Павлова","doi":"10.34922/ae.2022.35.5.006","DOIUrl":"https://doi.org/10.34922/ae.2022.35.5.006","url":null,"abstract":"Исследование заключается в проведении клинико-лучевых параллелей между тяжестью функциональной патологии височно-нижнечелюстного сустава (ВНЧС), плотностью костной ткани элементов сустава и влиянием на него биорегулирующей терапии в различном возрасте. Под наблюдением находились пациенты мужского пола 35-74 лет с функциональной патологией ВНЧС. Исследование показало достоверное снижение плотности костной ткани и корреляцию этого показателя с тяжестью течения заболевания у всех пациентов, имеющих дисфункцию ВНЧС, по сравнению с контрольной группой здоровых лиц. После курса биорегулирующей терапии показатели плотности костной ткани и индекса функциональной патологии ВНЧС свидетельствовали об изменении тенденции с тяжелого течения на форму средней тяжести в группе пациентов пожилого возраста. Выявлена корреляция тяжести течения функциональной патологии и снижения плотности костных структур ВНЧС, которая усиливается с возрастом. Использование в комплексном лечении биорегулирующей терапии оказывает благоприятное воздействие на течение функциональной патологии ВНЧС у пожилых пациентов.\u0000 The study consists of clinical and radiological parallels between the severity of functional pathology of the temporomandibular joint, bone density of the joint elements and the effect of bioregulatory therapy on it at different ages. Male patients (35-74 years old) with functional pathology of the temporomandibular joint were under observation. The study showed a significant decrease in bone density and correlation of this index with the severity of the disease course in all patients with temporomandibular joint dysfunction compared with the control group of healthy subjects. After a course of bioregulatory therapy the bone density and functional pathology index of the temporomandibular joint indicate a change from the severe course to a form of moderate severity in the group of elderly patients. The correlation correlation between the severity of the course of functional pathology and the decrease in the density of bone structures of the temporomandibular joint has been revealed, increasing with age. Освещены последние данные о роли различных факторов риска развития синдрома старческой астении как вероятных причин неравенства проявлений «хрупкости» у мужчин и женщин с различными сердечно-сосудистыми заболеваниями. Проанализированы данные о гендерном неравенстве в частоте выявления синдрома старческой астении и связанного с ним парадокса между уровнем здоровья и показателями смертности. Выявлены причины, лежащие в основе противоречивых результатов, полученных в ходе некоторых исследований. Для поиска литературы использовали систему PubMed.\u0000 The review describes sexual and gender differences as key factors affecting the pathological aging processes. The latest data presenting risk factors as possible causes of inequality in frailty in men and women with cardiovascular diseases are given. Data on gender inequality in detection of frailty and the associated paradox between health status and mortality rates are analyzed. The reasons for the contradictory results of previous studies have been identified. Многие клиницисты выступают за более широкое внедрение немедленного имплантационного протезирования у пожилых в силу скорейшего восстановления эстетических черт и функционального состояния. Цель работы - изучение результативности имплантационного протезирования у пожилых пациентов протяженными замещающими конструкциями с малым числом искусственных опор. Пациентам проводили имплантационное протезирование на нижней челюсти протяженной металлополимерной конструкцией «Трефойл» с префабрикованным стандартным каркасом и тремя искусственными опорами. Общий показатель сохраняемости имплантатов составил 96%. Кумулятивный уровень выживаемости пациентов составил 93%. Значение кумулятивного уровня сохраняемости протезов составило 98,3%. Высокие показатели сохраняемости имплантатов и протезов, хорошее состояние периимплантатной слизистой оболочки и кости спустя 3 года после протезирования, а также достигнутый значимо более высокий уровень их качества жизни, наряду с высокой медико-экономической эффективностью способа, позволяют считать его перспективным стандартом геронтостоматологии.\u0000 The use of implant prosthetics in the elderly and senile patients is highly relevant. Many clinicians advocate a wider introduction of immediate implant restorations in the elderly due to the fastest restoration of aesthetics and function. The purpose of the work is to study the efficacy of implant prosthetics in elderly patients with extended replacement structures with a small number of artificial supports. The patients underwent implant restoration of the mandible with an extended metal-polymer structure «Trefoil» with a prefabricated standard frame and three artificial supports. The overall implant survival rate was 96%. The cumulative survival rate of patients was 93%. The cumulative survival level of prostheses was 98,3%. В 1-й группе пожилых (28 пациентов) для коррекции вестибулярной дисфункции использовали вестибулярную гимнастику, во 2-й (28 пациентов) нефармакологическим методом воздействия послужила кетогенная диета, контрольную группу составили 28 пациентов, не получавших немедикаментозную коррекцию. Исследование показало, что вестибулярная гимнастика для коррекции вестибулярной дисфункции сопоставима по эффективности с кетогенной диетой у пожилых пациентов с данной патологией.