{"title":"Unusual Presentations for EBV and HHV8 Co-Infections: A Case Report and Review of Literature","authors":"Sara Cingarlini","doi":"10.47363/jccsr/2023(5)258","DOIUrl":"https://doi.org/10.47363/jccsr/2023(5)258","url":null,"abstract":"HHV-8 is known to be related to Kaposi sarcomas but can be responsible for other diseases such as Castleman’s disease (MCD) and primary effusion lymphoma (PEL). Epstein-Barr virus, on the other hand, is generally responsible for B-lineage lymphoproliferative diseases (LPD). In this case report we describe the case of a co-infection with HHV-8 and EBV with an unusual presentation. It is in fact a Kaposi sarcoma diagnosed in a heterosexual, immunocompetent and HIV-negative patient who initially presented with nodal disease only and a severe hemolytic anaemia. The same patient was also diagnosed with a T-lineage LPD with only bone marrow localization. The overall clinical presentation evolved favorably until complete resolution with the use of chemotherapy including drugs active in both histotypes. These data are contextualized with the evidence available in the current literature regarding the most common clinical manifestations of these infections. Overweight and obesity account for more than 80% of CHD patients. Obesity is sometimes viewed as a “minor” CHD RF, however it is a widely effective risk-factor approach. Soto","doi":"10.47363/JCCSR/2020(3)167","DOIUrl":"https://doi.org/10.47363/JCCSR/2020(3)167","url":null,"abstract":"We have observed semantic memory and episodic memory disorders (100%) in patients ranging from 40 to 92 years-old, associated to cardiovascular diseases and blood hypertension (82%), sleep disorders (50%), neurobehavioral disorders (44%), such as depression, anxiety, aggression, and vascular demencia, disorders of language (36%), neurosensory disorders (28%), as diminution of visual and hearing acuity, dizziness (26%), Parkinson disease (34%), Alzheimer disease (21%), gait disturbances (10%), vertigo (10%), cervicalgia and cervicogenic headache (10%) trigeminal neuralgia (2%,), We observed as comorbidities the following non-nervous diseases: metabolic diseases as diabetes (21%) and hypothyroidism (5%), gastrointestinal pathology (21%), such as constipation, loss of sphincter control, and gastritis, arthritis (13%), prostatic hypertrophy (1%) and loss of weight (1%). A sample of 117 residents was taken randomly from the residents. We found that 51.3% of the residents have knowledge about the consequences and side effects of the antibiotics, while 48.7%of the residents have no knowledge about the consequences and side effects of the antibiotics. A sum of 66.7% use antibiotics without a medical prescription, while 33.3% do not use antibiotics on their own. Most of the residents seek medical advice if they don’t feel better after using antibiotics. We report a case of a 62 year old gentleman with a primitive neuroectodermal tumor arising from the subcutaneous tissue of the right axilla. He was treated with multimodal treatment including surgery and radiotherapy. On admission she had no significant electrocardiographic and echocardiographic changes and a negative Troponin test. Coronary angiography revealed single vessel disease: severe stenosis of the right coronary artery (RCA) ostium (difficult to assess visually), 50% mid-vessel and 60% distal segment. The left anterior descending artery and circumflex artery had non-significant stenoses. Fractional flow reserve technique (FFR) was used to evaluate the RCA ostial lesion which proved to be significant, therefore angioplasty with three drug-eluting stents was performed for all three lesions of the right coronary artery, starting from the ostium. Due to its location, minimal aortic protrusion of the first stent occluded a small ostial branch which proved to be the conus artery and the patient developed mild transient angina during the procedure, but with good outcome regarding the intracoronary flow. After the angioplasty the patient presented anterior leads ST-elevation and developed mild chest pain with an increase in cardiac enzymes (CK-MB peak 39 U/L). Later on, she had two episodes of ventricular fibrillation with rapid defibrillation to sinus rhythm, with no further events or echocardiographic changes and no recurrent angina. The patient was started on amiodarone to prevent ventricular arrhythmias and continued double antiplatelet therapy with aspirin and clopidogrel. She was discharged six days later. The major indications for pancreatic stent placement are: pancreatic duct stones, chronic pancreatitis, pancreatic strictures, unresectable pancreatic cancer, preventing POPF and post-ERCP pancreatitis, papillary adenoma. Stent placement or retrieval and exchange is difficult process and sometimes it could be the serious clinical challenge. We report two cases of successful removal of firmly impacted pancreatic duct stent by open surgical intervention and transduodenal approach with lateral “side-to-side” pancreaticojejunostomy to prevent the risk of main pancreatic duct restenosis in the first case and with the sphincteroplasty in the second.
