Methods: A prospective study was conducted on 25 eyes of 33 patients were diagnosed as advanced keratoconus. The study population underwent combined simultaneous femtosecond kerarings implantation and corneal collagen cross-linking. Results were reported at the 1 st , 3 rd , 6 th and 12 th months post-operatively Results: a significant improvement was found in postoperative (UCVA) (BCVA) and (SER) started form the first month postoperatively and continued all over the year (P < 0,001). The flattest k (K1) the steepest k (K2) and the average k (Ka) showed marked decrease after 3 month and became just stable in next 3 months and increase slightly thereafter (P=0.001). {"title":"CLINICAL CHARACTERISTICS AND DETERMINANTS OF MACULAR CHANGES IN RETINITIS PIGMENTOSA IN UPPER EGYPT","authors":"A. Settings and design: descriptive cross-sectional study was conducted at a university hospital from January 2018 until May 2021. Subjects and Methods: The study included 178 patients with RP. From the patient notes, baseline data was collected including; age, sex, complaint, age of onset, family history and history of medical diseases such as deafness, neurological disorder. The collected data included best corrected visual acuity BCVA, intraocular pressure IOP, corneal topography, and fundus examination by both direct and indirect ophthalmoscopy. Findings of perimetry and macular examination by a spectral domain optical coherence tomography device were also collected. Results: Out of the 178 patients diagnosed with RP and included in the current study; about 53% (n=95) were males and 47% (n=83) were females. The mean age was 28.5 ± 12.3 years. BCVA was found better than 612 in 15 (8.4%) patients and worse than 160 in 35 (19.7%) patients. Emmetropic refraction (within 0.75 D) was found in 45 (25.3%) patients, myopic refraction was found in 75 (42.1 %) patients. OCT findings included, retinal thinning which was found in 73 (41%), epiretinal membranes in 8 (4.5%), cystoid spaces in 15 (8.4 %) and normal macula in 45 (25.3 %) patients. Conclusion: RP has significant visual disabling effect on Upper Egyptian patients. Accelerated epithelium-off corneal cross-linking with pachymetric-guided corneal epithelial removal by (Avedro, USA) was done for the affected eye. Hafez","doi":"10.21608/EJCO.2021.189053","DOIUrl":"https://doi.org/10.21608/EJCO.2021.189053","url":null,"abstract":"Background: Cataract surgery with phacoemulsification is one of the most common surgical procedures performed today .Compared to people without diabetes, diabetic patients have been shown to have an elevated risk of developing cataracts. Cataract surgery in diabetics is indicated not only to improve visual acuity, but also to allow assessment and treatment of fundus changes. Even in the absence of diabetic retinopathy, diabetes mellitus has been found to have about twice the occurrence rate of pseudophakic CME. Objectives: This study was done to document any subsequent post-phacoemulsification macular edema (PPME) as postoperative sequelae in the diabetic versus non-diabetic nuclear senile cataract patients. Methods: This study was conducted on patients complained from visually significant immature senile cataract in ophthalmology department at Sohag university hospital between January 2018 and December 2020.Corrected distance visual acuity, Keratomerty, refraction, slit-lamp examination, macular optical coherence tomography (OCT) and biometry. Phacoemulsification was performed to all patients by stop and shop technique. The macular OCT changes were documented preoperatively and at postoperative month 1, 3, 6. Results: 64 eyes of 64 patients with visually significant senile cataract were included 32eye of them were diabetics, (33 males, and 31 females), our study documented four cases of the entire 64 eyes (6%) complicated with PPME in groups A and B at the postoperative month 1. Both A and B groups exhibited the same incidence of postoperative macular edema i.e. two (6%) cases in each group. Conclusion: Our outcomes confirmed that there was a similar probability of PPME occurrence in both non-diabetic and diabetic patients as this probability was mostly related to the surgical procedure itself. Methods: A prospective study was conducted on 100 eyes of 62 patients who had severe non-proliferative (SNPDR) or proliferative diabetic retinopathy (PDR) and underwent PRP treatment. The study population underwent 4.5×4.5 MM scans centered on the optic nerve head using Optical Coherence Tomography Angiography (OCTA) (RTVue XR Avanti Optovue, Fremont, CA) for measuring the peripapillary RNFL thickness before and after PRP by 2 months and 6 months. Results: Mean age of the study population was 49.85± 8.26 years (25-64 years). There was a significant increase in thickness of the nasal, temporal, superior and inferior peripapillary RNFL from baseline to 2 months post-PRP (all P value 0.05). A totalof 1857 participated in the internet-based questionnaire. Full ethical considerations are fully followed;Sohag university hospitals research committee protocol. Results: 46 percent of the responders wereusing telemedicine. Sense of satisfaction of physician and patient were 51.6% and 52.5%.Themajor obstacle for practicing telemedicine is patient awareness 54.4%, followed by technologyinfrastructure problems 44.6%. Conclusion: Telemedicine is a good method, especially in that era,many ophthalmologists actually use it, others will hasten their practice, and the limitations are emergencies, awareness between patients and some physicians. Patients and methods: this is a case control cross sectional study including 99 persons, 61 chronic cigarette smokers and another age matched 38 control nonsmokers. Parameters used in our study were cell density (CD), polymegethism, pleomorphism, the average of cell size, and central corneal thickness (CCT). Results: The mean values of the CD were 2829.9±334.9 compared to 3243.6±237.9, CV were 44.13±9.73 compared to 32.84±2.97, HEX were 40.44±6.95% compared to 60.11±5.83% for smokers and non-smoker groups respectively, with a highly statistically significant value (p <0.001) and on the other hand values were statistically non-significant regarding CCT. There were non-significant relation between the type of smoking (cigarette, shisha or combined).
