Acharya, Kishor Kumar Tamrakar","doi":"10.54530/jcmc.1162","DOIUrl":"https://doi.org/10.54530/jcmc.1162","url":null,"abstract":"Background: Blunt abdominal trauma, one of the most common injuries caused by road traffic accident are usually not obvious hence often missed unless repeatedly looked for. In view of this, the study was aimed to diagnose the solid organ injury and its incidence, mode, age, sex distribution, time of presentation, clinical course, management, time interval between injury and surgery, postoperative complications and outcomes.\u0000Methods: This prospective observational study was conducted in the department of General surgery, Chitwan Medical College Teaching Hospital, from August 2020 to July 2021. Data were collected with clinical history/examination, injury assessment, appropriate investigations, modes and patterns. Documentation with identification, history, clinical findings, diagnosis, management, complications and outcomes were done and analyzed with SPSS version 16.\u0000Results: Among 83 patients, most of them aged between 16-30 years (57.8%). Male to female ratio was 1.96:1. The most common mode was road traffic accidents (55.5%). Commonly injured organ was spleen (51%) and liver (30%) with associated other organs (65%). Conservative management was done in 62% and operative in 38%. Majority of patients were operated after 4-6 hours of initial presentation. The more the time interval between injury and surgery, the more the complications, duration of hospital stay and mortality. Post-operative complications were noted in 38.5% and mortality in7.22%.\u0000Conclusions: Blunt abdominal trauma was common and mainly associated with road traffic accidents. The purpose of this study was to determine the extent of post-stroke depression and the factors that influence post-stroke depression among stroke survivors attending therapy centers at various hospitals in Chitwan.\u0000Methods: A hospital-based cross-sectional study using the face-to-face interview method was carried out in different therapy centers in Chitwan, Nepal. A total of 137 stroke survivors were considered as a sample. The data was collected from November 25, 2021 to January 29, 2021. We used Pearson’s Chi-square test and binary logistic regression analysis to assess the factors influencing post-stroke depression among stroke survivors in Chitwan.\u0000Results: Among 137 stroke survivors, just more than half i.e., 54.70% (75) suffered from any kind of depression, and the remaining participants i.e. 45.30% (62) did not have depression. Result showed that current depression status among stroke survivors were significantly associated with age group [above 60 years (COR =2.88; CI=1.17-7.10)], educational status [primary and below (COR =4.02; CI=1.47-10.95)], past employment status [unemployment (COR=2.79; CI=1.37-5.69)], past alcohol intake [alcohol intake (COR=1.04; CI=0.52-2.10)] and current BMI status [underweight (COR =1.23; CI=0.24-6.07)].\u0000Conclusions: More than half, or 54.70% (75), suffered from depression, which is an unacceptable rate. Increased literacy, improved socioeconomic status, and lifestyle changes among stroke survivors would reduce the severity of depression. Prevalence of Peutz-Jeghers syndrome is between 1 in 8300 and 1 in 280000 people. Here, we present a case of 18 years old lady who presented with complains of peri-umbilical pain and vomiting for 2 months. Physical examination revealed multiple pigmentations over lips, palms and soles. Ultrasonography was normal. On further evaluation using a contrast enhanced computerized tomographic scan of abdomen and pelvis, jejuno-jejunal intussusception was noted. She then underwent exploratory laparotomy along with resection and anastomosis of the involved jejunal segment. Serum uric acid is said to have a strong correlation with diabetes and might contribute to its risk. The present study aimed to compare the levels of serum uric acid in diabetic, pre-diabetic, and non-diabetic patients visiting a tertiary care center.\u0000Methods: This hospital-based cross-sectional study was conducted among 320 patients visiting medicine OPD of Universal College of Medical Sciences. Of them, 182 were diabetics, 48 were pre-diabetics, and 90 were non-diabetics. Serum uric acid, fasting blood glucose, post-prandial blood glucose, and glycated hemoglobin levels were measured. Kruskal-Wallis test, Chi-square test and Spearman’s correlation were performed for analysis. Finally, a multiple linear regression analysis was done to adjust for the confounding effects of various parameters. At a 95% confidence level, a p-value less than 0.05 was considered statistically significant.