{"title":"[Pricking and cupping at <i>Jianbo</i> area combined with conventional acupuncture for scapulohumeral periarthritis of frozen stage: a randomized controlled trial].","authors":"Tian-Yi Zhou,&nbsp;Qing Han,&nbsp;Feng Wang,&nbsp;Peng-Fei Gao,&nbsp;Jie Zhu,&nbsp;Jing Li","doi":"10.13703/j.0255-2930.20221118-k0004","DOIUrl":"https://doi.org/10.13703/j.0255-2930.20221118-k0004","url":null,"abstract":"<p><strong>Objective: </strong>To compare the clinical effect of conventional acupuncture combined with pricking and cupping at <i>Jianbo</i> area and conventional acupuncture in the treatment of scapulohumeral periarthritis of frozen stage.</p><p><strong>Methods: </strong>A total of 66 patients with scapulohumeral periarthritis of frozen stage were randomly divided into a combination group (31 cases) and an acupuncture group (35 cases, 1 case dropped off). Both groups were given functional exercise. Patients in the acupuncture group were treated with acupuncture at Jianyu (LI 15), Jianliao (TE 14), Binao (LI 14) and <i>ashi</i> point on the affected side, once every other day, three times a week, for a total of 4 weeks. On the basis of treatment in the acupuncture group, the patients in the combination group were treated with pricking and cupping at <i>Jianbo</i> area (the area surrounded by the 3 acupoints of Tianzong [SI 11], Naoshu [SI 10] and Jianzhen [SI 9]), once a week for 4 weeks. The University of California-Los Angeles (UCLA) shoulder joint score, visual analogue scale (VAS) score before treatment, after treatment and after 6 months of treatment completion (follow-up) and tenderness threshold before and after treatment, and the clinical effects of the two groups after treatment and in follow-up were evaluated.</p><p><strong>Results: </strong>In the two groups, after treatment and in follow-up, the UCLA shoulder joint scores were higher than those before treatment (<i>P</i><0.05), and the VAS scores were lower than those before treatment (<i>P</i><0.05). In the combination group, after treatment and in follow-up, the UCLA shoulder joint score was higher than that of the acupuncture group (<i>P</i><0.05), and the VAS score was lower than that of the acupuncture group (<i>P</i><0.05). After treatment, the tenderness thresholds of the two groups were higher than those before treatment (<i>P</i><0.05), and the tenderness threshold in the combination group was higher than that in the acupuncture group (<i>P</i><0.05). Except for the normal group, ovalbumin sensitization method was used to establish the asthma model in the other 3 groups. Salbutamol nebulization was adopted in the medication group, while acupuncture was applied at bilateral \"Kongzui\" (LU 6) and \"Yuji\" (LU 10) in the acupuncture group. The intervention was given once a day for 14 days in the two groups. The latent period of inducing asthma and pulmonary function were observed, the levels of ET-1 and tumor necrosis factor (TNF)-α in serum and bronchoalveolar lavage fluid (BALF) were detected by ELISA method, the morphology of the airway was observed by Masson staining, the ultrastructure of the airway smooth muscle was observed by transmission electron microscopy, the mRNA and protein expression of ET-1 and MT-2 in lung tissue was detected by real-time PCR and Western blot methods.</p><p><strong>Results: </strong>Compared with the normal group, in the model group, the latent period of inducing asthma was shortened (<i>P</i><0.01); the airway resistance (RL) was increased while the dynamic compliance (Cdyn) was decreased (<i>P</i><0.01, <i>P</i><0.05); the levels of ET-1 and TNF-α in serum and BALF were increased (<i>P</i><0.01); collagen fibers and collagen depositions were found around the bronchi, airway smooth muscle was thickened, the cell damage was severe and mitochondria were swollen; the mRNA and protein expression of ET-1 was increased while the mRNA and protein expression of MT-2 was decreased (<i>P</i><0.01). Compared with the model group, in the acupuncture group, the latent period of inducing asthma was prolonged (<i>P</i><0.05), the RL was