{"title":"[Application value of serum protein indicators in constructing the early prediction model for the prognosis of patients with pulmonary tuberculosis].","authors":"H F Song, M Y Wu, J P Zhang, Y J Feng, P Xu, J Zhao, J Xue, L J Huang, J Li","doi":"10.3760/cma.j.cn112147-20221021-00836","DOIUrl":"https://doi.org/10.3760/cma.j.cn112147-20221021-00836","url":null,"abstract":"<p><p><b>Objective:</b> To analyze the clinical significance of laboratory examination indicators as the key prognostic factors and to construct an early prediction model for prognosis assessment of pulmonary tuberculosis patients. <b>Methods:</b> The basic information, biochemical indexes and blood routine items of 163 tuberculosis patients (144 males and 19 females, aged 41-70 years, with an average age of 56 years) and 118 healthy persons who underwent physical examination (101 males and 17 females, aged 46-64 years, with an average age of 54 years) in Suzhou Fifth People's Hospital from January 2012 to December 2020 were retrospectively collected. According to the presence of <i>Mycobacterium tuberculosis</i> after six months of treatment, the enrolled patients were divided into a cured group (96 cases) and a treatment failure group (67 cases). To analyze the baseline levels of laboratory examination indicators between these two groups, we screened the key predictors and the binary logistic regression method in SPSS statistics software was used to construct the prediction model. <b>Results:</b> The baseline levels of total protein, albumin, prealbumin, glutamic-pyruvic transaminase, erythrocyte, hemoglobin and lymphocyte were significantly higher in the cured group than in the treatment failure group. After 6 months of treatment, the indexes of total protein, albumin and prealbumin increased significantly in the cured group, but remained at the low levels in the treatment failure group. Receiver operating characteristic (ROC) curve analysis showed that total protein, albumin and prealbumin as independent predictors for forecasting the prognosis of pulmonary tuberculosis patients had the highest prediction accuracy. Logistic regression analysis showed that the combination of these three key predictors could construct the best early prediction model for assessing the prognosis of pulmonary tuberculosis patients, with a prediction accuracy of 0.924 (0.886-0.961), sensitivity of 75.0%, specificity of 94%, showing an ideal prediction accuracy. <b>Conclusions:</b> The routine test indexes of total protein, albumin and prealbumin show good application value in the construction of early prediction model for prognosis evaluation of pulmonary tuberculosis treatment. However, the lack of equipment and real-time monitoring of ventilatory lung function during pulmonary rehabilitation treatment is a challenge. In addition, there is a need for improved methods that can directly guide physiotherapists to provide precise treatment. Electrical impedance tomography (EIT) is a novel medical imaging technology that allows real-time monitoring of lung ventilation status. It is currently being translated from basic research into clinical applications and is widely used in respiratory disease, particularly in critical care respiratory management. However, there is a lack of reports on pulmonary rehabilitation guidance and outcome evaluation. {"title":"[Relationship between respiratory event-related arousal and increased pulse rate in patients with obstructive sleep apnea].","authors":"S R Xu, C Peng, Y Wang, B Y Chen, J Q Wang, M R Yang, R Wang","doi":"10.3760/cma.j.cn112147-20221102-00867","DOIUrl":"https://doi.org/10.3760/cma.j.cn112147-20221102-00867","url":null,"abstract":"<p><p><b>Objective:</b> To investigate the relationship between respiratory event-related arousal and increased pulse rate in patients with obstructive sleep apnea (OSA), and to evaluate whether elevated pulse rate can be used as a surrogate marker of arousal. <b>Methods:</b> A total of 80 patients [40 males and 40 females, age range (18-63 years), mean age (37±13) years] who attended the Sleep Center of the Department of Respiratory and Critical Care Medicine, Tianjin Medical University General Hospital for polysomnography (PSG) from January 2021 to August 2022 were enrolled. Stable PSG recordings of non-rapid eye movement (NREM) sleep to compare the mean pulse rate (PR), the lowest PR 10 seconds before the onset of arousal, and the highest PR within 10 seconds after the end of arousal associated with each respiratory event. At the same time, the correlation between the arousal index and the pulse rate increase index (PRRI), as well as ΔPR<sub>1</sub> (highest PR-lowest PR) and ΔPR<sub>2</sub> (highest PR-mean PR), respectively, with the duration of respiratory events, the duration of arousal, the magnitude of pulse oximetry (SpO<sub>2</sub>) decline, and the lowest SpO<sub>2</sub> was analyzed. Among the 53 patients, 10 events without arousal and 10 events with arousal (matched for the magnitude of SpO<sub>2</sub> decline) were selected for NREM in each of the 53 patients, and ΔPR before and after termination of respiratory events in the two groups was compared. In addition, 50 patients were simultaneously subjected to portable sleep monitoring (PM) and divided into non-severe OSA group (<i>n</i>=22) and