\u0000Results: Unadjusted serum uric acid levels were significantly different among non-diabetics, pre-diabetes and diabetes group. Serum uric acid levels also correlated positively but weakly with all the glycemic parameters (p < 0.001). However, after adjusting for the confounders like age, sex, diet, BMI, smoking, alcoholism, and hypertension, serum uric acid levels did not vary significantly among the study groups. There was no significant association of serum uric acid with glycemic parameters.\u0000Conclusions: Serum uric acid levels did not vary significantly among the diabetic, pre-diabetic, and non-diabetic individuals. {"title":"FUNCTIONAL OUTCOME OF MEDIAL GASTROCNEMIUS MUSCLE PEDICLE FLAP FOR SOFT TISSUE RECONSTRUCTION AROUND THE KNEE AND PROXIMAL LEG","authors":"S. Pandey, S. There are variable results in the literature about its outcome and such study is lacking in our country. The present study was undertaken to evaluate the outcome of the medial GMPF to reconstruct the defect around the knee and proximal 3rd leg.\u0000Methods: It was a retrospective study of 21 cases of medial GMPF reconstruction to cover the defect around the knee and proximal third leg over 2 years in tertiary care hospital. The outcome variables measured were flap and graft take up, fracture union, knee range of motion, and complications. Data was analyzed using SPSS software version 20.0 with descriptive statistics for categorical and discrete variables.\u0000Results: The mean age of the patient was 37.19 years (range 21-65). Out of a total of 21 cases, 20 (95.2%) were traumatic wound defects, whereas 1 case was complications of necrotizing fasciitis. Flap and graft take-up was good in all the cases. The mean time for union of tibia fracture was 24.57 weeks (range 16-36). The mean knee range of motion was 125 degrees (range 90-145). Hence in the present study we assessed the knowledge and practice about COVID-19 among health care workers involving in various aerosol generating procedures.\u0000Methods: We piloted a questionnaire based descriptive cross-sectional study at various aerosol generating departments of KIST Medical College and Teaching Hospital, Imadol, Nepal from June 2020 to December 2020 with IEC No: 076/077/42. All the health care workers at same departments were included. The self- administered questionnaire had two sections; knowledge and practice. The responses noted and analyzed using the 2 tailed multivariate regressions to estimate influencing factors.\u0000Results: We observed that the about one third of the respondents get information about COVID-19 mainly from TV and social media. In the multivariate model, source of information, type of health institution and knowledge of COVID-19 are strongly associated with preventive practice of health care workers. HCWs who had good knowledge on COVID-19 were 2 times more likely implement good preventive practice.\u0000Conclusions: Type of health facilities, level of education, training on COVID, work experience, type of source of information were significantly associated with knowledge, and practice of HCWs toward COVID-19. Various aerosol generating departments showed good knowledge and practice. Among all OMFS department showed good knowledge and practice as compared to other departments of Dentistry. The aim of the study was to determine the signs and etiologies of respiratory distress in patients under mechanical ventilation.\u0000Methods: A hospital-based cross-sectional study was done on adult patients on mechanical ventilation (MV) with Respiratory distress admitted at Chitwan Medical College, Nepal from January 2021 to June 2021.We studied various signs and etiologies of RD in different days on MV. Descriptive analysis was carried out using IBM SPSS version 20.\u0000Results: Total 66 patients were included in the study with mean ± SD of age was 55.3 ± 15.8 years with equal proportion of male and female. Out of total 154 occurrences of 11 signs in 66 patients, agitation was the most common one occurring 29.3% of times followed by use of accessory muscle in 15.9%. Most common cause of respiratory distress was interstitial syndrome of increased extra-vascular lung water or density on ultrasound was present more in patients on MV for ≥ 5 days. Most patients (74.2%) had multiple RD signs and 25.8% had only a single sign. Additional malformations are genitourinary abnormalities, gastrointestinal abnormalities, anomalies of the lumbosacral spine and pelvis and absence or poor development (agenesis) of one or both kidneys. It has incidence of 0.98 to 4.2 per 100,000 live births.
