Many of these fractures are surgically fixed with internal fixation. However, among the complications that can occur are peri-implant fractures, which are somewhat infrequent. They are due mainly to high-energy injuries. Therefore, not much information about them is found in the literature. Reusing implants is not a frequent or well-documented practice, and it is carried out only when the necessary resources are unavailable, particularly in developing countries. We are reporting a 16-year-old male patient who suffered a peri-implant fracture distal to plates that were placed 3 years earlier in both left forearm bones. The material was removed successfully and reused, achieving an adequate reduction and fixation with one dynamic compression plate (DCP) plate for the radius and one reconstruction plate for the ulna. The patient recovered successfully. This case report aimed to understand the lack of resources in a developing country and a possible solution for this problem in this type of scenario.","PeriodicalId":266222,"journal":{"name":"Journal of Musculoskeletal Surgery and Research","volume":"55 11","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-12-08","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"138588247","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

These types of tumors can present like malignant tumors like liposarcoma. Unlike superficial lipomas, deep lipomas (intramuscular/parosteal) require magnetic resonance imaging to evaluate for and rule out malignant features. We report this patient with giant and mixed types of rarest lipomas (intramuscular and parosteal) that arise from the humeral periosteum and within the brachioradialis muscle with encasement of the radial nerve. We aimed to increase awareness to think outside the box and shed light on this giant, rarest mixed type of intramuscular and parosteal lipomas. Furthermore, it emphasizes the need for careful investigations to rule out malignant tumors like liposarcoma. Finally, many orthopedic oncology centers designate large, deep lipomas as low-grade liposarcoma due to the risk of local recurrence.","PeriodicalId":266222,"journal":{"name":"Journal of Musculoskeletal Surgery and Research","volume":"45 7","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-12-02","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"138606502","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

It provides direct exposure to the acetabulum, adequate reduction, and access to treat associated hip fractures. However, there is no consensus on the positioning of the patient; prone or lateral. This review aimed to investigate the effect of patient positioning on the outcomes of the K-L approach in acetabular fracture fixation. Cochrane, Web of Science, Google Scholar, and PubMed were searched from inception until February 22, 2022. Inclusion criteria encompassed studies that investigated acetabular fracture fixation utilizing the K-L approach in relation to patient positioning (Prone and/or Lateral). Only English manuscripts that reported at least one of the outcomes of interest were included in the study. Studies that utilized both positions without specifying the outcomes in relation to the patient position were excluded from the study. The Newcastle–Ottawa Scale and the Joanna Briggs Institute tool were utilized to assess the quality of the included studies. This study included 521 patients from six articles. Of those patients, 46.1% were operated on in the lateral position and 53.9% in the prone position. The mean operative time of lateral and prone positions was 160.9 min (95% confidence intervals [CI]: 135.11–186.64) and 212.5 min (95% CI: 167.08–257.94), respectively. The mean blood loss in the lateral position was 502.mL (95% CI: 471.61–532.47) compared to 482.5 mL (95% CI: 396.77–568.28) in the prone position. The pooled complications in the lateral position were 29% (95% CI: 19–39%) compared to 37% (95% CI: 6–75%) in the prone position. The outcomes did not significantly differ when comparing lateral and prone patient positioning for posterior acetabular fracture fixation through the K-L approach. Further prospective randomized controlled studies are recommended to reach the best evidence.","PeriodicalId":266222,"journal":{"name":"Journal of Musculoskeletal Surgery and Research","volume":"20 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-08-25","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"114837522","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

This study aimed to analyze the complication using surgical interventions, including intramedullary telescopic rod fixation, Rush pin fixation, and Kirschner-wire (K-wire) fixation. This retrospective and cohort study was conducted in our institution, Riyadh, Saudi Arabia. A total of 23 patients with OI were treated from 2018 to 2022, out of which 19 were treated surgically. A total of 29 long bones were surgically managed. Our patients’ data were collected from the hospital’s electronic system. The overall incidence of Rush pin fixation post-operative complications was 57% (five out of seven cases) and K-wire fixation was 50% (one out of two cases). The main indications for intramedullary telescopic rod implantation were osseous deformities and fractures, with the overall incidence of post-operative complications reaching around 35% (seven out of 20 cases). In our population, the overall incidence of complications after intramedullary telescopic rod fixation was lower than those after Rush pin fixation and K-wire fixation. In addition, more studies are needed to add to the literature new ways of tackling post-operative complications following the insertion of intramedullary telescopic rods, such as nail migration and new fractures or refractures.","PeriodicalId":266222,"journal":{"name":"Journal of Musculoskeletal Surgery and Research","volume":"30 5 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-08-21","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"116906096","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
