{"title":"The role of a clinical nurse specialist in the care pathway of children referred for an autism assessment","authors":"Barbara Kombe","doi":"10.12968/johv.2023.11.10.416","DOIUrl":"https://doi.org/10.12968/johv.2023.11.10.416","url":null,"abstract":"The Covid-19 pandemic has led to an increase in demand for assessment for children with suspected autism spectrum disorder (ASD), with waiting times in the UK varying from 1–3 years. This article outlines a project to introduce a neurodevelopment clinical nurse specialist (CNS) to a care pathway for assessment in response to this need. The role included undertaking assessments in a clinical setting and providing early intervention support to children and their families. The clinics targeted the 0–5 age group, streamlining the ASD diagnostic pathway, offering a differential pathway to other specialist services in neurodevelopment. A service audit showed that around 60% of children identified by a CNS went on to receive a diagnosis of ASD in comparison to paediatricians, who were able to identify 80% of patients. A total of 70% of children were discharged by the CNS, with the remaining 30% needing a follow-up or a differential diagnosis and/or consideration of other investigations. It is concluded that health visiting teams would benefit from the clinical expertise of a CNS to build on evidence-based practice in this specialist area. This study is the first to present model–neutral evidence of an association between home-visiting participation, well-child visits and emergency department visits. De-identified, propensity-score matched claims data from South Carolina Medicaid in the US was employed to evaluate healthcare use. Logistic regression was used to examine the association between well-child visit compliance and home-visiting participation with differences in emergency department visits and well-child visit examined via linear regression. Well-child visit compliance varies by age, but for certain age groups, home-visiting clients are significantly more likely to meet well-child visit recommendations. On average, home-visiting clients had a significantly higher number of emergency department visits. Twenty health visitors were interviewed over 22 months, commencing in 2018, using interpretative phenomenological analysis (IPA) combined with an art-based method. A comprehensive professional identity for health visitors was established, as discussed in a previous article ( Seaman, 2022 ). The effects of service changes as described by the study participants were then mapped onto the core dimensions of their perceived professional identity. This article charts these study findings and offers recommendations for health visitor managers, employers and policy makers.
