Despite the existence of many therapies, the management of pain remains unsatisfactory globally. Medicinal plants have been used since medieval times and are still being used today for treating some ailments. Desmodiumadscendens is used traditionally for the treatment of epilepsy, pain, and inflammatory conditions. However, data on its effect on pain is very scanty. \u0000Objective: This study sought to investigate the antinociceptive effect of an ethanolic extract of D.adscendensin rodents.\u0000Methods: The pulverized whole plant material of D.adscendens was extracted by cold maceration with 70% ethanol. Chemical, thermal, and neuropathic pain were induced in rodents. The possible mechanisms of analgesia of the extract were also investigated.\u0000Results: The extract of D.adscendens (DAE) attenuated acetic acid-induced writhing (p=0.0012), ameliorated formalin-induced nociceptive pain in both the first (p =0.0058) and second phases (p = 0.0116), increased the percent maximal possible effect (%MPE) in the hot plate test (p <0.0001) and significantly reduced paclitaxel-induced neuropathic pain in both thermal hyperalgesia (p <0.0001) and cold allodynia (p =0.0024). The analgesic effect exhibited by DAE was significantly reversed in the presence of naloxone, glibenclamide, ondansetron, prazosin, and yohimbine. However, the analgesic effect of DAE was not significantly affected by theophylline, atropine, L-Nitro-arginine methyl ester(L-NAME),and nifedipine. \u0000Conclusion: The ethanolic extract of D.adscendens inhibited chemical, thermal, and paclitaxel-induced neuropathic nociception. Ahuru, Ada M Anyiwe, CHIMEZIE IGWEGBE NZOPUTAM","doi":"10.46829/HSIJOURNAL.2020.12.1.2.105-113","DOIUrl":"https://doi.org/10.46829/HSIJOURNAL.2020.12.1.2.105-113","url":null,"abstract":"Background: In Nigeria, there is a dearth of evidence on predictors of maternal care utilization in primary healthcare centres (PHCs).\u0000Objective: In this study, we examined the predictors of antenatal care (ANC), delivery care, and postnatal care (PNC) utilization in PHCs in four rural communities in Ughelli North Local Government Area in Delta State, Southern Nigeria.\u0000Methods: We performed a cross-sectional household survey which used a pretested structured questionnaire to elicit information from 380 women within the reproductive ages in their homes. Descriptive and predictive analyses were undertaken to assess the prevalence of ANC, delivery care, and PNC in PHCs. The odds for utilizing the three indicators were estimated using multivariate logistic regression. \u0000Results: The data revealed that 72.96% (n = 224/380) of the women utilized ANC in PHCs. Among women who reported recent birth, 56.34% (n = 191/339) delivered in PHCs, 4.72% (n = 87/339) in other government hospitals, 13.27% (n = 445/339) in private hospitals and 25.66% (n =87/339) delivered either at home or in the homes of Traditional Birth Attendants. Also, 64.24% (n = 106/339) of women who reported recent birth use PHC for PNC. Women who required to walk for 30 -59 min and ≥ 60 min to the nearest healthcare centre were, respectively, 56% [Odds ratio (OR) = 0.44; 95% confidence interval (CI): 0. 18 -1.04] and (OR = 0.37, 95% CI: 0.19 -0.72) significantly less likely to use PHC for ANC. Those who had primary educational qualification (OR = 0.36; 95% CI: 0.19 –9.76) and at least secondary educational qualifications (OR = 0.43;95% CI: 0.20-1.18) were respectively 64% and 57% significantly less likely to use PHC for delivery care. Attending ANC in PHCs significantly increases the chances of using PHC for both delivery care (OR = 10.52;95% CI: 5.94 -18.61), and PNC (OR = 2.11; 95% CI: 0.99-4.53). Also, using PHC for delivery care (OR = 11.72; 95% CI: 5.14-26.69) is associated with 1,072% increase in the odds to use PHCs for PNC.\u0000Conclusion: The study concluded that the rate at which women in the study area deliver at home is high despite proximity to PHCs. Also, the use of PHCs for a lower level of care influences its usage for a higher level of care. Early identification and interventions yield a better outcome. Yet, there is a gap in knowledge about the range of NDD that exists in Ghana among laypersons, health, and education professionals.