Fiase","doi":"10.11648/J.IJCEMS.20170305.12","DOIUrl":"https://doi.org/10.11648/J.IJCEMS.20170305.12","url":null,"abstract":"The purpose of the study was to assess the prevalence of caesarean-section among primigravida in general hospital Gboko, Benue State, Nigeria. The study was a hospital-based cross-sectional retrospective survey conducted from January 2016-December 2016. The target population comprised all the primigravida within the age of 16-45 who underwent caesarean section in General Hospital Gboko, Benue State, Nigeria. The sample size consisted of 224 patients that met the inclusion criteria of caesarean deliveries performed after period of viability (28 weeks). Data collection instrument was the hospital delivery records from the maternity units, theatre records, admission and discharge registers in labour ward with outcome of caesarean section using delivery collection sheet. Collected data were analysed using frequency distribution table. Findings revealed increased incidence of caesarean section from a steep of 17.9% recorded in January-February after a rapid rise to 20.5% in September-October to 22.3% in November-December. Finding also showed that most of the cases of caesarean-section achieved a life viable fetus with an APGAR score of 8/10 in the first five minutes of life. Major indications for caesarean-section were due to failure to progress during labour, previous uterine scar, no reassuring fetal status and fetal mal-presentation. Proximal migration of the VPS catheter is a rare complication following ventriculo-peritoneal shunting. This is a report of a management of 8-month-old-boy who had extrusion of peritoneal end of VPS catheter through scalp wound. VPS was placed at the age of 4-month for congenital / obstructive hydrocephalus. One and half month after VPS placement, he had redness and subcutaneous fluid collections around the VPS catheter tract at neck and chest area and were treated well with oral antibiotics. Four months after the shunt implantation, he was admitted with a complaint of that the end of peritoneal catheter was extruding out through scalp wound. He was managed well with removal of entire VPS tubing through scalp incision, injectable antibiotics, and delayed re-VPS placement on right side. Three-week after shunt revision, he again presented with extrusion of part of peritoneal catheter from neck area and was managed with removal of peritoneal catheter, and ventricular catheter was used as external ventricular drainage (EVD). His Computed Tomography (CT) scan of head showed dilatation of lateral ventricles (more on right side) and therefore right sided VPS was placed again after a month. This is a cross-sectional comparative study. 24 patients with BS operated of SO sharpening were enrolled in this study. The adduction-elevation restriction (AER), torticollis, visual acuity, treatments, trochlea triamcinolone-injections, age of surgery and systemic diseases were assessed. Fisher’s test was used to analyze if there is any association between the variables analyzed with systemic diseases. Differences between preoperative and postoperative status were analyzed using the Wilcoxon test with Bonferroni correction post hoc. We found a preoperative mean AER (0-3) was 2.88. One year after the surgery, mean AER it was 0.59 (P 0.05). Gastric perforation and extrusion of peritoneal part of VP shunt catheter through mouth is an extremely rare complication. This is a report of a management of a two-year-old girl who had extrusion of peritoneal part of VP shunt catheter through mouth. Right sided VP shunt was done for communicating hydrocephalus associated with meningitis at the age of one year. She required revision of peritoneal part of shunt for extrusion of peritoneal catheter through the abdominal healed scar, 3 months after the placement of VP shunt. At the age of two-year she was admitted with bouts of vomiting and cough, followed by peroral extrusion of peritoneal part of VP shunt catheter. She was managed with removal of peroral extruded part of the shunt, external ventricular drainage, and delayed shunt revision. Three weeks later, she was admitted again with a history of extrusion of part of VP shunt catheter through abdominal wound, and was managed with re-insertion of peritoneal catheter in to the peritoneal cavity through a different abdominal site. At last follow-up done two months later, she was doing well and her abdominal wounds were also healed well.
