Daniela","doi":"10.2478/acm-2019-0013","DOIUrl":"https://doi.org/10.2478/acm-2019-0013","url":null,"abstract":"Abstract Pulmonary fibrosis, represented mainly by idiopathic pulmonary fibrosis, develops chronic and progressive changes in lung parenchyma with high mortality and limited therapeutic options. The aim of this review was to summarize the most common experimental models used in the research of pulmonary fibrosis. Lung damage associated with development of pulmonary fibrosis can be caused by irradiation or by instillation of bleomycin, fluorescein isothiocyanate (FITC), silicon dioxide (silica), asbestos, etc. This article reviews the characteristics of the most frequently used animal models of fibrosis, including the limitations of their use. Kolarovszki","doi":"10.2478/acm-2019-0014","DOIUrl":"https://doi.org/10.2478/acm-2019-0014","url":null,"abstract":"Abstract The sinking skin flap syndrome represents a less-frequent complication in patients after a decompressive craniectomy. It is defined as a neurological deterioration accompanied by a flat or concave deformity of the craniectomy-related skin flap. The underlying brain parenchyma is distorted correspondingly with its blood flow and metabolism being impaired and cerebrospinal fluid hydrodynamics being disturbed, thus causing cerebral dysfunction and neurological symptomatology. The most important options for reversal of this syndrome include Trendelenburg position, maintaining of the cerebrospinal fluid balance, and cranioplasty as a definite solution. We present a patient who underwent a decompressive craniectomy complicated by a cerebrospinal fluid leak in the operative wound treated by means of an external lumbar drainage. Subsequently he developed the sinking skin flap syndrome and a paradoxical cerebral herniation after the drainage system malfunction with a massive cerebrospinal fluid leak at the site of the lumbar drain insertion parallel to the drain itself. His symptoms were, however, successfully alleviated by a positional change, rehydration, and interruption of the lumbar drainage. Staško","doi":"10.2478/acm-2019-0012","DOIUrl":"https://doi.org/10.2478/acm-2019-0012","url":null,"abstract":"Abstract Introduction: Sticky platelet syndrome (SPS) is referred to as a platelet hyperaggregability triggered by low concentrations of platelet agonists adenosine diphosphate (ADP) and/or epinephrine (EPI). Platelet aggregation with other inducers (collagen, arachidonic acid, ristocetin, and thrombin) remains within a normal range. MicroRNAs (miRNAs) are small, non-coding RNA molecules that play an important role in post-transcriptional regulation of protein expression. More recently, several studies show that the platelets are an abundant source of miRNAs and that the miRNA expression profiles within platelets correlate with the platelet reactivity. Aim: The principle objective of this article is to describe the method which we developed for the preparation of the pure platelet samples and report the results of this method. These final pure platelet samples are intended to be the first step for the platelet miRNA testing. Methods: The blood samples from 50 subjects were examined in the study. Then, the platelet rich plasma (PRP) samples obtained by centrifugation of the patient blood samples were used for our experiments. Subsequently, the erythrocytes and leucocytes remaining in PRP sample were magnetically labelled by CD45 Microbeads and CD235a Microbeads. After incubation the PRP sample passed through the magnetic separation system and the magnetically labelled cells (erythrocytes and leucocytes) were retained within the column of separator. The number of cells in the final PRP samples was measured by the blood cell analyser. Invasive candidiases are one of the leading causes of mortality and morbidity in Neonatal intensive care units (NICUs). A more cautious approach is adequate when dealing with fungal infections in premature neonates. Sometimes it is necessary to cure an infection at the very beginning just before manifestation of clinical symptoms. Neonatal colonization due to Candida albicans or non-albicans Candidae predisposes to invasive candidiasis. Pregnancies complicated by preterm delivery should be considered for screening and treatment of maternal Candida colonization to decrease the occurrence of neonatal fungal colonization and its consequences. It is important to prevent infection to spread among patients and avoid complications. Prophylaxis in neonates must be safe and effective. Most authors prefer selective prophylaxis. Fluconazole is the drug of choice for prophylaxis in extremely low birth weight (ELBW) neonates. The prophylaxis is beneficial especially in NICUs with high rates of invasive candidiases. The authors describe benefits and trends in prophylaxis. Lacey","doi":"10.2478/ACM-2019-0006","DOIUrl":"https://doi.org/10.2478/ACM-2019-0006","url":null,"abstract":"Abstract Aim of the study: The aim was to perform a morphometric analysis of subcutaneous and visceral adipose tissue of morbidly obese women and to determine the relationship between adipocyte size and the development of type 2 diabetes (T2D). Materials and methods: White adipose tissue of morbidly obese women was obtained from subcutaneous and omental adipose tissue during bariatric surgery. The same tissues were obtained at judicial autopsy in non-obese (lean) non-diabetic patients. The harvested tissue was embedded in paraffin and 5 μm thick hematoxylin-eosin stained sections were analyzed by the Olympus cellSens system. Statistical evaluation was performed by GraphPad Prism 6.1 software. Results: We found a relationship between adipocyte size and the presence of T2D. The most pronounced changes were seen in visceral adipocytes (cell diameter increased from 61.9 μm in controls to 79.5 μm in patients with T2D). Also, the size of the subcutaneous adipocytes increased against the control. A statistically significant difference between diabetic and non-diabetic patients was not proven in subcutaneous adipocytes. We also observed differences in the distribution of adipocyte mean diameters. Whilst in the control group there was a normal (Gaussian) distribution, in the morbidly obese we found an asymmetric distribution with a positive skewness to the right. In the world of Orthodox medicine, the Rod of Asclepius is regarded as the true symbol of medicine. However, there exists to be an issue of interchange of the correct medical symbol (i.e. Rod of Asclepius) with another similar symbol (i.e. the Caduceus). This study aims to explore medical students’ knowledge and opinion on the appropriate symbol of medicine. Methods: This study was a cross-sectional survey of 84 medical students at the Usmanu Danfodiyo University (UDU), North-West Nigeria. Study tool was a paper questionnaire. The collected data were analyzed using the Epi info 7 Software. Results: The mean (±SD) age of the participants was 23.7 (±3.4) years, 72.6% were males, and 73.8% were in their 4th year. Only 59.5% had interest in non-medical literature. Also, only 6.0% had doctors as their parents. The majority (88.1%) of the participants erroneously identified the Caduceus symbol as the most appropriate symbol of medicine. Furthermore, only 45.2% indicated that the Rod of Asclepius and the Caduceus symbols originated from ancient Greece. Virtually all (97.6%) the participants opined that the Caduceus symbol is the most popular symbol of Medicine. Finally, the majority (73.8%) of the participants recommended that a course on the History of Medicine should be added to the medical curriculum of their school. Conclusion: This study found that the majority of the surveyed medical students did not know much about the historically correct medical symbol. The vast diversity of commercially available NIRS devices makes it difficult to compare in the published clinical studies. This review provides a view on the practical use of NIRS as a tool for cStO2 measurement, its limitations and pitfalls, with a focus on brain dysfunction caused by hypoxic-ischemic encephalopathy. This syndrome of disturbed neurologic function in the earliest days after the birth in the term infants is manifested by difficulty with initiating and maintaining respiration, depression of tone and reflexes, subnormal level of consciousness, and often seizures. Dysregulation of autonomic nervous system - especially sympathetic overdrive – is one of the factors considered to play a key role in its aetiology and development. There is a growing evidence of a large impact of sympathetic system on the development of MVP. Exaggerated sympathetic activity may lead to morphologic changes in valves tissue such as thickening and redundancy. Nowadays, few investigative methods are known for evaluation of the regulatory state of sympathetic nervous system, which could be, theoretically, used to identify the subjects with sympathetic overactivity associated with an increased cardiovascular risk. Electrodermal activity or blood pressure variability represent promising non-invasive methods for evaluation of the regulatory outputs of sympathetic nervous system. There is a possibility to extend a set of investigative methods in MVP and include the monitoring of sympathetic activity in the assessment of cardiovascular risk.
