Semyonova","doi":"10.24884/0042-4625-2018-177-6-63-68","DOIUrl":"https://doi.org/10.24884/0042-4625-2018-177-6-63-68","url":null,"abstract":"Objective.To assess the outcomes of the treatment of patients with suppurative necrotic complications of diabetic foot syndrome (DFS) without critical ischemia with the use of programmed rehabilitation technologies (PRT).Materialand methods.The study included 82 patients. 40 patients (reference group) underwent conventional rehabilitation, while 42 patients (study group) were treated with PRT with the use of the original equipment AMP-01.Results.Use of PRT has been established to significantly improve the quality of rehabilitation, which helps to reduce the duration of inpatient treatment. The number of suppurative complications in the study group proved to be significantly less (p=0.014), and postoperative mortality was lower. Kosenko","doi":"10.24884/0042-4625-2018-177-6-31-35","DOIUrl":"https://doi.org/10.24884/0042-4625-2018-177-6-31-35","url":null,"abstract":"The objectiveof the article is to discuss diagnostic peculiarities of artificial pyoinflammatory diseases of soft tissues (APIDST).Material and methods.We analyzed the clinical data of 302 military men.Results.As compared to the non-artificial pyoinflammatory diseases of soft tissues of level 2-4 (PIDST of level 2-4), APIDST are characterized by the presence of local self-induced skin injuries that are mostly located on shins, the crepitation during palpation in the projection of the abnormal focus, and the signs of anaerobic infection (86.1%) and cellular-fascia-myositis (57.6%) during the surgical intervention in the lesion.Conclusion.The revealed clinical features of pathology should be taken into account in the treatment. We carried out retrospective analysis of the results of endoscopic papillosphincterotomy (EPST) due to the presence of choledocholithiasis (group 1, n=117) and papillary stenosis (group 2, n=55) along with the combination of the above (group 3, n=6) in a total of 178 patients. Clinical and endoscopic results of the intervention were studied in all patients in the long-term (from 1 to 30 years).Results. Based on the analysis of the complex examination, including endoscopic functional tests, treatment results were evaluated as good in 80 % of patients, satisfactory in 16 % of patients and unsatisfactory in 4 % of patients (after endoscopic correction of papillary stenosis).Conclusion. The cicatrisation process in the papillotomy opening exhibits the most intensively in patients with papillary stenosis combined with choledocholithiasis and without it, lasts until 4.5–5 years after surgery. MATERIAL AND METHODS. The prospective randomized controlled research of 68 patients who underwent endoscopic sleeve gastroplasty. The 1st group (n=23) – neuromuscular blockade (NMB) was performed by bolus injection of rocuronium, the patients on the operating table were in a flat position. The 2nd group (n=23) – NMB was maintained by intravenous infusion of rocuronium, patients on the operating table were in the Trendelenburg position. The 3rd group (n=22) – basic anesthesia in combination with prolonged epidural analgesia (PEA), NMB were maintained at a deep level by intravenous infusion of rocuronium, patients on the operating table were in the «beach chair» position. Neuromuscular monitoring, monitoring of central and peripheral hemodynamics were performed. The elasticity of the anterior abdominal wall (E), intra-abdominal pressure (IAP), intra-abdominal volume (IAV) were calculated. RESULTS. During the analysis of the dynamics of the average level of intra-abdominal volume and pressure, it was found that the mean IAV level in the 3rd group was significantly higher at all stages of the study than in the 1st and the 2nd groups, and the IAP level was significantly lower in the 3rd group at all stages of the study than in the 1st and the 2nd groups. CONCLUSION.
