It is believed that insomnia results from the unstable spirit in pathogenesis of TCM. The basic therapeutic principle is regulating the spirit, in which, stabilizing the primary spirit and tranquilizing the heart spirit are emphasized. Main acupoints are Baihui (GV 20), Sishencong (EX-HN 1) and Yintang (GV 24<sup>+</sup>) to stabilize the primary spirit, located on the head; and Shenmen (HT 7) located on the wrist to calm-down the heart spirit, as well as Sanyinjiao (SP 6) and Yongquan (KI 1) in the lower extremities to benefit <i>yin</i> and balance <i>yang</i>, and then to house the spirit eventually. The needles are inserted in different depths and to various directions. The external application of herbal plaster is combined at Yongquan (KI 1) and the supplementary acupoints are selected in terms of syndrome differentiation. In terms of their respective characteristics, sham acupuncture has a wider scope, including various types of acupoints, needle insertion at non-acupoint or non-insertion at acupoints, while placebo acupuncture mainly focuses on non-insertion at acupoints. Sham acupuncture mainly emphasizes the appearance similarity to real acupuncture, while placebo acupuncture emphasizes both similarity in appearance and the absence of therapeutic effects. Properly distinguishing and applying sham acupuncture and placebo acupuncture can help standardize their usage in terminology. {"title":"[Effect of moxibustion on plaque psoriasis complicated with obesity].","authors":"Nan-Nan Zhang, Jun Li, Yan-Ran Ban, Rui-Cheng Li, Rui Tian, Zhen-Hua Nie","doi":"10.13703/j.0255-2930.20220724-k0004","DOIUrl":"https://doi.org/10.13703/j.0255-2930.20220724-k0004","url":null,"abstract":"<p><strong>Objective: </strong>To observe the clinical efficacy of moxibustion combined with <i>coptis chinensis</i> ointment sealing on plaque psoriasis complicated with obesity.</p><p><strong>Methods: </strong>A total of 52 patients of plaque psoriasis complicated with obesity were randomized into an observation group (26 cases) and a control group (26 cases, 2 cases dropped off). <i>Coptis chinensis</i> ointment sealing was adopted in the control group. On the basis of the treatment in the control group, moxibustion was applied at <i>ashi</i> point (area of local target lesions), Zhongwan (CV 12) and bilateral Zusanli (ST 36), Fenglong (ST 40), Quchi (LI 11), Tianshu (ST 25), Shangjuxu (ST 37) in the observation group. The treatment was given 30 min each time, once a day for 4 weeks in both groups. The psoriasis area and severity index (PASI) score, obesity related indexes (body mass, waist circumference, body mass index [BMI]), triglyceride, cholesterol, uric acid and plasma glucose were compared before and after treatment, and the clinical efficacy was evaluated in the two groups.</p><p><strong>Results: </strong>After treatment, the PASI scores were decreased compared with those before treatment in the two groups (<i>P</i><0.01), and the PASI score in the observation group was lower than that in the control group (<i>P</i><0.05); the body mass, waist circumference, BMI, triglyceride, cholesterol, uric acid and plasma glucose were decreased compared with those before treatment in the observation group (<i>P</i><0.01, <i>P</i><0.05), the triglyceride and cholesterol in the observation group were lower than those in the control group (<i>P</i><0.05). However, compared with the <i>yuan</i>-source points of <i>yin</i> meridians for the treatment of <i>zang</i>-organ diseases, there is less attention to the indication of <i>yuan</i>-source points of <i>yang</i> meridians for <i>fu</i>-organ diseases, and this statement is even questioned. Collating the early literature and associating with the researches of medical experts, it is found that <i>Nanjing</i> (<i>Classic of Difficult Questions</i>) should be the theoretic origin that <i>yuan</i>-source points of <i>yang</i> meridians are indicated in <i>fu</i>-organ diseases. Regarding the reasons why this theory has not received clinical attention, the three aspects are involved, i.e. the theoretic completion of \"<i>he</i>-sea points of three-foot-<i>yang</i> meridians for the diseases of six <i>fu</i>-organs\", the limitation of the theory itself, and the lack of literature materials. {"title":"[Effects of transcutaneous electrical acupoint stimulation on the postoperative sleep quality and inflammatory factors in frail elderly patients].","authors":"Meng Zhang, Nan Zhao, Jin-Hua He, Jian-Li Li","doi":"10.13703/j.0255-2930.20220919-k0002","DOIUrl":"https://doi.org/10.13703/j.0255-2930.20220919-k0002","url":null,"abstract":"<p><strong>Objective: </strong>To observe the effects of transcutaneous acupoint stimulation (TEAS) on sleep quality and inflammatory factor in frail elderly patients undergoing laparoscopic colorectal cancer surgery.