosynovitis. Synovectomy was planned for the patient, and following the thenar incision, mass formations in the form of pale-yellow rice grains were found inside the thickened sheath of the flexor pollicis longus. Since these formations suggested TT intraoperatively, the intraoperative wound culture, samples for acid-resistant staining, TB DNA, and TB culture, as well as pathology samples, were taken. Intraoperative suspicion of TT is extremely important in order to prove the diagnosis. Although TB that affects the hand is rare, it is an element of tenosynovitis that must be kept in mind in areas where TB is endemic. {"title":"Volar versus Dorsal Percutaneous Screw Fixation for Acute Scaphoid Waist Fractures: a retrospective study","authors":"Birkan Kibar, H. Methods: Acute fractures with less than 10 days of injury time, nondisplaced or minimally displaced fractures (<2 mm), and fractures at the scaphoid waist were included in the study. A single type of compression screw (3.5 mm, headless, cannulated, and full threaded) was used in all patients. Postoperatively, a short arm palmar splint with thumb support that held the wrist in a neutral position was applied to all patients for 2 weeks. Results: Nineteen patients were operated with the dorsal approach (group 1), and 17 patients were operated with the volar approach (group 2). There were 17 men and 2 women in group 1, and 16 men and 1 woman in group 2. The mean age of group 1 was 28.1 (range: 18-38) years, and the mean age of group 2 was 27.2 (18-39) years. Union was obtained and radiological consolidation was achieved in all patients in the study and there were no statistically significant differences between the groups in terms of wrist movements, grip and pinch force, or functional scores. {"title":"A Bibliometric Picture of Hand Surgery Subspecialty In Turkey: Where Are We Heading?","authors":"H. Kocaoğlu, M. There are no specific studies that evaluate the current academic productivity of hand surgery at Turkey. With this study we aimed to compare and contrast the quality and the quantity of the hand surgery publications derived from Turkey for two five-year periods. Method: Between 2008-2017, publications containing original articles and reviews published with country of origin as Turkey (CU=TURKEY) in Web of Science (WoS) indexed journals were included in the study. Papers were sorted from WoS categories of surgery, orthopedics and emergency medicine. Then works containing ''Hand” as a keyword grouped into two according to the publication year, 2008-2012 and 2013-2017. The bibliometric data such as the journal in which the publication is published, index of the journal and the number of citations were compared. Results: A total of 16848 publications that meet the criteria published between the specified dates have been evaluated. 421 of these publications contain the keyword \"Hand\". 187 of these publications were published in between 2008 and 2012 and 234 were printed after 2012. A chi-square test showed that there was no significant association between these two periods, X2 (1, N=16848) = .206, p=.65. However, it is seen that after 2012, more publications were started to be published about orthopedics and traumatology (p = .032) and the publications were published more in high-quartile journals (p = .014). There was no significant difference between the number of citations, where average citation per paper was 3.88±5.56 (h-index:12) prior to 2012 and 4.26±6.09 (h-index:14) after (p= .505). Conclusion: Considering the tendency of total publications between 2008-2017, a significant quantitative change is not observed for the second half of this ten-year period. However, contrary to the previous period, publications were accepted by high top-tier journals. {"title":"Is Fixation With K-Wire Sufficient In Distal Radioulnar Joint Instability Accompanying The Distal Radius Fracture?","authors":"A. Şahin, Ş. Recently, the tendency towards surgical treatment has increased in distal radius fractures. Distal radioulnar joint instability is thought to negatively affect the results of distal radius fracture. Our aim in this study is to compare the patients with and without distal radioulnar joint instability, who were operated with the diagnosis of distal radius fracture, clinically and radiologically. Materials and methods: 31 patients who were operated with the diagnosis of distal radius fracture and whose fracture union was completed were included in the study. Patients with an isolated distal radius fracture (19 individuals) were called group 1, while patients with a distal radioulnar joint instability accompanying the distal radius fracture (12 individuals) were called group 2. The groups were compared functionally with DASH and MAYO scores in terms of pain and radiologically with VAS score in terms of radial length, radial slope and volar slope. Joint range of motion was also measured with a standard goniometer and compared between the groups. Results: In the group with isolated distal radius fractures, 11 (57.9%) patients were male and 8 (42.1%) were female. The mean length of follow-up was 22.5 months. In the group with distal radius fractures and accompanying distal radioulnar joint instability, 9 (75.0%) patients were male and 3 (25.0%) were female. The mean length of follow-up in this group was 27.08 months. There was no difference between MAYO, DASH and VAS scores between the groups (p>0.05). The mean radial length was 9.95 ± 1.96 (6; 14) in group 1 and 10.67 ± 2.71 (4; 15) in group 2. The mean radial slope was 19.53 ± 2.78 (14; 26) in group 1 and 18.92 ± 3.6 (11; 26) in group 2. The mean volar slope was 8.32 ± 3.59 in group 1 and 6.92 ± 4.81 (-2; 14) in group 2. It was understood that there was no significant difference between the groups (p>0.05). While the mean supination angle was 80.53 ± 6.64 (60; 90) in group 1, it was 85.42 ± 5.42 (75; 90) in group 2, and it was found to be significantly higher in group 2 (p<0.05). The mean pronation angle was significantly lower in group 2 (p<0.05). Conclusions: DRUJ instability after a distal radius fracture is an important factor affecting the functional outcome. Providing distal radioulnar joint stability is important in this sense.
