Compared to surgical treatment, it offers a safer alternative with better results and a faster recovery for the patient. Complications that may occur after polypectomy by EMR are amenable to endoscopic treatment with good success following the recommendations for endoscopic management after iatrogenic perforations. The presented clinical case proves that the risk of perforation increases with the size of the polyp, and that polypectomy in such cases should be performed by an experienced endoscopist and a trained team to ensure successful treatment.","PeriodicalId":298689,"journal":{"name":"Journal of Endourology and Minimally Invasive Surgery","volume":"161 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2022-11-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"115415105","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

It represents 0.073% of all external hernias. Frequency is greater in older women between the 7th and 9th decades. The male: female ratio is 1:6. It mainly presents with symptoms of small bowel obstruction and accounts for 0.2-1.6% of all causes of ileus. Other concomitant complaints are pain in the thigh radiating to the knee (a symptom of Howship-Romberg) or swelling in the inguinal region with abdominal pain. Case Report: We present an 83 -year -old woman who was admitted to the Surgery department of the Military Medical Academy – Sofia with complaints of abdominal pain, vomiting and stop of defecation for several days. From the tests done, ileus of small intestinal origin was established. The diagnosis of incarcerated, obturator hernia was made after a contrast-enhanced CT scan of the abdomen and pelvis. The patient underwent emergency surgery, during which the incarcerated, small intestinal loop was released, retrograde debarasage of the small intestine and plastic of the obturator channel with its own tissues was done. Conclusions: Small bowel obstruction caused by incarceration in obturator hernia is one of the rare causes of intestinal obstruction in surgery, but on the other hand is a common manifestation in this type of hernia and one of the main causes of its detection. The combination of operative technique for the treatment of ileus condition and the plastic of the obturator channel produces good postoperative results.","PeriodicalId":298689,"journal":{"name":"Journal of Endourology and Minimally Invasive Surgery","volume":"12 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2022-11-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"125354242","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

To facilitate retrograde access to the kidney, extraction of fragments and reduction of intrarenal pressure during surgery, the ureteral access sheath (UAS) was introduced. The aim of the present study was to investigate the incidence of postoperative stricture after the use of UAS in unstented patients and the effectiveness and safety of different sizes of UAS. Material and methods: A retrospective study was performed, including 240 fURS procedures with the use of UAS, performed between January 2019 and April 2022. We excluded cases in which a previous intervention was performed, patients with a JJ stent, nephrostomy or impacted stones, patients who underwent radiation treatment, or had urinary tract malignancies. The postoperative stricture was considered the presence of hydronephrosis on ultrasound examination or on CT scan in the first and third months. A comparative analysis of the effectiveness and safety of using the 11/13 Fr UAS and the 12/14 Fr UAS was performed. Results: The cohort included 167 patients with a postoperative follow-up of 3 months. In 143 patients (85.7%) UAS 11/13 Fr was used, in 24 patients (14.3%) UAS 12/14 Fr. No postoperative stricture was found in any of the patients. There was no significant difference in efficacy after one procedure using UAS 11/13 versus UAS 12/14 (93.0% vs. 91.7%, p=0.684), despite the larger stone volume in UAS 12/14 (123.4±93.5 mm2 vs. 95.5±41.5, p=0.009). Mean operative time was significantly longer in UAS 11/13 (38.7±11.2 min vs 50.2±16.2 min; p=0.049). Larger UAS size was not associated with more complications compared to a smaller size, comparative safety analysis found no significant differences in the incidence of intra- and postoperative complications (0% vs. 0.7%, p=0.821 and 8.4% vs. 7.2% p=.705, respectively). Conclusions: The results of this study suggest that UAS use has no impact on the efficacy of fURS and is not associated with an increased rate of intra- and postoperative complications.","PeriodicalId":298689,"journal":{"name":"Journal of Endourology and Minimally Invasive Surgery","volume":"531 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2022-11-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"133304428","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Ureteral access sheath (UAS) was introduced in the clinical practice to facilitate endoscopic access into the upper urinary tract, reduce intrarenal pressure during the procedure and allow the