{"title":"Integrated Care for Type 1 Diabetes: The West Bengal Model.","authors":"Sujoy Ghosh, Masuma Yasmin, Kaushik Sen, Soumik Goswami, Tapas C Das, Subir C Swar, Indira Maisnam, Partha P Chakraborty, Bobby Paul, Dipta K Mukhopadhyay, Pradip Mukhopadhyay","doi":"10.4103/ijem.ijem_124_23","DOIUrl":"10.4103/ijem.ijem_124_23","url":null,"abstract":"<p><strong>Introduction: </strong>A structured dedicated health programme for Type 1 diabetes mellitus (T1DM) has been initiated in the state of West Bengal, India.</p><p><strong>Aim: </strong>The aim is to provide comprehensive healthcare to all children, adolescents and young adults living with T1DM, along with the provision of free supply of insulin, glucose measuring devices, blood glucose test strips, and other logistics. The strategic framework for programme implementation is to utilise the infrastructure and manpower of the already existing non-communicable disease (NCD) clinic under National Health Mission.</p><p><strong>Methodology: </strong>Establishing dedicated T1DM clinics in each district hospital by utilising existing healthcare delivery systems, intensive training and hand-holding of named human resources; providing comprehensive healthcare service and structured diabetes education to all T1DM patients; and building an electronic registry of patients are important components of the programme. T1DM clinics run once a week on the same day throughout the state. All T1DM patients are treated with the correct dose of insulin, both human regular insulin and glargine insulin. Patients are routinely monitored monthly to ensure good glycaemic control and prevent complications of the disease. Routine anthropometric examination and required laboratory investigations are conducted in the set-up of the already existing NCD clinic. Ongoing monitoring and evaluation of the T1DM programme are being conducted in terms of glycated haemoglobin (HbA1c) values, growth and development, complication rates, psychological well-being, quality of life, and direct and indirect expenditure incurred by families. L -1 ) was evaluated in the laboratory. To assess the activity on oviposition and mortality of the mite (1), three weekly applications of the product in an aqueous solution (30, 48, and 66 mg i.a. . L -1 ) were performed. The applications were carried out in the soil, next to the base of yerba mate seedlings. After seven days of each application, leaves were removed for infestation with 15 adult mite females. To evaluate the activity in the development, survival, and reproduction of the progeny (2), yerba mate seedlings were irrigated with azadirachtin solution of 30 mg i.a. . L -1 (recommended concentration). After seven days, leaves were removed and infested with adult females for oviposition and observation of the progeny. Reduction in fecundity (from 23.7 to 44.2%), fertility (27.4 to 48.1%), and an increase in mortality from 14.6 to 47.5% were observed in females fed on azadirachtin-treated plants. Continuous feeding on plants treated with azadirachtin solution significantly reduced oviposition, pre-oviposition period, and longevity of males. In addition, the viability of the egg-adult, and the duration of the egg-adult period increased. The objective of the current study was to evaluate the applicability of the emission of extremely low and specific frequencies by Effatha technology in the soybean Asian rust control, nutrition, and its impact on yield. The in-vivo test followed the detached leaves method, with six treatments: frequencies 1 and 2 individually and in association; the conventional chemical treatment (fungicide azoxystrobin + benzovindiflupyr); and witnesses in presence and absence of the fungus. Frequency 1 relates to inhibition of the enzyme succinate dehydrogenase and 2 to ubiquinone oxidase. In the field, frequencies 1 and 2 associated (with the same fungicidal action of the in-vivo study); nutritional frequency; application of azoxystrobin + benzovindiflupyr + mancozeb, and control without application were evaluated. In vivo, the fungicide provided 85% control of the disease symptoms, against 65% of frequencies 1 and 2 in association, which showed a higher efficiency compared to the isolated frequencies. In the field, the rate of increase of symptoms were reduced by all treatments compared to the control. At the end of the soybean cycle, the conventional fungicide resulted in 33% severity against 56% of frequencies 1 and 2 associated, and 69.2% of the control. The emission of the frequency for increased nutrient efficiency stood out positively on yield in relation to all the other ones. In this work, we propose to reduce or even eliminate metal artifacts by providing better sampling of data using non-circular orbits.