\u0000 The purpose of the study was to compare the effectiveness of non-drug treatment methods (vestibular rehabilitation, ketogenic diet) for leveling vestibular dysfunction in elderly patients with vestibular migraine. The study included elderly patients with vestibular migraine. In the first group (28 patients), vestibular gymnastics was used to correct vestibular dysfunction, in the second group (28 patients), a ketogenic diet served as a non-pharmacological method of exposure and a control group (28 patients) who did not receive non-drug correction measures. Оценка вовлеченности в туристские практики старшего поколения Ростовской обл. на общероссийском фоне выглядит оптимистичной, несмотря на существование значительной группы людей, не совершавших турпоездок более 5 лет, и снижение туристской активности старшего поколения на фоне пандемии. Наиболее активно путешествуют женщины, жители городов, пожилые люди 60-70 лет и респонденты с высшим образованием. Показано, что туристские практики старшего поколения в силу сочетания объективных и субъективных факторов широко дифференцированы. У старшего поколения Ростовской обл. преобладают купально-пляжные, оздоровительные и экскурсионно-познавательные туристские практики в пределах регионов Юга России. Внутрироссийские практики, как правило, в целях экономии организуются туристами самостоятельно, а выезды за рубеж - с обращением в туристическое агентство. Наблюдается склонность к заблаговременному планированию турпоездок и повторным практикам.\u0000 Based on the materials of a quantitative and qualitative sociological survey conducted in 2022 in the Rostov region, the article analyzes the involvement of the older generation in tourist practices and their content characteristics. The assessment of the involvement of the older generation of the Rostov region against the all-Russian background looks quite optimistic, despite the existence of a significant group of people who have not made a tourist trip for more than 5 years, and a decrease in the tourist activity of the older generation amid the pandemic. The most active travelers are women, urban residents, elderly people 60-70 years old and respondents with higher education. It is shown that the tourist practices of the older generation are widely differentiated due to a combination of objective and subjective factors. The older generation of the Rostov region is dominated by bathing and beach, health and sightseeing tourist practices within the regions of Southern Russia. Domestic practices, as a rule, are organized by tourists independently in order to save money, and trips abroad - with an appeal to a travel agency. Противоэпилептические препараты могут оказывать обезболивающее действие при различных видах хронической боли (преимущественно при невропатической). Различные противоэпилептические средства, используемые в качестве анальгетиков, обладают вариабельной эффективностью и безопасностью у пожилых людей. В обзоре рассматриваются «старые» противоэпилептические средства (фенобарбитал, клоназепам, этосуксимид, карбамазепин, фенитоин, вальпроат и другие) и «новые» (габапентин, прегабалин, окскарбазепин, ламотриджин, леветирацетам, топирамат, зонисамид, тиагабин) с учетом особенностей их фармакокинетики у пожилых пациентов, профиля безопасности и параметров анальгетической эффективности.\u0000 High prevalence of chronic pain in elderly results in active search of new approaches for the effective and safe management of this category of patients. Antiepileptic drugs may provide analgesic affect in different types of chronic pain (mainly, in neuropathic pain). Пожилой человек рассматривается как активный агент, который может влиять на собственное здоровье. На материалах 25 глубинных интервью проанализировано четыре тематических блока, связанных со здоровьем. Результаты показывают, что современного пожилого горожанина вполне можно назвать активным агентом в отношении собственного здоровья. В качестве основных способов поддержания/улучшения здоровья информанты отметили физическую активность и сбалансированное питание. Гораздо реже (в единичных случаях) упоминались поддержание психического и эмоционального благополучия, профессиональная и интеллектуальная активность. При этом представления о способах поддержания здоровья, как правило, влияют на регулярные практики участников исследования. Репертуар практик достаточно широк (разного рода физическая активность и ограничения в питании, стремление его сбалансировать). Наряду с этим, медицина практически не рассматривается информантами как инструмент профилактирования заболеваний и поддержания здоровья, скорее, это способ решать уже возникшие проблемы.