extraction of fragments. The objective of this study is to analyze the impact of UAS on the efficacy and safety of flexible ureteroscopy (fURS) for the treatment of renal stones. Material and methods: a prospectively collected database including 251 consecutive patients treated with fURS between January 2018 and November 2022 was analyzed. Patients were divided into two groups: group 1 – without UAS, 21 patients (8.3%) and group 2 – with UAS, 231 patients (91.7%). UAS was used at the surgeon’s discretion and 3 patients (1.2%) underwent a sheathless procedure due to failure of UAS insertion. Data on patients’ preoperative characteristics and fURS efficacy and safety were compared. Results: There were no statistically significant differences in patients’ preoperative characteristics except for stone size and surface, which were significantly larger in group 2 (9.6±4.1 vs 13.8±5.2 mm; р<0.001 and 68.6±74.2 vs 128.3±109.3 mm2, p=0.002, respectively). Operating time was significantly longer in group 2 (33.6±12.9 vs 42.0±12.9 min, p=0.005). Stone-free rate after a single procedure and at 3rd postoperative month was similar between groups (95.2% vs 87%, p=0.486 and 95.2% vs 88.7%, p=0.709, respectively). The intra- and postoperative complication rates were higher for group 1 (4.8% vs 0.8%, р=0.011 and 14.3% vs 12.1%, p=0.062, respectively). Multivariate regression model identified stone surface (р<0.001), number of stones (p=0.005) and calcium oxalate monohydrate stone composition (р=0.005), but not UAS use (p=0.580), as independent predictors for stone-free rate following fURS. Conclusions: The results of this study suggest that UAS use has no impact on the efficacy of fURS and is not associated with an increased rate of intra- and postoperative complications.","PeriodicalId":298689,"journal":{"name":"Journal of Endourology and Minimally Invasive Surgery","volume":"8 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2022-11-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"131191757","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The increased technical complexity of this procedure implies a gradual increase in surgical difficulty, relative to the surgeon’s experience. Various difficulty scoring systems (DSS) have been developed to facilitate patient selection for laparoscopic hepatectomy. Methods: We performed an analysis of a prospectively maintained database for the period January 2019 – January 2022. We identified patients, who underwent laparoscopic hepatectomy. They were staged according to the four DSS: Iwate, Institut Mutualiste Montsouris (IMM), Southampton and Hasegawa. We compared the results between the difficulty levels with the actual intraoperative parameters (Pringle-maneuver time, blood loss, hemotransfusion rate and operative time) and postoperative results. The data was evaluated using Jonckheere-Terpspa trend analysis and AUROC analysis. The correlation was assessed using Spearman’s rho. Results: We identified 60 patients, satisfying the inclusion criteria. The mean age of the group was 60 years, with 43% being male. The analysis confirmed the positive correlation between the four DSS and the intraoperative parameters. The risk of conversion or adverse postoperative outcome could not be adequately predicted by either of the DSS. Regarding the conversion risk, the IMM scale has the best discriminative ability (AUC=0,597), while the Hasegawa scale has the best ability to predict postoperative complications (AUC=0,612). Conclusions: We validated the four DSS for the Bulgarian population. From a practical standpoint, the IMM scale is most convenient for application during routine surgical practice.","PeriodicalId":298689,"journal":{"name":"Journal of Endourology and Minimally Invasive Surgery","volume":"62 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2022-11-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"114239147","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The objective of our study is to compare the efficacy and safety of minimally invasive percutaneous nephrolithotripsy (mini-PCNL) and ureteroscopy (URS) in the treatment of impacted proximal ureteral stones. Material and methods: The medical records of 345 patients with impacted proximal ureteral stones 10-20 mm, treated between January 2016 – November 2022 were retrospectively reviewed. 178 patients (51.6%) underwent mini-PCNL, and 167 (48.4%) – URS. Data on patients’ preoperative characteristics, stone-free rates, operating times, intra- and postoperative complications were compared. Results: Patients’ preoperative characteristics were comparable between the two groups, except for mean stone surface and stone size, which were significantly higher in the mini-PCNL group (respectively, 137.87±44.32mm2 vs 80.94±29.66mm2, p=0.000; and 15.03±2.43 vs 12.02±2.22mm, p=0.000). The stone free rate after single procedure was 94.9% for mini-PCNL and 89,3% in the URS group (p=0,116). Four patients (2.3%) in the mini-PCNL group had significant hemorrhage necessitating selective renovasography and angioembolization of AV fistula. 