Franco","doi":"10.1684/orthodfr.2019.190034","DOIUrl":"https://doi.org/10.1684/orthodfr.2019.190034","url":null,"abstract":"Obstructive sleep apnoea syndrome (OSAS) is a frequent sleep-related breathing disorder in children with a prevalence of approximately 3% between the ages of 3 and 8. Its origin is multifactorial (hypertrophy of pharyngeal soft tissues, narrowing of the bone airways, damage to the neuromuscular tone). The symptoms and clinical signs to look for during the day and at night are directly related to upper airway (UA) obstruction or poor sleep quality. After a complete anamnestic and clinical evaluation, including nasofibroscopy, the ENT specialist will refer to an adeno-amygdalectomy or night recording by polysomnography or polygraph to confirm the diagnosis in the child. Among adolescents, the prevalence of OSAS is reported to be between 0.5 and 6%. The main risk factors are obesity, male sex and a history of tonsillectomy. In addition to the classic symptoms of childhood OSAS, this syndrome may, in adolescents, manifest itself as a misleading semiology of dyssomnia, excessive daytime sleepiness and/or mood disorders. Differential diagnoses with risk behaviours, phase delay or narcolepsy should be systematically discussed. It is essential to record breathing during sleep. Even if the obstructive apnea/hypopnea index is low, it must be considered. In both children and adolescents, multidisciplinary management (ENT, orthodontist, maxillofacial physiotherapist, pulmonologist) should be early in order to avoid neurocognitive, behavioural, cardiovascular and metabolic complications. Maxillofacial surgery can be discussed from the age of 15. A systematic review of the literature was performed on data specific to humans. Two aspects are involved: on one hand, the mechanisms contributing to intermittent obstruction of the upper airways (UA) during sleep and, on the other hand, the impact of this obstruction, e.g. neurocognitive disorders, cardiovascular disease and metabolic dysregulation. UA obstruction can be explained by anatomical, mechanical and neuro-functional conditions, especially the proprioceptive and chemical feedback of UA neuromuscular activity. Our understanding of the impact of OSAS has benefited from the recently developed concepts of oxidative stress and low-grade systemic inflammation, the discovery of hypoxia-sensitive agents and of the role of cytokines. The purpose of this review is to show how all these aspects of connected health are already used in the management of sleep apnea syndrome (SAS) and its comorbidities. First, it can give patients a better understanding and a better assessment of their health. It also facilitates their healthcare by allowing them a greater role in their care pathway. For healthcare providers, connected health tools make it possible to set up new procedures for diagnosing and monitoring ambulatory patients, and for the making of joint decisions by health professionals and patients. Finally, for researchers, e-health generates massive amounts of data, thus facilitating the acquisition of knowledge in real life situations and the development of new methodologies for clinical studies that are faster, less expensive and just as reliable. Pételle","doi":"10.1684/orthodfr.2019.190030","DOIUrl":"https://doi.org/10.1684/orthodfr.2019.190030","url":null,"abstract":"The obstructive sleep apnea syndrome is a chronic condition characterized by repeated nocturnal pharyngeal collapse leading to poor-quality sleep and an increased risk of cardiovascular episodes. The classic management by continuous positive-pressure ventilation has its limitations and is not always well tolerated. The treatment alternative comprising an orthotic device is effective in 2/3 of cases, despite the dental limitations and risks inherent in long-term usage. Through a review of the literature, this paper describes the different surgical solutions used to correct obstructive sleep apnea syndrome in adults. The proposed corrections aim to maintain pharyngeal permeability during sleep either by reducing soft tissue volume or by enlarging the facial skeleton or, more recently, by acting on the tone of the pharyngeal dilator muscles. Among these different approaches, the highest success rate was reported for the maxillomandibular advancement procedure. This technique demands rigorous patient selection combined with an appropriate orthodontic work-up and management. Patient selection and the respective benefits and drawbacks of the different approaches are discussed. Correction of craniofacial risk factors (maxillary deficiency and retrognathia) is commonly used by practitioners alongside orthodontic treatment such as OMA and RME in the optimal conditions afforded by childhood growth. Myofunctional therapies are performed to restore correct stomatognathic function and play a central role in the management of SDB in children. The evaluation of the effectiveness of treatments is now based on a normal apnea/hypopnea index, a normal oxygen saturation and the disappearance of sleep fragmentation. Clinical and biological follow-up of co-morbidities must be ensured. Compliance with treatment, a major element of success, must be monitored. Continuous Positive Airway Pressure (CPAP) is still the most effective and widely used treatment. However, mandibular advancement orthosis is now recommended as a first-line treatment in moderate forms in the absence of severe cardiovascular comorbidities (CV) and remains a therapeutic alternative in the event of refusal or intolerance to CPAP. Nutritional management in cases of obesity and a physical activity program must be systematically offered. A reduction in nocturnal breathing disorders can also be achieved by using a device in the event of positional OSAHS, or by limiting fluid movements. The contribution of myofunctional therapy is currently being evaluated. Very recently, a study testing the efficiency of pharmacological treatment on the severity of OSAHS and the reactivity of geniogloss appears to be promising.