decreased while the Cdyn was increased (<i>P</i><0.01, <i>P</i><0.05). Patients in the western medication group were treated with glucocorticoid + batroxobin + ginkgo leaf preparation. On the basis of the western medication group, patients in the acupuncture and medication group were treated with acupuncture at Baihui (GV 20), Ningshen, Yunting area, Mastoid 1 point, Mastoid 2 point, Fengchi (GB 20), Gongxue, Tinghui (GB 2), etc., repeated transcranial acupuncture was applied at Baihui (GV 20), Ningshen and Yunting area; Mastoid 1 point and Mastoid 2 point, Fengchi (GB 20) and Gongxue of the affected side were connected to the electroacupuncture instrument, continuous wave, 2 Hz in frequency. The needle was retained for 30 min, once a day, and rest for 1 d after 6 d of continuous treatment. All patients were treated for 3 weeks. The average hearing threshold, tinnitus handicap inventory (THI) score, dizziness handicap inventory (DHI) score and speech discrimination test score were compared between the two groups before and after treatment, and the clinical effect was evaluated.</p><p><strong>Results: </strong>After treatment, the average hearing threshold, THI and DHI scores of the two groups were lower than those before treatment (<i>P</i><0.05), and above indexes in the acupuncture and medication group were lower than those in the western medication group (<i>P</i><0.05). The speech discrimination test scores of the two groups were higher than those before treatment (<i>P</i><0.05), and the score in the acupuncture and medication group was higher than that in the western medication group (<i>P</i><0.05). The conventional rehabilitation therapy was delivered in the three groups. Additionally, acupuncture was applied to Jianyu (LI 15), Jianzhen (SI 9), Jianliao (TE 14) and Jianqian (Extra) etc. on the affected side in the manual acupuncture group. In the EA continuous wave group and the EA disperse-dense wave group, besides the treatment as the manual acupuncture group, the electric stimulation was attached to two pairs of acupoints, i.e. Jianyu (LI 15) and Jianliao (TE 14), and Quchi (LI 11) and Shousanli (LI 10), with 15 Hz continuous wave, and 2 Hz/ 100 Hz disperse-dense wave, respectively. The treatment was given once daily, 5 times a week, for 4 weeks consecutively. The score of visual analogue scale (VAS) before treatment and after 2 and 4 weeks of treatment, as well as the passive range of motion (PROM) of shoulder forward flexion and PROM of shoulder abduction, muscle strength of the upper limb, the score of modified Barthel index (MBI) and the score of Fugl-Meyer assessment (FMA) before and after treatment were observed in each group.</p><p><strong>Results: </strong>Compared with before treatment, VAS scores were reduced after 2 and 4 weeks of treatment in each group (<i>P</i><0.05); and VAS scores after 4 weeks of treatment were lower than those after 2 weeks of treatment (<i>P</i><0.05). After 2 and 4 weeks of treatment, VAS score in either the EA continuous wave group or the EA disperse-dense wave group was lower compared with the manual acupuncture group (<i>P</i><0.05). After 4 weeks of treatment, VAS score in the EA disperse-dense wave was lower than that of the EA continuous wave group (<i>P</i><0.05). Compared with before treatment, PROM of the shoulder forward flexion and abduction on the affected side after treatment was enlarged (<i>P</i><0.05), the muscle strength of the upper limb was increased (<i>P</i><0.05), and the scores of MBI and FMA were increased (<i>P</i><0.05) in the patients of each group. It is proposed that the treatment of <i>jingjin</i> diseases should be combined with the characteristic advantages of fire needling and bloodletting technique, on the base of \"eliminating stagnation and bloodletting/fire needling\". The patients in the control group were treated with intramuscular injection of compound diclofenac sodium injection and oral administration of tamsulosin hydrochloride sustained release capsules. In addition to the treatment in the control group, the patients in the observation group