</p><p><strong>Methods: </strong>A total of 100 frail elderly patients undergoing elective laparoscopic colorectal cancer surgery were randomly divided into an observation group and a control group, 50 cases in each one. Patients in the observation group received TEAS, 30 min before surgery until the end of surgery, at 18:00 on the day of surgery and on the 1st, 2nd and 3rd day after surgery (30 min each time). TEAS was delivered at bilateral Neiguan (PC 6), Shenmen (HT 7) and Hegu (LI 4). The disperse-dense wave of 2 Hz/100 Hz was selected, and the maximal stimulation intensity depended on patient's tolerance. The operation procedure in the control group was same as the observation group, but without electric stimulation exerted. The 1st day before surgery and on the 1st, 3rd and 7th day after surgery, the scores of Pittsburgh sleep quality index (PSQI) and Athens insomnia scale (AIS), as well as the serum levels of C reactive protein (CRP) and interleukin-6 (IL-6) were observed in the patients of two groups. At 24 h, 48 h and 72 h after surgery, the score of pain visual analogue scale (VAS) was recorded in the two groups, as well as the pressing times of analgesic pump and the usage of flurbiprofen axetil during analgesic stage. The occurrence of post operative adverse reactions was observed in the patients of two groups.</p><p><strong>Results: </strong>On the 1st and 3rd day after surgery, except the usage of hypnotic drug scores, the scores of each item and the total scores of PSQI, as well as AIS scores were all increased in the two groups compared with those of 1 day before surgery (<i>P</i><0.05)<i>;</i> and the scores in the observation group were lower than those in the control group (<i>P</i><0.05). On the 7th day after surgery, the scores of each item and the total scores of PSQI, and AIS scores were not different statistically in comparison between the two groups (<i>P</i>>0.05). On the 1st, 3rd and 7th day after surgery, serum levels of CRP and IL-6 were all increased in the patients of two groups when compared with those of 1 day before surgery (<i>P</i><0.05), serum levels CRP and IL-6 in the patients of the observation group were lower than those of the control group (<i>P</i><0.05). The patients in the control group were treated with routine obstetric care. Based on the treatment of the control group, the patients in the observation group were treated with auricular thumbtack needle at Neifenmi (CO<sub>18</sub>), Xiong (AH<sub>10</sub>), Xiongzhui (AH<sub>11</sub>), Shenmen (TF<sub>4</sub>), and Jiaogan (AH<sub>6a</sub>), etc., with one side of auricular point selected, only once for a total of 3 d. The lactation initiation time, lactation adequacy rate at postpartum 72 h, exclusive breastfeeding rate at postpartum 42 d, and breastfeeding score after treatment were compared between the two groups. Real-time quantitative PCR and Western blot method were used to detect the mRNA and protein expression levels of TDP-43, Btn1A1 and XDH.