Nouh, Maged Abdelaziz, Nabil Fathy Esmael Hassan","doi":"10.58675/2682-339x.1657","DOIUrl":"https://doi.org/10.58675/2682-339x.1657","url":null,"abstract":"Background : Cardiovascular alterations such as atherosclerosis, calci fi cation, heart failure, diastolic and systolic dysfunction, and stroke are the primary causes of mortality in people with chronic kidney disease (CKD) and end-stage renal disease (ESRD). Individuals with CKD have been demonstrated to have increased serum levels of visfatin. A new study suggests that high visfatin levels in individuals with CKD accurately predict cardiovascular abnormalities. Chronic renal failure, atherosclerosis, and calci fi cation in the carotid arteries vary in individuals. The researchers set out to look at the correlation of visfatin level and cardiovascular abnormalities in CKD patients. In this case e control comparative study, conventional echocardiogram and carotid duplex assessment were used to evaluate the differences between 40 patients with stage III or stage IV CKD receiving conservative treatment, 40 ESRD patients receiving regular hemodialysis, and 20 participants as a control group. Results : Hemodialysis-dependent carotid atherosclerosis, systolic and diastolic dysfunction, and calci fi cations are more common in ESRD patients compared with nonhemodialysis-dependent individuals that signi fi cantly correlated with high serum visfatin levels compared with healthy control participants. Carotid atherosclerosis, diastolic dysfunction, and systolic dysfunction are all more likely to occur in those with CKD who had increased carotid intima-media thickness ( P ¼ 0.036) of carotid Doppler, decreased EF % ( P ¼ 0.032), increased left ventricular mass index ( P ¼ 0.004), and reversed Early diastolic over late diastolic trans mitral fl ow (E/A) ratio ( P ¼ 0.003) demonstrated by echocardiographic and carotid Doppler assessment. Aim and objectives : To compare between COVID-19-associated stroke and non-COVID-19-associated stroke cases regarding risk factors, characteristics, and outcome. Patients and methods : In a prospective study, 297 patients diagnosed clinically and radiologically as having stroke and admitted to Emergency Department and stroke unit of Al-Azhar University hospitals were recruited in the period from beginning of December 2021 to the end of May 2022. They were classi fi ed into ischemic stroke (IS), cerebral venous thrombosis, subarachnoid hemorrhage, and intracerebral hemorrhage groups. Workup for COVID-19 was done in each group followed by comparison between both groups. Results : We evaluated 297 patients, and 46 (15.4%) had COVID-19 infection. In IS group, age, hypertension, and previous cerebrovascular stroke were signi fi cantly lower. Large vessel occlusion, median baseline National Institutes of Health Stroke Scale score, and serum ferritin were signi fi cantly higher in COVID-19-associated IS group. Moreover, COVID-19-associated IS had a worse outcome regarding median Modi fi ed Rankin scale, hemorrhagic transformation, and mortality rate. According to TOAST classi fi cation, cryptogenic subtype was signi fi cantly higher in COVID-19-associated IS group. Lymphocyte (%) was signi fi cantly lower but D-dimer was signi fi cantly higher in COVID-19-infected patients in all stroke groups. Non-Hodgkin lymphoma and Hodgkin disorder are two types of lymphoid neoplasms. Aim : To examine how positron emission tomography/computed tomography (PET/CT) may be used to diagnose nodal lymphoma, analyze the effectiveness of 18-FDG-PET/CT in the fi rst staging of nodal lymphoma, analyze the function of PET-CT in patient monitoring both during and after the therapeutic course, and examine the function of PET-CT in foretelling and spotting relapses. Patients and methods : A total of 30 patients (12 females and 18 males; 11 Hodgkin disorder and 19 non-Hodgkin disorder) with histologically con fi rmed malignant lymphoma were enrolled in this investigation. They were split into three groups judging by the outcomes of the PET/CT examination. Results : CT for diagnosis was compared with PET/CT results. The current study demonstrated the critical relevance of PET/CT in lymphoma staging, monitoring therapeutic response, and follow-up. The present investigation demonstrated the signi fi cance of 18F-FDG. When it comes to the early staging of lymphomas, PET/CT