were treated with <i>Daoma</i> needling technique at the \"<i>Sanqi</i> points\" (<i>Qimen</i> point, <i>Qijiao</i> point, and <i>Qizheng</i> point) combined with <i>Dongqi</i> needling technique at \"<i>Sanhuang</i> points\" (sub-<i>Tianhuang</i> point, <i>Dihuang</i> point, <i>Renhuang</i> point), with each session lasting 30 min. The treatment in the two groups both started on the first day after surgery, and was given once daily for 14 consecutive days. Visual analog scale (VAS) score was compared between the two groups on postoperative day 1, 7, and 14; bladder residual urine volume, spontaneous voiding volume, and urinary catheterization frequency were assessed after treatment on postoperative day 1; and anorectal dynamic indexes (anal canal resting pressure, rectal resting pressure, maximum squeeze pressure of the anal canal, and minimum rectal sensory threshold) were evaluated before surgery and on postoperative day 4. Clinical efficacy was assessed in both groups one month after surgery.</p><p><strong>Results: </strong>On postoperative day 7 and 14, the VAS scores of both groups were lower than those on postoperative day 1 (<i>P</i><0.05), and the VAS scores in the observation group were lower than those in the control group (<i>P</i><0.05). The bladder residual urine volume and urinary catheterization frequency in the observation group were lower than those in the control group (<i>P</i><0.05), while the spontaneous voiding volume was higher than that in the control group (<i>P</i><0.05). On postoperative day 4, the anal canal resting pressure, maximum squeeze pressure of the anal canal, and the minimum rectal sensory threshold were lower than preoperative values (<i>P</i><0.05), while the rectal resting pressure was higher than preoperative value (<i>P</i><0.05) in both groups. The anal canal resting pressure, maximum squeeze pressure of the anal canal, and minimum rectal sensory threshold were lower than those in the control group, and the rectal resting pressure was higher than that in the control group (<i>P</i><0.05). It is found that <i>qicimai</i> points are the \"upward moving points\" in reference to the meridian distribution rule of \"rooting, running, infusing and moving\"; and corresponding to the sites of \"running outwards and inwards\" of the meridians' \"separating, meeting and running outwards and inwards\". It also includes the infusing points for the sea of <i>qi</i> and marrow. The new idea, \"selecting <i>qicimai</i> points for the treatment of <i>qi</i> obstruction in the neck gate\", is proposed. The basic western medication treatment was adopted in the two groups. In the control group, mild moxibustion was applied at Shuaigu (GB 8), Fengchi (GB 20) and Yanglingquan (GB 34) on the affected side. In the observation group, the frequent acupoint areas of the affected side i.e. Shuaigu (GB 8), Fengchi (GB 20), Taiyang (EX-HN 5), Taichong (LR 3), Yanglingquan (GB 34) were determined, 3 acupoints with strong heat-sensitive sensation were selected each time and mild moxibustion was adopted. The treatment was given once a day, 5 times of treatment was as one course and 2 courses were required in the two groups. Before and after treatment, the scores of migraine symptom, visual analogue scale (VAS), migraine specific quality of life questionnaire (MSQ) were observed, and the clinical efficacy was evaluated after treatment in the two groups.</p><p><strong>Results: </strong>After treatment, the scores of migraine symptom and VAS were decreased compared with those before treatment (<i>P</i><0.01), while the MSQ scores were increased compared with those before treatment (<i>P</i><0.01) in the two groups. After treatment, the scores of migraine symptom and VAS in the observation group were lower than those in the control group (<i>P</i><0.05), while the MSQ score in the observation group was higher than that in the control group (<i>P</i><0.01). {"title":"[Effects of moxa smoke through olfactory pathway on learning and memory ability in rapid aging mice].","authors":"Ying-Zhu Zuo,&nbsp;Yao Lin,&nbsp;Lue Ha,&nbsp;Bai-Xiao Zhao","doi":"10.13703/j.0255-2930.20220523-0003","DOIUrl":"https://doi.org/10.13703/j.0255-2930.20220523-0003","url":null,"abstract":"<p><strong>Objective: </strong>To observe the effects of moxa smoke through olfactory pathway on learning and memory ability in rapid aging (SAMP8) mice, and to explore the action pathway of moxa smoke.