Pes planus and pes cavus are fairly common foot deformities, but it is uncommon to see both in the same patient simultaneously. This study aimed to describe the clinical presentation and prognosis of an uncommon combination of bilateral foot abnormalities named “windswept heels” resembling windswept knees. A 43-year-old female employee experienced frequent pain in her right foot and swelling and pain in her left foot, mainly when walking barefoot. She had a severe cavovarus deformity of the right foot with pressure callosities on the lateral side, clawing of the toes, and a positive Coleman’s block test. With the loss of the medial arch, severe heel valgus, and forefoot abduction, the left foot developed a planovalgus deformity. The left foot’s talus-first metatarsal angle and the right foot’s calcaneal inclination angle increased on weight-bearing radiographs. The left foot underwent staged surgical treatments, including medial sliding calcaneal osteotomy, gastrocnemius recession, and cotton osteotomy. For the right foot, lateral sliding calcaneal osteotomy, plantar fasciotomy, and peroneus longus tendon transfer. The American Orthopaedics Foot and Ankle Society Score (AOFAS) was used to measure the clinical and functional outcomes. At 1 year, her AOFAS scale was 85 compared to 59 before surgery. Since this presentation resembles the windswept knee deformity, we propose calling this disorder windswept heel deformity.","PeriodicalId":266222,"journal":{"name":"Journal of Musculoskeletal Surgery and Research","volume":"10 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-04-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"114614881","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

It is rare to have atraumatic lumbar pedicle fractures. However, it is reported mainly in predisposed patients, like those with contralateral spondylolysis, degenerative spondylolisthesis, previous spine surgery, stressful activities, and metabolic bone disease. We are reporting a rare case of a 37-year-old renal failure male patient who is an office worker who started dialysis only 1 month before the presentation. After a fall from the bed, the patient was found to have four levels pedicle fractures. We aimed to increase the awareness that lumbar pedicle fractures can coexist in patients with renal failure and increase the suspicion index that can promote early diagnosis and effective management.","PeriodicalId":266222,"journal":{"name":"Journal of Musculoskeletal Surgery and Research","volume":"27 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-04-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"121067145","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The correlation between the infectious organism(s) isolated from blood and synovial fluid remains unclear. Due to poor general and local conditions, despite a vascular repair, we performed a femoral fracture fixation followed by transfemoral amputation of the lower limb. The report aimed to demonstrate the effectiveness of this definitive treatment as one of the options in these difficult cases. The knee dislocation reduction was performed in the emergency operating room and the lower limb was stabilized with an external fixator. Vascular surgeons performed revascularization of the extremity with a femoropopliteal vascular bypass. Leg fasciotomies were performed and broad-spectrum antibiotics were administered. Several attempts at fasciotomy revisions and necrotic tissue debridement were made by plastic surgeons. Ten days after hospitalization, concomitant local rhabdomyolysis and deep soft-tissue infection lead to life-threatening sepsis. An above-the-knee amputation was quickly required. We performed an emergency proximal nailing of the femoral fracture and, subsequently, a transfemoral amputation at the distal third. In this case, we described the successful use of intramedullary nailing as a good option for stabilizing acute subtrochanteric fracture of a limb that required an above-the-knee amputation. This strategy saved the patient’s life, and intramedullary fixation with a short nail provided reliable bone stability for rapid and better functional recovery.","PeriodicalId":266222,"journal":{"name":"Journal of Musculoskeletal Surgery and Research","volume":"3 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-03-18","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"124270850","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

It does not present with the typical features of TB and can be confused with other pathologies affecting the heel. Very few case reports and case series studies are present to date for describing this rare entity. We report a 23-year-old female who presented to the orthopedic outpatient clinic with pain and swelling around the left heel and posterior aspect of the ankle for 3 months. Her radiograph showed a lytic lesion in the posterosuperior aspect of the calcaneum. Therefore, magnetic resonance imaging was done, which showed a lesion around the posterior-superior aspect of the calcaneum with the involvement of retrocalcaneal bursa. An incisional biopsy was performed, followed by en bloc excision of the lesion. The histopathological examination report suggested granuloma formation with caseous necrosis and the cartridge-based nucleic acid amplification test was also positive for mycobacterial TB infection. Proper clinical history, radiological examination, and biopsy are of utmost importance in these rare entities as they help in making the definitive diagnosis. Anti-Koch treatment is an effective treatment for TB, and the patient improved within 2 months of treatment both clinically (pain, swelling, and activity of daily routine) and radiologically (for bone formation and no recurrence). This report aimed to present a lytic lesion of the calcaneum that turned out to be TB and its management. The follow-up was 18 months post-surgery and showed no signs and symptoms of recurrence.","PeriodicalId":266222,"journal":{"name":"Journal of Musculoskeletal Surgery and Research","volume":"31 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-03-17","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"114651492","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