has shown to be a valuable tool in the treatment of lymphomas. PET/CT exhibited substantial effects on early and delayed therapy response evaluation. Conclusion : The outcomes of the present research highlighted the value of 18F-FDG PET/CT as a technique for the fi rst staging of lymphomas. It is particularly dif fi cult in patients with cancer, where accurate tumor staging depends on the de fi nition of the hepatic focal lesion for best treatment planning. Aim and objectives : Was to evaluate the signi fi cance of dynamic magnetic resonance in identi fi cation and description of hypovascular hepatic tumor. Patients and methods : Our current research study included 50 patients, comprising 34 men and 16 women, who had hepatic lesions that were detected using a variety of imaging modalities such as ultrasound, computed tomography scan, and dynamic and diffusion MRI. Results : On noncontrast MRI, 26 lesions were detected in our 21 patients. Different signal intensities were displayed by these lesions on noncontrast MRI; we found that hypointensity in T1 is the commonest signal, whereas, hyperintensity was the commonest in T2 weighed image. In this context, accurate preoperative biometric measurement is a crucial requirement. Aim : Comparing the refractive predictability of optical biometry versus applanation ultrasonic biometry in high axial myopic eyes undergoing cataract surgery. Patients and methods : A total of 50 eyes from 50 patients with axial lengths greater than 26.5 mm and just cataracts were analyzed in this study. In this study, patients were randomly assigned to one of the two groups. Twenty-fi ve patients were split evenly between the two groups, with group 1 using optical biometry and group 2 using applanation ultrasound. Results : Group 1 mean age was 53.8 ± 8.9 years as it was 51.8 ± 10.8 years in group 2. There were seven (28%) males and 18 (72%) females in group 1, while there were 11 (44%) males and 14 (56%) females in group 2. There was no statistically signi fi cant variation in axial length between groups ( P ¼ 0.101). There was a statistically signi fi cant increased K1 and K2 among group 1 when compared with group 2 ( P ¼ 0.003 and 0.001, respectively). There was statistically signi fi cant ( P ¼ 0.015) increased anterior chamber depth in group 1 when compared with group 2. Signi fi cant differences in postoperative refraction were seen across the groups ( P ¼ 0.001) (spherical equivalence values). The atonic PPH rates of developed nations are rising despite regular active control of the third stage of work. Primary PPH continues to be the greatest cause of maternal mortality in less developed nations. Aim and objectives: To assess the effectiveness of rectal misoprostol, oxytocin infusion, and oxytocin intravenous bolus and infusion in reducing blood loss during and after elective caesarean delivery. Patients and methods: This prospective research included 200 pregnant patients who were hospitalized to the labor ward at the Department of Obstetrics and Gynecology of Al-Hussein University Hospital. Result: There was a significant difference among included groups regarding additional uterotonic use intraoperatively, as it was used in 17, 12, 13, and 5 in groups 1, 2, 3, and 4, respectively. Ascending infection from the nasopharynx can affect the conductive part of the hearing pathway, which leads to middle ear (ME) effusion. Otitis media may be the fi rst presentation of severe acute respiratory syndrome coronavirus-2 infection. Objective : To evaluate ME function in recovered severe acute respiratory syndrome coronavirus-2-infected patients, and also to compare between the complaint of the patient and the test results. Patients and methods : A case e control study was performed in which the cases and controls groups were age and sex matched, with ages ranging from 18 to 50 years. The ME function of both groups was assessed using tympanometry and the Eustachian tube function test. Results : Tympanometry showed a statistically signi fi cant difference in the existence of abnormal tympanometry (type C or As) and Eustachian tube dysfunction in both ears between patients and controls. According to the comparison, there is a statistically signi fi cant difference in both ears between the Eustachian tube function and the patient ' s complaints of ear fullness or a sense of blockage.
