In this paper, a telepathology system is discussed which costs less than $25,000 while supplying excellent diagnostic-functionality and network connectivity. The architecture proposed allows for dynamic interaction between pathologists that scales to the network bandwidth. Three important factors influence diagnostic accuracy in a telepathology system. First, preliminary data supports that dynamic systems perform better than static ones mainly because of better physician interaction and the ability to visualize the proper subset of diagnostic clues. Second, a frame rate below 8 frames/sec will not convey sufficient information for real-time interaction. Last, synchronization of the video between the 2 sites ensures that proper fields are transmitted. When considering telepathology systems, one must weigh the benefits, and the costs involved in the implementation. This paper addresses the general evolution of this environment through a specific examination of three image formats likely to be present: Digital Image Communication in Medicine (DICOM) Image Processing and Interchange (IPI)-Image Interchange Format (IIF) and the National Image Transmission Format (NITF). For each format, its approach to image format development and data encapsulation are both presented and compared to the other formats, and distinguishing capabilities and limitations are identified. A possible integration scheme for IIF and DICOM is presented and the relationship of NITF to IPI-IIF and DICOM is identified. Since a mixed image format environment could include acquired imagery, reference imagery material, or even document imagery, generalized interoperability will be a requirement of future systems. The programs integrate images, text, sound and even simulation to facilitate learning and comprehension. The programs were created jointly by the library and the School of Medicine with the Office of Medical Informatics. They operate on Macintosh computers, and where possible, they were developed with similar format to facilitate use. The programs include five educational projects in varying medical disciplines and a problem-based learning series of case studies. There was no statistical difference in the time spent setting up and reading hard copy vs. the setup and reading time for soft copy interpretation. Several early prototypes were developed but were neither practical nor economical. The PACS concept has undergone several major revisions. Recently, distributed digital archiving systems have emerged, and the progress towards all digital radiology departments is underway. One of the economic justifications for digital solutions has been to eliminate wet-processed silver films. In fact, many researchers have equated the PACS environment with a film-less radiology department in order to create an economic justification. This objective was emphasized to a degree that skewed the research direction away from the goals of establishing an effective and efficient radiology department. Film is a storage as well as a display medium, and the economics of replacing it with electronic devices can be challenged. However, the difficulties with controlling the quality of chemically processed film and the requirements of wet processing do not fit the modern environment of digital radiology. These issues are troublesome enough to cause some users to turn to less efficient display solutions. In fact, researchers have experimented with dry hardcopy systems, although these devices create hardcopy output inferior to silver film. This trend towards an all digital department will be made possible by advances in computer and communications technology. During the next hundred years, images will be acquired, stored, and displayed digitally. These images will be available to any authorized health care worker not only throughout a single hospital, but across large hospital networks. Computers will permit sophisticated image enhancement and computer assisted diagnosis will become routine. This tutorial provides an overview of the technologies currently utilized in large scale picture archiving and communication systems (PACS), an assessment of how successful today's technologies are at meeting the clinical and economic requirements for diagnostic imaging, and includes a discussion of future trends and expectations. It focuses on large scale, hospital wide, or multiple hospital PAC systems. Surgeons and OR staff were interviewed to assess the degree to which the PACS in the OR met their requirements and expectations. PACS was favored over film in the OR for the majority of those surveyed. Image quality was perceived as equivalent to or better than film. Image availability was cited as the major advantage over film. Areas for improvement in workstation design and PACS training were described and solutions offered. A filmless OR was preferable to a film environment for surgeons and staff. During four years of experience in archiving all clinical CT and MR images, the archive design has been improved to provide reliable and consistent operation. While the acquisition performance has been adequate, initial design deficiencies have limited retrieval performance. However, in this paper, a dynamically adaptive multi-disciplinary workstation (DAMDW) is discussed which is usable by a significant number of specialists and primary care physicians which not only adapts to the specialty of the physician, but also to the bandwidth available to the participating health care centers. The telemedicine system architecture described easily incorporates advances in computer and communication technology that minimizes early obsolescence. In addition, the system maximizes remote logistical support, providing significant operational cost reduction. The DAMDW is a based on the Unix operating system with software components which can be used on various computer architectures that provide cost alternatives during the system acquisition process.
