Thus, the differential diagnosis of this clinical dilemma is of paramount importance in providing appropriate clinical care because misdiagnosis of this condition may result in healing failure or unnecessary treatments. For this reason, a dental cause must be considered for any cutaneous sinus tract involving the face or neck. Its diagnosis is not always easy unless the treating clinician considers the possibility of its dental origin. {"title":"General approach to the treatment of odontogenic abscesses and cost analysis.","authors":"Aylin Sipahi Calis, Banu Ozveri Koyuncu, Kerem Ozturk, Ali Mert, Cem Bilgen","doi":"10.17096/jiufd.90557","DOIUrl":"https://doi.org/10.17096/jiufd.90557","url":null,"abstract":"<p><strong>Purpose: </strong>The purpose of this study was to evaluate the relationships between age, gender, symptoms, treatment, length of hospital stay and hospital cost in a group of patients with severe odontogenic infection who has been admitted within the last 6 years.</p><p><strong>Subjects and methods: </strong>This study was carried out on 30 patients who had been treated in Ege University, Faculty of Medicine, Otolaryngology Department with the diagnosis of odontogenic abscess. Variables such as age, gender, symptoms, systemic disease, imaging techniques, treatment modalities, hospital length of stay and hospital cost were analyzed statistically.</p><p><strong>Results: </strong>There were 12 female and 18 male patients and their mean age was 39 ± 19.78 years. Antibiotics were used in all subjects and their abscesses were mostly drained surgically. The mean hospital length of stay was 8.1 days. There was a statistically significant relationship with the presence of systemic disease and hospital length of stay variables (p=0.017).</p><p><strong>Conclusion: </strong>The cost for treatment of severe odontogenic infections in inpatient units is high in hospitals. They are more frequent in males and although most often affected teeth are maxillary canines and mandibular third molars, they may also be related with supernumerary or an ectopically erupted tooth. Our aim was to report the management of ectopic third molar tooth related with a dentigerous cyst in maxillary sinus. 16 years old girl referred to our clinic with a complaint of painful swelling over her right cheek since past six months. All permanent teeth were present except the right upper third molar. External examination showed that the face was gently asymmetrical; levels of eye globes were abnormal. Radiographic examination revealed a cystic lesion related with ectopic third molar which located in the roof of the left maxillary sinus. Under local anesthesia, hard drain was inserted into the hole and the cystic pressure was decreased. After the six month of followup, the operation was performed under general anesthesia. The patient's symptoms were resolved completely after surgery and remained symptom-free for over a postoperative follow-up period of 3 year. To avoid unwanted effects of a dentigerous cyst, unerupted tooth should be observed with radiographic imaging. Twostep self-etch dentin adhesive (Clearfil SE Bond®) was applied to the flat dentin surfaces according to the manufacturer's instructions. Composite blocks (Z- 250; 3M ESPE) of 5 mm in height have been prepared by using layering technique. Teeth were stored in water for 24 hours at 37°C and longitudinally sectioned to obtain dentin sticks of 1 mm2.Randomly selected samples from half of the teeth were immediately subjected to micro tensile test and. Remaining specimens were tested after 1 year storage in water. Bond strengths were calculated in megapascal (MPa).</p><p><strong>Results: </strong>Means and standard deviations of the Clearfil SE Bond® micro tensile bond strength values were, respectively, 37.31 ± 13.77 MPa and 24.78 ± 2.99 MPa after 24 h and 1 year of storage in water. This condition can be localized or systemic. Its estimated prevalence is 250 cases in a million and it is more common in women than in men. Resorption of the mandibular angle and coronoid process can be observed in patients with scleroderma. Pressure of fibrous mucocutaneous tissues is thought to be the cause of the resorption. Decreased number of wrinkles due to sclerosis and distinct facial features because of the atrophy of ala nasi are among common clinical characteristics of this condition. In rare cases, odontogenic or peritonsillar abscesses may spread through the deep fascial cervical spaces and cause life-threatening complications. Odontogenic infection is the most common cause of deep neck infections and it accounts for 43% of the cases. Early diagnosis, immediate antibiotic treatment, and surgical drainage are the basis of therapeutic success. Deep neck infections are potentially life threatening complications if they are not diagnosed in time and treated quickly. Prevention of healthy dental structure is essential to help mechanical stabilization of tooth-restoration integrity, increase the amount of suitable surfaces for adhesion and thus positively affect the long-term success. ETT are affected by a higher risk of biomechanical failure than vital teeth. With the development of adhesive systems, the need for post-core restorations is also reduced. Especially for restoration of excessively damaged ETT, endocrowns have been used as an alternative to the conventional post-core and fixed partial dentures. They can be either peripheral, central or extraskeletal. The peripheral osteoma arises from surface of the bone (periosteal) whereas the central osteoma arises from the bone medullary (endosteal) and the extra-skeletal soft tissue osteoma usually develops within the muscle. Osteomas are most commonly found in the skull and facial bones. Multiple osteomas may be associated with Gardner's Syndrome. These lesions are usually painless and recurrence is uncommon after local excision. Periapical radiographs obtained with the parallel as well as the bisecting angle technique, digital and conventional panoramic radiographs were used for implant and peri-implant bone measurements. The measurement results at each implant were statistically analyzed.</p><p><strong>Results: </strong>The ICC values for the inter-observer reliability were 0.79 for implant diameters and 0.96 for implant lengths. Statistical significance was not detected between the differences of the measurements of the 2 examiners from the original implant dimensions related to anatomic locations.