\u0000Objective: This study examined the clinical presentations and patterns of NDD in a clinical psychological clinic of a quasi-governmental hospital in Ghana. \u0000Methods: Using a retrospective chart review design, secondary data were extracted from eligible psychological reports of children managed at the clinic from January 2012 to December 2018. Results: Most children(62.96%, n = 85/135)were aged 6-13 yr. at the time of establishing a diagnosis. They had been referred from private schools (49.62%, n = 67/135) because they were doing poorly academically (38.51%, n = 52/135) or exhibited some behavioural problems (22.22%, n = 30/135). Most of them had been born at term by spontaneous delivery (58.51%, n= 79/135) but went through traumatic delivery and suffered perinatal complications (32.6%). There was a significant association between birth asphyxia and labour complications (p <0.0001), gestational age (p< 0.008), mode of delivery (p < 0.016), and cerebral palsy (p< 0.018). Almost forty percent had comorbid neurodevelopmental disorders. \u0000Conclusion: Findings from this study have implications in the prevention and early identification of NDDs. Ahorlu","doi":"10.46829/HSIJOURNAL.2020.12.1.2.121-127","DOIUrl":"https://doi.org/10.46829/HSIJOURNAL.2020.12.1.2.121-127","url":null,"abstract":"Background: Complementary and alternative medicine (CAM) use is on the increase globally and there is an increased rate of usage among hypertensive patients.\u0000Objective: This study was designed to describe CAM use among hypertensive patients at a municipal hospital. \u0000Methods: A cross-sectional survey of hypertensive patients at a Municipal Assembly Hospital was conducted. Respondents were made up of 209 (99 CAM users and 110 non-Cam users) selected randomly from patients attending the hypertension clinic. The pattern of CAM use, clinical profiles of respondents, and their attitudes toward CAM were examined. Descriptive statistics and the Chi Square test were used for data analysis. \u0000Results: Out of the 209 patients interviewed, 99 (47.36%) had used at least one type of CAM mostly herbal products (72.72%, n =72).The CAM users demonstrated poorer adherence to antihypertensive medications (30.30%, n =30) as compared to non-CAM users (8.08%, n = 80). We observed a positive association between non-CAM use and medication adherence (p = 0.001). The majority (72.72%, n = 72) of the CAM users claimed to have obtained the expected effect, however,89.89% (n = 89) did not disclose their CAM use to their physician. They cited lack of inquiry by physicians as the main reason for non-disclosure. \u0000Conclusion: A considerable proportion of hypertensive patients receiving conventional treatment at a municipal hospital also use CAM therapies. Coronavirus protective measures may sound simple to adhere to compared to chronic disease-related behavioural practices, but this is not the case. Understanding adherence to COVID-19-related health behaviours using the health belief model as an explanatory model, can help to design interventions for change and maintenance of this change. The health belief model posits that, practicing a health behaviour depends on the individual’s perceived susceptibility to a disease, his/her perceived severity of the disease, and the perceived benefits/cost of engaging in the health behaviour, all of which are prompted by internal and external cues to action. Some Ghanaians may believe they are not susceptible to the disease or may downplay the severity of the disease, and thus will be less likely to engage in the health behaviours to avoid COVID-19 infection. Again, if Ghanaians believe the cost of engaging in these behaviours outweigh the health benefits of adhering to the behaviours, then they are less likely to follow these health behaviours. Finally, internal (e.g. experiencing COVID-19 symptoms) and external (e.g. daily COVID-19 updates) cues may act as triggers to engaging in health behaviours. Ghanaians should be well educated about the coronavirus and well informed, to be equipped to adhere to the COVID-19 protective measures. Profiling the prevalence of kinesiophobia and its association with demographic variables could become useful during the rehabilitation process.\u0000Objective: The study aimed to determine the prevalence of kinesiophobia and its correlation with lower limb functions among patients with LEF post-operatively.