Elevated hemoglobin A1c percentage of more than 7% will result in high production of advanced glycation end-products. Th e elevated level of advanced glycation end-products in diabetic patients means a high risk for diabetic complications. Th e primary endpoint was to evaluate the hemoglobin A1c levels among diabetic patients and its effect on the prognosis of this metabolic disease in all regions inside Saudi Arabia. \u0000Methods: Th is was a cross-sectional observational study conducted between March and August 2018. Th e study was done by using a questionnaire containing nine questions which planned to involve at least 390 diabetic patients. \u0000Results: Six hundred and ninety-four diabetic patients from Saudi Arabia had answered the nine-question questionnaire about hemoglobin A1c percentage and prognosis of diabetes. Th e mean age of these patients is 43.4 years old and most of those patients (75.5%) were visiting governmental hospitals. Th e number of diabetic patients having hemoglobin A1c less than 7% are 259 patients with hemoglobin A1c mean 6.66% + 3.33%, however, there are 435 patients having hemoglobin A1c more than 7% with mean equals 7.75% + 1.2%, the difference between them is statistically significant (P value < 0.0001). \u0000Conclusion: Th e glycated hemoglobin of diabetic patients in all regions of Saudi Arabia was significantly elevated and uncontrolled based on most diabetic guidelines and significantly affects the prognosis by causing diabetic complications especially cardiovascular diseases.","PeriodicalId":280480,"journal":{"name":"Saudi Journal of Internal Medicine","volume":"119 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2019-06-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"116393460","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Sanosi","doi":"10.32790/sjim.2019.9.1.5","DOIUrl":"https://doi.org/10.32790/sjim.2019.9.1.5","url":null,"abstract":"Background: Unplanned hospital readmission is concerning health care providers and is frequently used as a quality indicator for hospital care. \u0000Objectives: This study was conducted in order to examine the rate and associated factors for 30-day readmission in internal medicine units at King Abdulaziz University Hospital, Jeddah. \u0000Methods: We retrospectively collected data for patients who had readmission within 30 days of hospital discharge from January 2010 to December 2013. We obtained the following information for all patients: patient demographics, admitting medical service, diagnosis at the first admission, presence of hypoxia, sepsis, and hospital length of stay at the first admission. We then compared the data for patients with 30-day readmissions to those who did not have readmission within 30 days \u0000Results: Of the 3838 patients who had hospital admission within the study period, 678 (17.7%) had readmission within 30 days. The mean patient age was 52 years (SD, 21.2 years). Patients who had readmission within 30 days were more likely to be female; older than 65 years of age; diabetic; hypertensive; bed ridden; and to have a history of stroke, bed sores and/or sepsis (all P values ≤0.001, except for stroke, P=0.003). \u0000Conclusion: Our study showed that about 18% of patients had readmission within 30 days. Niazi","doi":"10.32790/sjim.2019.9.1.6","DOIUrl":"https://doi.org/10.32790/sjim.2019.9.1.6","url":null,"abstract":"Background: Since conduction abnormalities represent a major cause of sudden death and cardiovascular complications, with a high incidence of increased length of hospital stay, risk of disabilities and healthcare costs, the aim was to discern the influence of conduction abnormalities at the time of admission on the clinical outcomes of patients admitted at the Coronary Care Unit in King Abdulaziz University Hospital. \u0000Method: The study adopted a retrospective-record review, episode-based design for a period of 2 years. All patients with sustained conduction abnormalities at the time of admission that had either self-terminated or required intervention were recruited. Statistical analysis was conducted by SPSS. \u0000Results:  A total of 1763 admissions occurred during the study period, conduction abnormalities represented 11.5% of all these cases. The most common types were left bundle branch block and atrial fibrillation. Types that were significantly associated with the length of stay were atrial fibrillation (p = 0.045), ventricular tachycardia (p = 0.020), and ventricular fibrillation (p = 0.007) and complete heart block (p = 0.004). Right bundle branch block was the only conduction abnormality that was significantly associated with higher mortality (p = 0.011). \u0000Conclusion:  Patients with right bundle branch block need close monitoring and optimal care to reduce risks of increased hospital stay and death. Waterpipe smoking produces toxins similar to those in cigarette smoke and is associated with substantial health risks. Th e aim of the study was to estimate the prevalence of waterpipe smoking among medical students, to evaluate their attitudes, motivations and knowledge about the hazardous effects. \u0000Methods:  This cross-sectional study of King Abdulaziz University Health Science students required a sample size of 700 participants. Th e participants were selected using the multi-stage stratified random sampling technique, and a