Amarasiri","doi":"10.4038/amj.v17i2.7780","DOIUrl":"https://doi.org/10.4038/amj.v17i2.7780","url":null,"abstract":"Background: Asthma causes a significant disease burden in children and it is objectively diagnosed by demonstration of reversible airway obstruction. However, objective diagnosis of airway obstruction in preschool children with asthma symptoms is not frequently practised in Sri Lanka.Methodology: This study was a descriptive cross-sectional preliminary study conducted in a convenience sample of 11 consecutive children aged 3-6 years presenting with symptoms of asthma. Asthma Control Test (ACT) was used to assess symptom control. Lung function assessment was carried out using spirometry and impulse oscillometry (IOS) according to European Respiratory Society and American Thoracic Society guidelines. Airway obstruction cut-off was considered as the forced expiratory volume in the first second (FEV1): forced vital capacity (FVC) ratio less than 0.9.Results: The study sample consisted of 4 boys and 7 girls. Nine participants had good control of asthma symptoms (ACT>19). Only three participants performed spirometry in an acceptable and reproducible manner and two of them demonstrated evidence of airway obstruction. FVC less than 0.8 was noted in two participants probably attributable to poor effort (pseudo-restriction). Significant bronchodilator reversibility was not demonstrable in any patient, and two participants had reduced FEV1:FVC ratio following administration of beta-2-agonists probably due to paradoxical bronchoconstriction or improvement of FVC.IOS assessment was completed successfully by seven children. Increased R5-20 was detected in 3 participants and increased resonance frequency in another two children, suggesting probable peripheral airway involvement.Conclusion: Airway obstruction in preschool children could be assessed using spirometry and IOS. Cyanosis, platypnoea and orthodeoxia are the main clinical features and contrast echocardiography is used for its diagnosis. The only available curative therapy is liver transplantation. Here we report a case of 62-year-old lady with CLCD and portal hypertension, who was investigated for hypoxia, clubbing and cyanosis. The epidemiology of eczema among adolescents is distinct from that of children and adults.Methodology: An analytical cross-sectional study recruited 13-14-year-old adolescents from government secondary schools in the Anuradhapura municipal council area, Sri Lanka. The validated International Study of Asthma and Allergy in Childhood questionnaire was used to assess the epidemiology of eczema including the association with demographic, domestic, dietary and allergen factors.Results: The study sample consisted of 1029 participants(528 male, 501 female) with 826(80.3%) 13-year-olds and 203(19.7%) 14-year-olds. The majority were born in the Anuradhapura district(n=931, 90.5%). One hundred and twenty-eight(12.4%) adolescents have had at least one episode of an itchy rash that was periodical at least for six months and 72(7.0%) had a rash in the preceding year. Most of the adolescents developed these symptoms after 5 years of age(n=55, 76.4%). The prevalence of eczema was 3.2%(n=33, 95%CI 2.1–4.3). Comorbid severe asthma (OR 6.7; 95%CI 3.16-14.07), using kerosene lamps for lighting (OR 6.35; 95%CI 1.79-22.54), having close contact with domestic animals (OR 2.29; 95%CI 1.03-5.09), presence of smokers in the house(OR 2.58; 95%CI 1.04-6.40) and female sex(OR 1.95; 95%CI 0.92-4.12) were independently associated with eczema.Conclusion: The prevalence of eczema among adolescents of the Anuradhapura municipal council area was 3.2% and it is not different from previously reported prevalence in preschool children of the district. It is due to the reactivation of the Varicella Zoster Virus (VZV) in the facial nerve ganglion. It is considered the second most common cause of peripheral facial nerve palsy. A 14-year-old immunocompetent boy presented with fever associated with vertigo, facial asymmetry and difficulty in swallowing. He had a few crusted lesions in his left ear pinna and ear canal on examination. A cranial nerve examination revealed a left-sided facial and palatal palsy.