\u0000Methods: One-hundred and nine (n = 109) patients [male = 79 (72.50%)] with mean age of 41.83 ± 17.37 yr. participated in the cross-sectional study. Participants with LEF who had either undergone closed reduction and immobilization or open reduction with internal or external fixation were recruited into this study using convenience sampling during the rehabilitation phase at the orthopaedic wards of two tertiary healthcare facilities in Accra. Data collection was performed using the Tampa Scale of Kinesiophobia (TSK) and Lower Extremity Functional Scale. Data were analyzed using Spearman’s Rho correlation coefficient and Chi-square tests at p < 0.05.Results: Seventy-nine (72.50%) of the participants exhibited kinesiophobia based on their TSK scores (≥37). The TSK scores were significantly inversely correlated with participants’ lower limb function (r = -0.345; p < 0.001). Participants’ sex as well as the causes, types and sites of the fractures sustained had no significant association (p > 0.050) with the level of kinesiophobia. Conclusion: There was a high prevalence of kinesiophobia which negatively correlated with LEF but was not associated with the type, site, and cause of fracture as well as the participants’ sex. It is a life-threatening condition whose true incidence is unknown. The obstruction may be due to tracheal/laryngeal atresia, stenosis, or the presence of a mass lesion. Prognosis is poor, generally resulting in stillbirth or intrauterine fetal demise. Ex utero intrapartum treatment (EXIT) is possible if the condition is detected early. They are composed of blood vessels, adipose tissue, and smooth muscle in varying amounts. It is quite rare to find AML in a horseshoe kidney, although there is at least one such reported case discovered in pregnancy and managed by resection. Spontaneous hemorrhage which could be fatal is the most feared sequelae of AML. The first-line of management for AMLs was previously surgical excision. However, advances in minimally invasive techniques that have a lower risk of complications have broadened treatment options for reducing tumour size and preventing hemorrhage. One of such effective techniques is selective arterial embolization (SAE) of renal AMLs > 4cm. Patients not requiring any immediate intervention maybe followed up with active surveillance. Yawson","doi":"10.46829/10.46829/HSIJOURNAL.2020.12.1.2.93-104","DOIUrl":"https://doi.org/10.46829/10.46829/HSIJOURNAL.2020.12.1.2.93-104","url":null,"abstract":"Background: There is an established relationship between the concept of social support (SS) and depression among Type 2 diabetes mellitus (T2DM) patients, however, the magnitude, direction, and outcomes of the relationship are not well established in Ghana. \u0000Objective: This study was conducted to assess the factors influencing social support and further quantify its association with depression among Type 2 diabetic patients.\u0000Methods: Three hundred and seven adults diagnosed with T2DM were sampled from four public healthcare facilities in Ghana. Ten respondents from the sample were further selected for in-depth interviews using the convenience sampling technique. A modified negative binomial model was used to determine significant factors influencing SS scores, while the influence of SS on scores for depression was assessed using a modified generalized negative binomial model. All analyses were performed using Stata 15. The qualitative arm of the study was also analyzed using thematic inductive analyses.\u0000Results: Overall, the mean[ ±standard deviation (SD)]of SS scores among participants was 46.24(±20.57), while SS domains had mean(±SD) values of 17.69(±9.23), 11.57(±8.93),and 16.97(±9.88) respectively for family support, friends support and significant others. The SS equality of variance test indicated a significant mean difference by sex, educational background, and self-reported income differentials[F-test=6.46, 6.64,and 4.23 respectively; p<0.05]. A unit increase in SS score had a significant negative relationship with depression across all domains. For the focus model, social support significantly decreased the log count score by 0.002 {β[95%confidence interval (CI)]= -0.002(-0.002-0.000). A unit increase of SS significantly decreased the probability count of depression. Themes from the qualitative data also revealed that participants received SS from family and significant others, but not from friends. Support received from family included emotional support and financial aid.\u0000Conclusion: Among T2DM patients with depression, SS had a significant decremental association that could contribute to improved health outcomes. Sex, marital status, educational level, working status, and self-rated income level influenced the SS.