severe OSA group (<i>n</i>=28), and ΔPR≥3 times,≥6 times,≥9 times, and≥12 times after respiratory events were used as surrogate markers of arousal, and ΔPR was scored manually and integrated into the respiratory event index (REI) of PM. Then, we compared the agreement between REI calculated from the four PR cut-off points and the apnea-hypopnea index (AHI<sub>PSG</sub>) calculated by the gold standard PSG. <b>Results:</b> ΔPR<sub>1</sub> [(13±7)times/min] and ΔPR<sub>2</sub> [(11±6)times/min] were significantly higher in patients with severe OSA than in patients with non-OSA,mild and moderate OSA. The arousal index was positively correlated with the four PRRIs (<i>r</i> 0.968, 0.886, 0.773, 0.687, <i>P</i><0.001, respectively), and the highest PR [(77±12) times/min] within 10 s after the end of arousal was significantly higher than the lowest PR [(65±10) times/min, <i>t</i>=113.24, <i>P</i><0.001] and the mean PR [(67±11) times/min, <i>t</i>=103.02, <i>P</i><0.001]. ΔPR<sub>1</sub> and ΔPR<sub>2</sub> were moderately correlated with the decrease in SpO<sub>2</sub> (<i>r</i>=0.490, 0.469, <i>P</i><0.001). Immunohistochemical methods were used to identify lung adenocarcinoma tissues, para-cancerous tissues and parallel transcriptome sequencing. Genetic prognostic analysis was conducted by TCGA and GTEx databases.The expression level of COL11A1 gene in lung adenocarcinoma and adjacent tissues was detected by Western blotting.The primary human lung adenocarcinoma cells cultured. The COL11A1 siRNA was transfected into primary human lung adenocarcinoma cells, then the transcriptome sequencing of differential genes was performed,and KEGG enrichment analysis of differential gene enrichment pathway was conducted. Protein expression and phosphorylation were detected by Western blot method. Cell migration was detected by scratch healing test. Cell proliferation was detected by CCK8 method and invasion ability was detected by Transwell method. <b>Results:</b> Ten differentially expressed genes were screened by transcription sequencing in lung adenocarcinoma. Prognostic analysis of single gene showed that COL11A1 gene expression level was correlated with survival rate (<i>P</i><0.001). The expression of COL11A1 in lung adenocarcinoma was higher than that in adjacent tissues by Western blot (<i>P</i><0.001). Transcriptome sequencing of COL11A1 siRNA transfection into primary human lung adenocarcinoma cells showed that differential genes were concentrated in PI3K-akt pathway. The expression of tumor suppressor gene PTEN in siRNA transfection group was significantly higher than that in control group and negative transfection group by Western blot. The expression of Aktp-Akt 473 p-Akt 308 p-PTENp-PDK1p-c-Rafp-GSK-3 β was down-regulated (all <i>P</i><0.05).Compared with the negative control group, the ability of migration, proliferation and invasion of primary human lung adenocarcinoma cells in siRNA transfection group decreased (all <i>P</i><0.05). It brings new challenges to the standard diagnosis, treatment and comprehensive management of PH in China. Compared with the world, there are still many problems in the field of PH in China. The heterogeneity of PH causes the complexity of the disease and the difficulty of clinical management, and the early identification and diagnosis of pH face great challenges. Individualized and precise treatment needs to be further optimized, and standardized diagnosis and treatment strategies need to be popularized and promoted. In recent years, rapid progress has been made in the field of PH, including its pathogenesis, diagnostic thresholds, classification and comprehensive treatment methods, prompting an update of the guidelines, which brings a new level of standardized diagnosis and comprehensive management of PH in China. Chest CT showed the nodule was lobulated measuring 24 mm×23 mm, with obvious enhancement and adjacent pleural traction. As the PET-CT showed increased <sup>18</sup>F-FDG uptake suggesting malignancy, the wedge resection of the right lower lobe was performed. Grossly, the mass was adjacent to the pleural area with indistinct boundary. On cut sections, the lesion was solid and tough, with a greyish-pink colour. Microscopically, the lesion had an ill-defined margin, and was composed of spindle and polygonoid histiocytes with rich eosinophilic cytoplasm similar to rhabdoid muscle cells. The cytoplasm of histiocytes was filled with diamond-shaped or club-shaped crystals. Immunohistochemistry (IHC) showed the histiocytes were positive for CD68, κ, λ, IgG, IgM and IgA. The patient had been followed up for 41 months and had shown neither recurrences nor new diseases. CSH is a rare non-neoplastic histiocytic proliferative disease. Pulmonary CSH should be differentiated from multiple diseases. Accurate pathological diagnosis depends on its morphology and immunophenotype. This disease is often related to potential lymphoproliferative or plasma cell disorder. However, epidemiological information and relevant reviews on CTEPH are currently lacking in China. The present study is a successful case report of pulmonary vein stenosis and pulmonary infraction secondary to mediastinal seminoma. The HRQOL of patients with tuberculosis sequelae has gradually received attention, but there are few relevant studies.
