Mueller, Konstantinos Koskinas, Isabella Sudano Noll, Franois Mach","doi":"10.4414/cvm.2022.02183","DOIUrl":"https://doi.org/10.4414/cvm.2022.02183","url":null,"abstract":"Die Cholesterinhypothese ist eines der am besten dokumentierten Konzepte der modernen Medizin mit über zweieinhalb Millionen Patienten, die in randomisierten kontrollierten Langzeitstudien mit Statinen vsersus Placebo, verschiedenen Statinen im Vergleich oder verschiedenen Dosierungen desselben Statins eingeschlossen wurden [1]. Alle diese Untersuchungen zeigen einheitlich, dass eine Senkung des Cholesterins, insbesondere des Low-density lipoproteins (LDL-C), dosisabhängig zu einer Reduktion von Herzinfarkt, Hirnschlag und verfrühtem Tod um ca. 25% pro Jahr führt [2]. Die absolute Risikoreduktion durch eine LDL-C Senkung hängt von der Höhe des individuellen Risikos, der Höhe des Cholesterins sowie dem Ausmass und der Dauer der LDL-C Senkung ab [2]. Entsprechend wurden die Statine seit Ende der 80erJahre zunehmend bei allen Herzpatienten, vor allem denjenigen mit koronarer Herzkrankheit, aber auch bei noch gesunden Individuen (mit kardiovaskulären Risikofaktoren, wie erhöhtem Cholesterin, Hypertonie oder Diabetes) eingesetzt. Die Statine senken die Cholesterinproduktion in der Leber, die etwa 85% des Gesamtpools von Cholesterin im Körper ausmacht, durch Hemmung der HMG-Coenzyme-A Reduktase. Dies führt zu einer Hochregulation der hepatischen LDL-Rezeptoren mit nachfolgender Senkung der LDL-Serumkonzentration. Die Statine gehören zu den wohl wirksamsten und am besten verträglichen Medikamenten der kardiovaskulären Medizin und werden entsprechend breit verwendet. Von der Potenz her, sind Atorvastatin, Pitavastatin und Rosuvastatin, die wirksamsten Statine, welche das LDLCholesterin dosis-abhängig bis zu 50% senken, gefolgt von Simvastatin, Fluvastatin und schliesslich Pravastatin, der am wenigsten wirksamen Substanz [3, 4]. Zusätzlich zu den Statinen wird vermehrt auch eine Kombination mit Ezetimib verwendet, einem Hemmer des Niemann-Pick C1-like Proteins 1 (NPC1L1) im Darm. Damit wird die Cholesterinaufnahme aus der Nahrung reduziert, wodurch es zu einer Senkung des LDL-Cholesterins um etwa 20–25% kommt [5].