</p><p><strong>Results: </strong>After treatment, the lactation initiation time in the observation group was earlier than that in the control group (<i>P</i><0.01), and breastfeeding score in the observation group was higher than that in the control group (<i>P</i><0.01). The lactation adequacy rate at postpartum 72 h was 63.8% (30/47) in the observation group, which was higher than 41.7% (20/48) in the control group (<i>P</i><0.05). The exclusive breastfeeding rate at postpartum 42 d was 72.3% (34/47) in the observation group, which was higher than 47.9% (23/48) in the control group (<i>P</i><0.05). {"title":"[Effects of acupuncture on hemorrhagic transformation and motor function in stroke patients after intravenous thrombolysis with rt-PA: a prospective cohort study].","authors":"Chen-Xi Liang, Ling-Yong Xiao, Jing-Ying Gan, Xiao-Xiao Shi, Xiao-Xuan Wang, Yi Liu, Chun-Lei Tian, Xiao-Yu Dai","doi":"10.13703/j.0255-2930.20221206-0002","DOIUrl":"https://doi.org/10.13703/j.0255-2930.20221206-0002","url":null,"abstract":"<p><strong>Objective: </strong>To observe the effects of the <i>Xingnao Kaiqiao</i> (regaining consciousness and opening orifices) acupuncture on hemorrhagic transformation and limb motor function after intravenous thrombolysis with recombinant tissue plasminogen activator (rt-PA) in stroke patients.</p><p><strong>Methods: </strong>A total of 130 stroke patients after rt-PA thrombolytic were divided into an acupuncture group (58 cases, 1 case dropped off) and a non-acupuncture group (72 cases, 7 cases dropped off) according to whether they received acupuncture treatment. Propensity score matching (PSM) was used to match each group, with 38 patients in each group. The patients in the non-acupuncture group received rt-PA thrombolytic therapy and western medical basic treatment. In addition to the basic treatment, the patients in the acupuncture group received <i>Xingnao Kaiqiao</i> acupuncture at Shuigou (GV 26), bilateral Neiguan (PC 6), and ipsilateral Sanyinjiao (SP 6), Chize (LU 5), once a day for 14 days. The incidence of hemorrhagic transformation within 30 days after onset was compared between the two groups. The Fugl-Meyer assessment (FMA) score and activities of daily living (ADL) score were observed at baseline and 30 days, 6 months, 1 year after onset in the two groups. The disability rate at 6 months and 1 year after onset was recorded, and safety was evaluated in both groups.</p><p><strong>Results: </strong>The incidence of hemorrhagic transformation in the acupuncture group was 5.3% (2/38), which was lower than 21.1% (8/38) in the non-acupuncture group (<i>P</i><0.05). At 30 days, 6 month, and 1 year after onset, the FMA and ADL scores of both groups were higher than those at baseline (<i>P</i><0.01), and the scores in the acupuncture group were higher than those in the non-acupuncture group (<i>P</i><0.01). The disability rate in the acupuncture group at 1 year after onset was 10.5% (4/38), which was lower than 28.9% (11/38) in the non-acupuncture group (<i>P</i><0.05). Using systematic review, semi-structured interview, Delphi questionnaire survey, analytic hierarchy process and expert consensus meeting, the primary outcomes are obtained, i.e. local tenderness, pain degree during movement, range of motion, changes in range of motion, function score, and score of local symptoms of shoulder joint. The secondary outcomes are myofascial thickness, thickness of the inferior wall of the joint capsule, health status, activity of daily living, incidence of adverse events, laboratory indexes, vital signs, cost-effectiveness, total effective rate, and patient satisfaction. Through the motion control of the stepping motor by programmable logic controller (PLC), the automatic control is obtained for the acupoint detection of heat-sensitive moxibustion therapy and the manual operation of moxibustion. The skin temperature is monitored in real-time, using infrared non-contact temperature measurement technology. Based on the deviation of the temperature set value and the monitoring one, the distance between the moxibustion device and the exerted region is adjusted automatically by PLC so that the temperature is controlled practically. The multifunctional moxibustion treatment machine based on the heat-sensitive moxibustion therapy is capable of the operation control of mild moxibustion, circling moxibustion, sparrow-pecking moxibustion and along-meridian moxibustion techniques, as well as real-time monitoring of skin temperature. The temperature change curve of this machine is coincident with that obtained by the manual operation of heat-sensitive moxibustion. As a relatively safe non-drug treatment, acupuncture has the underlying advantages in participating into multidisciplinary coordination in the enhanced recovery after surgery (ERAS).