</p><p><strong>Methods: </strong>Forty-eight six-month-old male SAMP8 mice were randomly divided into a model group, an olfactory dysfunction group, a moxa smoke group and an olfactory dysfunction + moxa smoke group, with 12 mice in each group. Twelve age-matched male SAMR1 mice were used as the blank group. The olfactory dysfunction model was induced in the olfactory dysfunction group and the olfactory dysfunction + moxa smoke group by intraperitoneal injection of 3-methylindole (3-MI) with 300 mg/kg, and the moxa smoke group and the olfactory dysfunction + moxa smoke group were intervened with moxa smoke at a concentration of 10-15 mg/m<sup>3</sup> for 30 min per day, with a total of 6 interventions per week. After 6 weeks, the emotion and cognitive function of mice was tested by open field test and Morris water maze test, and the neuronal morphology in the CAI area of the hippocampus was observed by HE staining. The contents of neurotransmitters (glutamic acid [Glu], gamma-aminobutyric acid [GABA], dopamine [DA], and 5-hydroxytryptamine [5-HT]) in hippocampal tissue of mice were detected by ELISA.</p><p><strong>Results: </strong>The mice in the blank group, the model group and the moxa smoke group could find the buried food pellets within 300 s, while the mice in the olfactory dysfunction group and the olfactory dysfunction + moxa smoke group took more than 300 s to find them. Compared with the blank group, the model group had increased vertical and horizontal movements (<i>P</i><0.05) and reduced central area residence time (<i>P</i><0.05) in the open field test, prolonged mean escape latency on days 1-4 (<i>P</i><0.05), and decreased search time, swimming distance and swimming distance ratio in the target quadrant of the Morris water maze test, and decreased GABA, DA and 5-HT contents (<i>P</i><0.05, <i>P</i><0.01) and increased Glu content (<i>P</i><0.05) in hippocampal tissue. The articles of the randomized controlled trial (RCT) on stable COPD treated with acupuncture and moxibustion were searched electronically in CNKI, Wanfang, VIP, SinoMed, PubMed, EMbase, Web of Science and Cochrane library. The search was conducted from the inception of the databases to March 20th, 2022. Data analysis was performed using R4.1.1, Stata16.0 and RevMan5.3 softwares. A total of 48 RCTs were included, involving 15 kinds of acupuncture and moxibustion interventions and a sample size of 3 900 cases. The results of network Meta-analysis showed that: ① For the forced expiratory volume in one second predicted (FEV<sub>1</sub>%), both the governor vessel moxibustion combined with conventional treatment (G+C therapy) and the <i>yang</i>-supplementing moxibustion combined with conventional treatment (Y+C therapy) obtained the better effect than that of the conventional treatment (<i>P</i><0.05), and the G+C therapy was more effective compared with the thread-embedding therapy combined with conventional treatment (E+C therapy) and warm needling (<i>P</i><0.05). ② Concerning to COPD assessment test (CAT) score, the results indicated that the Y+C therapy, and the mild moxibustion combined with conventional treatment (M+C therapy) were more effective when compared with the conventional treatment (<i>P</i><0.05), and the effect of the Y+C therapy was better than that of the E+C therapy (<i>P</i><0.05). ③ Regarding six-minute walking distance (6MWD), the effect of acupuncture combined with conventional treatment (A+C therapy) was better than that of either the E+C therapy or the conventional treatment (<i>P</i><0.05). The effect of the G+C therapy was optimal for improving FEV<sub>1</sub>%, the Y+C therapy obtained the best effect for improving CAT score, and A+C therapy was the most effective for improving 6MWD.