\u0000 The main purpose of the study: to analyze the ideas of older people about ways to influence their own health and their health promoting behavior. The study considers the elderly as an active agent that can influence their own health. Based on the materials of 25 in-depth interviews, 4 thematic blocks related to health were analyzed. It is confirmed that the modern elderly city dweller can be called an active agent in relation to his own health. Informants noted physical activity and a balanced diet as the main ways to maintain/improve health. Much less often (in isolated cases) maintenance of mental and emotional well-being, and professional and intellectual activity were named. At the same time, ideas about how to influence health correlate with the daily practices of the older people. The repertoire of practices is quite wide (all kinds of physical activity and dietary restrictions, the desire to balance it). Во многих странах люди, выходя на пенсию, перемещаются «вниз по социальной лестнице», теряя как приобретенный статус, так и уровень доходов. Однако в XXI в. появился новый вид неравенства - цифровое. Его отправной точкой является низкая цифровая грамотность пожилых людей, хотя в настоящее время ситуация не кажется столь драматичной, как 12-15 лет назад. В Петербурге обучение пожилых ведется с начала 2000-х гг., как НКО, так и КЦСОН, оно доступно и в большинстве случаев бесплатно, но, как показали наши исследования, значительная часть пожилых цифровых навыков до сих пор не имеет. В целом в России, по последним данным Росстата, в 2020 г. активных пользователей интернета старше 60 лет было всего 13,5%. Отсутствие интеграции в цифровое пространство стало рассматриваться как свидетельство уязвимости пожилых, особенно во время пандемии CODID-19 и после. В статье представлены результаты исследования, включающего онлайн-опрос жителей Петербурга об использовании городских электронных сервисов, а также дополненного интервью с представителями организаций, проводящих обучение пожилых информационно-коммуникативно-технологическим навыкам, и пилотным опросом использования пожилыми цифровых устройств - голосовых ассистентов. В результате были получены данные о востребованности электронных каналов коммуникации и электронных городских сервисов, а также «барьерах доступа» к овладению цифровыми навыками с точки зрения преподавателей курсов для пожилых. В заключение делается вывод о связи между овладением цифровыми навыками и включением пожилых в коммуникационное пространство.\u0000 In recent decades, the world’s population has been aging due to the fact that life expectancy is increasing, and the birth rate in developed countries is falling below the level of simple reproduction. In many countries, people retire and move «down the social ladder», losing both their acquired status and income level. However, in the XXI century, a new kind of inequality has appeared - digital. Its starting point is the low digital literacy of the elderly, although at present the situation does not seem as dramatic as 12-15 years ago. In St. Petersburg, training for the elderly has been conducted since the beginning of the 2000s, both NGOs and CCSS, it is available and in the most cases, free of charge, but, as our research highlighted in this article has shown, a significant part of the elderly still do not have digital skills. In general, in Russia, according to Rosstat, in 2020 there were only 13.5% active Internet users over the age of 60. Задача статьи - показать связь «старения на месте» с другими теоретическими подходами, а именно заботой в сообществе (community care), деинституционализацией обслуживания и ухода за пожилыми, и особенно с социальным исключением пожилых. В таком понимании нас убеждают действия европейских стран по развитию местных сообществ и популярность идеи «aging-in-place», поддержанные в Мадридском международном плане действий по проблемам старения. Цель политики во всех странах в отношении пожилых одна - обеспечить пожилым возможность оставаться в своих домах как можно дольше, то есть стареть по месту жительства [10]. Необходимо также уточнить применимость данного подхода в российских условиях. Метод статьи - теоретический анализ. Вывод: в России из-за огромных территорий, слабо развитых коммуникаций и большого числа мелких населенных пунктов, жизнь и старение там приводит к сужению возможностей получения помощи и социальных контактов, что не может оцениваться как соответствующее подходу «старение на месте».\u0000 In the article, we analyze a theoretical approach to the organization of the life of older people, which is called «aging-in-place» (in the English literature). In recent years, this approach, developed in the UK and positively received in Western Europe, has attracted some interest among Russian researchers as well. The purpose of the article is to show the connection of «aging-in-place» with other theoretical approaches, namely community care, deinstitutionalization of services and care for the older persons, and especially with the social exclusion of the elder. We are convinced of this understanding by the actions of European countries in the development of local communities and the popularity of the idea of «aging-in-place», supported by the Madrid International Plan of Action on Aging. The goal of the policy in all countries regarding the older persons is the same: to provide the older persons with the opportunity to stay in their homes as long as possible, that is, to grow old at the place of residence [10]. It is also necessary to clarify the applicability of this approach in Russian conditions. The method of the article is theoretical analysis.