<i>Approach.</i>We treat any measurements intersecting metal as missing data, and aim to design a universal orbit that can generally accommodate arbitrary metal shapes and locations. We adapted a local sampling completeness metric based on Tuy's condition to quantify the extent of sampling in the presence of metal. A maxi-min objective over all possible metal locations was used for orbit design. A simple class of sinusoidal orbits was evaluated as a function of frequencies, maximum tilt angles, and orbital extents. Experimental implementation of these orbits were performed on an imaging bench and evaluated on two phantoms, one containing metal balls and the other containing a pedicle screw assembly for spine fixation. Metal artifact reduction (MAR) performance was compared amongst three approaches: non-circular orbits only, algorithmic correction only, and a combined approach.<i>Main results.</i>Theoretical evaluations of the objective favor sinusoidal orbits with large tilt angles and large orbital extents. Furthermore, orbits that leverage redundant azimuthal angles to sample non-redundant data have better performance, e.g. even or non-integer frequency sinusoids for a 360° acquisition. Experimental data support the trends observed in theoretical evaluations. Reconstructions using even or non-integer frequency orbits present less streaking artifacts and background details with finer resolution, even when multiple metal objects are present and even in the absence of MAR algorithms. The combined approach of non-circular orbits and MAR algorithm yields the best performance. {"title":"The declining incidence of cervical spine surgery in patients with rheumatoid arthritis: a single-surgeon series and literature review.","authors":"Roberto J Perez-Roman, Vaidya Govindarajan, David J Levi, Evan Luther, Allan D Levi","doi":"10.3171/2022.2.SPINE226","DOIUrl":"10.3171/2022.2.SPINE226","url":null,"abstract":"<p><strong>Objective: </strong>With an increasing number of disease-modifying drugs available to manage rheumatoid arthritis (RA), spine surgeons have anecdotally noted decreased rates of cervical spine surgical procedures in this population. Although these medications have been shown to mitigate RA progression and its systemic effects on joint destruction, there are currently no large-scale studies of RA patients that suggest the use of these disease-modifying drugs has truly coincided with a decline in cervical spine surgery.</p><p><strong>Methods: </strong>Patients with RA who underwent cervical spinal fusion from 1998 to 2021 performed by the senior author were retrospectively reviewed. The cohort was stratified into 3 categories based on procedure level: 1) occipitocervical, 2) atlantoaxial, and 3) subaxial. The number of surgical procedures per year in each subgroup was evaluated to determine treatment trends over time. National (Nationwide) Inpatient Sample (NIS) data on both RA and non-RA patients who underwent cervical fusion were analyzed to assess for surgical trends over time and for differences in likelihood of surgical intervention between RA and non-RA patients over the epoch.</p><p><strong>Results: </strong>From 1998 to 2021, the number of overall cervical fusions performed in RA patients significantly declined (-0.13 procedures/year, p = 0.01) in this cohort, despite an overall significant increase in cervical fusions in non-RA patients over the same period. NIS analysis of cervical fusions across all patients similarly demonstrated a significant increase in cervical fusions over the same epoch (19,278 cases/year, p < 0.0001). When normalized for changes in population size, the incidence of new surgical procedures was lower in patients with RA regardless of surgical technique. Anterior cervical fusion was the most common approach used over the epoch in both RA and non-RA patients; correspondingly, RA patients were significantly less likely to undergo anterior cervical fusion (OR 0.655, 95% CI -0.4504 to -0.3972, p < 0.0001).</p><p><strong>Conclusions: </strong>At the authors' institution, there was a clear decline in the number of cervical fusions performed to treat the 3 most common forms of cervical spine pathology in RA patients (basilar impression, atlantoaxial instability, and subaxial cervical deformity).