21 (12.5%) patients had stones showing upward migration during the URS procedure and 3 (1.8%) had partial perforation of the ureteral wall. Hemoglobin drop was significantly higher in mini-PNL group (13.58±12.79g/l vs 4.69±5.72g/l, p=0,000). Mean operative time was significantly longer in the URS group, despite the smaller size of the stones (respectively, 41.41±13.62min vs. 30.67±10.34min, p=0,000). There were statistically significant difference of the necessity of auxiliary procedures in the URS group (respectively, 78 patients (46,4%) vs 6 patients (3.6%), p=0.000). Conclusions: For an impacted, proximal ureteral stone mini-PNL had better stone-free rates, shorter operative time and low percentage of auxiliary procedure and hospitalizations. However, URS had the advantages of less invasiveness procedure and shorter postoperative hospital stay.","PeriodicalId":298689,"journal":{"name":"Journal of Endourology and Minimally Invasive Surgery","volume":"15 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2022-11-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"114614091","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Management of such patients is challenging due to the body habitus and the presence of multiple comorbidities. It is unclear whether percutaneous nephrolithotomy (PCNL) has similar efficacy and safety in obese patients as compared to patients with normal body mass index (BMI). The objective of this study is to assess the impact of body mass index on the efficacy and safety of PCNL in patients with BMI > 30 kg/m2. Material and methods: The medical records of 124 obese patients with BMI > 30 kg/m2, treated between January 2017 and September 2022 were retrospectively reviewed. All patients underwent standard PCNL with ultrasound probe disintegration of stones. Data on patients preoperative characteristics, stone free rates, operating times, intra- and postoperative complications were analysed. Results: Patients’ mean age was 57.0±9.2. The majority of patients had at least one comorbidity – 53 (43.4%) had diabetes, 45 (36.3%) – hypertension and 7 (5.6%) had ischemic heart disease. 100 (80.6%) patients had BMI between 30-34 kg/m2, 12 (9.7%) – between 35-39.9 kg/m2 and 12 (9.7%) patients had BMI measured over 40 kg/m2. Mean stone length was 40.0±14.9 mm and mean stone width 29.3±28.7 mm. In the majority of patients 118 (95.2%) lower pole puncture was performed for access to the stone. 54 (43.5%) of the patients had staghorn stones and 58 (46.8%) had stones composed of uric acid. Mean operative time was 50.3±12.2 min and mean hospital stay 3.5±0.9 days. Stone-free rate after a single procedure was 80.6%. Postoperative complications were fever in 5 (4%) patients, haematuria in 2 (1.6%), formation of AV fistula in 2 (1.6%) and renal colic – in 3 (2.4%) patients. Management of these complications included medical treatment in 7 (5.6%) patients, double JJ stent insertion in 4 (3.2%) and angioembolization in 2 (1.6%). Conclusions: PCNL is safe and effective procedure in obese patients with BMI > 30 kg/m2. It can be primary in origin over the lumbar triangles that can be categorized into superior (Grynfeltt) and inferior (Petit), or secondary as a result of trauma or surgery. Lumbar abdominal wall hernia in which clinical presentations may vary from an asymptomatic bulge in the lumbar area to a symptomatic lumbar mass with back pain. Anatomical location makes challenging diagnosis and repair. Individual approach for every patient is a key for successful surgical treatment. In this paper, we present a minimally invasive approach to the treatment of incisional lumbar hernia. Case presentation: A 75-year-old patient from Pazardzhik town, admitted to our unit at the Military Medical Academy for swelling in the left lumbar region at the site of an old surgical scar from a previous nephrectomy (2020) caused by nephropathy. The swelling has a one-year history and causes periodic local pain and discomfort during physical exercise. CT-scan show us incisional left site lumbar hernia, 10cm defect diameter with descending colon and spleen as a hernia contents. We conducted preoperative preparation and consultations with an anesthesiologist, cardiologist and nephrologist. The patient underwent minimally invasive transabdominal preperitoneal lumbar hernia repair with a prosthesis. The postoperative period went smoothly and the patient was discharged on the fourth day. Three months later, he feels well and there is no evidence of recurrence. Conclusions: Preoperative CT-scan of the abdomen with contrast in lumbar hernia patients is a mandatory requirement for proper diagnosis and selection of an appropriate surgical approach. Minimally invasive transabdominal preperitoneal incisional lumbar hernia repair provides optimal view in operative field, minimal tissue trauma, fast recovery and good cosmetic results. This is a new method, but a feasible, useful and safe alternative to conventional surgical techniques for incisional lumbar hernia repair.
