It can be diagnosed easily by thoracic computed tomography. \u0000Objective: In this study, by thoracic computed tomography, azygos lobe frequency and associated variations were evaluated. \u0000Materials and Methods: In this study, 5854 thoracic computed tomography with and without contrast applied for patients in the Kars Harakani State Hospital Chest Diseases Polyclinic between January 2014 and January 2018 were screened retrospectively. \u0000Results: A total of 5854 thoracic computed tomography showed that frequency of azygos lobe was 1.09%. Out of 64 patients with azygos lobe, 36 (56.25%) were male and 28 (43.75%) were female. Akter","doi":"10.3329/jemc.v9i3.43244","DOIUrl":"https://doi.org/10.3329/jemc.v9i3.43244","url":null,"abstract":"Background: Among term infants, hypoxic-ischemic encephalopathy due to acute perinatal asphyxia remains an important cause of neurodevelopmental deficits in childhood. Treatment is currently limited to supportive intensive care, without any specific brain-oriented therapy. Objective: To determine whether the risk of death or moderate/severe neurodevelopmental impairment in term infants with hypoxic-ischemic encephalopathy increases with relatively high skin or rectal temperature between 12 and 72 hours of birth. Materials and Methods: This was a prospective observational study. Asphyxiated newborns who came within 12 hours of birth were enrolled in this study. Both axillary and rectal temperature were recorded 6 hourly for 72 hours and each infant`s temperature for each site were rank ordered. Then mean of all axillary and rectal temperatures of each neonate was calculated. Outcomes were related to temperatures in logistic regression analyses for the elevated/relatively high temperatures and normal/low temperatures group, with adjustment of the level of encephalopathy and gender. Results: The mean axillary temperature was 36.07 ± 6.10C and in 25.71%, 11.92% and 6.32% cases axillary temperatures were >370C, >37.50C and >380C respectively. The mean rectal temperature was 36.8 ± 60C, and in 43.53%, 30.02% and 19.97% cases rectal temperatures were >370C, >37.50C and >380C respectively. Mean ambient temperature was 26.170C. There was significant correlation between axillary and rectal temperatures (r=0.889). For elevated temperature, the odds of death or moderate to severe impairment increased 8.9-fold (CI 0.906–88.18) and the odds of death alone increased 4.6-fold (CI 0.373–56.83). The odds of impairment increased 1.84-fold (CI 0.45– 7.50). Conclusion: Relatively high temperature during usual care after hypoxic-ischemia in term neonates was associated with adverse neurodevelopmental outcomes. {"title":"Triglyceride to High Density Lipoprotein-Cholesterol Ratio as Predictor of Cardiometabolic Risk among Doctors","authors":"M. Obesity, hyperglycemia, hypertension, insulin resistance and dyslipidemia are the contributing factors for cardiometabolic risk. \u0000Objective: This cross sectional analytical study was conducted to evaluate triglyceride to high density lipoprotein-cholesterol ratio as predictor of cardiometabolic risk among doctors and to compare the risk between male and female doctors. \u0000Results: One hundred ninety five doctors were enrolled in this study. According to triglyceride to high density lipoprotein-cholesterol ratio 63.5% doctors have cardiometabolic risk in which male and female doctors were 66.3% and 57.5% respectively. The prevalence was apparently higher in male doctors than in female doctors. In some parts of the world it is decreasing whereas in the other parts it is increasing. Also there are variations in the risk factors of the disease. \u0000Objective: To see clinicopathological profile of patients of carcinoma stomach in North-East part of Bangladesh. \u0000Materials and Methods: Consecutive new patient diagnosed as carcinoma of stomach were interviewed and data was recorded in a data sheet. \u0000Results: Total 66 cases, age varying from 26−77 (mean 52) years, male 45 (68.2%) and female 21 (31.8%) were enrolled. Out of them 44 (66.7%) were above 45 years. People from lower economic group (51, 78.8%) and rural area (39, 59.1%) were predominantly affected. Commonest presenting symptoms were weight loss, abdominal pain and vomiting. Common site of lesion was antrum and common histopathological type was adenocarcinoma (64, 97%). \u0000Conclusion: Carcinoma stomach is a disease of older age group and male are predominantly affected. Smoking, tobacco and betel nut chewing are risk factors. Isolated gastric and duodenal involvement without pulmonary infection is rare. The presentation of these patients varies. Patients may present with haematemesis, non-healing chronic ulcer, mimicking malignancy, gastric perforation and gastric outlet obstruction. High degree of suspicion is needed for early diagnosis of gastro-duodenal TB. A young female patient who was being treated as a case of nonhealing chronic ulcer was referred for treatment. Histopathological examination of endoscopic biopsy specimen of the patient showed presence of granulomas composed of epitheloid cells and Langhan’s giant cells with caseation with no evidence of tuberculosis at pulmonary or other body sites. After anti-tubercular chemotherapy there was resolution of symptoms and healing of ulcers. According to Global HIV & AIDS statistics 2018, approximately 36.9 million people are living with HIV globally, 77.3 million [59.9 million–100 million] people have become infected with HIV since the start of the epidemic, 35.4 million [25.0–49.9 million] people died from AIDS-related illnesses since the start of the epidemic and 940000 [670000–1.3 million] people died from AIDS-related illnesses in 2017. It weakens a person’s immune system by destroying important cells that fight disease and infection. Dermatologic diseases are common in the HIV-infected population. Skin disease can be uniquely associated with HIV disease and many of the cutaneous diseases are not unique to this group, but the presentation can be more severe and recalcitrant to treatment. The spectrum of skin conditions includes skin findings associated with primary HIV infection and a broad range of skin problems related to the immune deficiency of advanced AIDS. Recognition of characteristic eruptions can facilitate early diagnosis of HIV. A broad variety of neoplastic, infectious and non-infectious diseases can manifest in the skin and may alert the clinician of declining of the immune system.