validated self-administered questionnaire in English was used. Th e study was conducted from 24 April 2016 to 15 May 2016. \u0000Results:  Seven hundred twenty-one participants (325 (45.1%) males and 396 (54.9%) females) were selected for analysis. The prevalence of waterpipe smoking was 16.1%. The most common motives for smoking were socialization with friends, relaxation and taste, and most of the students were aware of the effects of waterpipe smoking on health. \u0000Conclusion:  There is a high prevalence of waterpipe smoking among medical students. One NMRI male mouse was caged with the female mice CD1 for natural mating. Th e female mice were flushed of the two-cell embryos from the oviduct after 24 hours. Th e mating was confirmed by the presence of a plug in the vagina (whitish gelatine substance in the vagina) next day. Th e osmolarity values were measured by advanced micro-osmometer. One-way ANOVA test was used to compare between different values of osmolarity. \u0000Results: The findings indicated that the increased osmotic pressure can significantly affect the rate of development. The developmental rate of two-cell mouse embryos was significantly decreased when cultured in 310 milliosmole and significantly increased in 280 milliosmoles as compared to the 260 milliosmole control group (9.52 ± 16.49, P < 0.001; 83.3 ± 28.7 to 76.29 ± 31.92, P < 0.001). \u0000Conclusion: The study has concluded that the osmotic pressure can affect the rate of development and it depends on the quality of environment. Dozens of articles addressing lupus complications in Saudi patients were published in recent years; to our knowledge, this is the first review focusing on pulmonary complications. In this narrative literature review, we conducted a systematic search of MEDLINE and Google Scholar using the words pulmonary complications of systemic lupus in Saudi Arabia. Th e search covered the period between 2000 and 2019. We found that the rate of pulmonary complications in Saudi lupus patients was consistent with that in global patients; exceptionally higher incidence of acute lupus pneumonitis was reported in Saudi patients. It is well recognized as a major risk factor for multiple medical conditions and poor quality of life. It has a great burden on health care resources and a substantial increase in health care use costs. Unfortunately, despite the prevalence, it is widely recognized that it remains under-diagnosed. \u0000Effective treatment needs a comprehensive management plan approach that considers the patient as the centerpiece of management. Regular monitoring and a detailed follow up can have an impact on adherence to therapy. Around 80% of all stroke cases are classed as ischemic stroke, which affects both gray and white matter brain regions. During ischemia, reactive oxygen species are generated and initiate multiple damaging processes such as lipid peroxidation, mitochondrial dysfunction, blood–brain barrier damage and brain edema. Melatonin is a potent antioxidant which can act as a direct oxygen species scavenger. Melatonin can also act at receptors (melatonin receptor 1 and 2), which are expressed in the nervous system cells including astrocytes. \u0000Methods: Using live cell imaging of transgenic mice, the viability of astrocytes in the corpus callosum of adult brain sections was monitored during a standard 60 minutes period of modeled ischemia (oxygen-glucose deprivation) and 30 minutes of reperfusion in 8 groups; Control, artificial cerebrospinal fluid + Melatonin, oxygen-glucose deprivation, oxygen-glucose deprivation + Melatonin, oxygen-glucose deprivation + Melatonin + Luzindole (a nonselective melatonin receptor antagonist), oxygen-glucose deprivation + Luzindole, oxygen-glucose deprivation + Melatonin + propionamidotetralin (a selective melatonin receptor 2 antagonist), and oxygen-glucose deprivation + propionamidotetralin. \u0000Results: Addition of melatonin significantly reduced the level of astrocyte death during oxygen-glucose deprivation from 71.94% ± 1.45 to 37.84% ± 1.9; p < 0.0001. This protective effect was blocked by luzindole or propionamidotetralin. Thirty-five salons were randomly selected of the 785 salons in Jeddah, Saudi Arabia. A total of 160 employees were interviewed using the Chicago Community Asthma Survey to assess their knowledge about asthma and their behaviors in response to chemicals in the workplace. \u0000Results: The participants’ mean age was 34 years; 28% of participants were hairdressers. Moreover, 30% of employees only possessed adequate knowledge about asthma. Education level (p = 0.044), Saudi nationality (p = 0.003), and a family history of asthma (p = 0.008) were significantly related to knowledge level. Further, a significant gap was noted between employees’ knowledge of the benefits of masks and gloves and the practice of wearing these devices while working. \u0000Conclusions: Asthma knowledge among most salon employees was inadequate, and a significant gap was found between employees’ knowledge and practice. Pre-employment education programs about asthma are recommended and may enhance the knowledge levels among this group of workers. In addition, future research directed to further understanding the gap between employees’ knowledge levels of the benefits of masks and gloves, and their actual implementation at work, is needed.
