{"title":"[Pharmacology of modern anti-arrhythmia drugs in therapy of supraventricular tachycardia].","authors":"P Honerjäger, G Schmidt","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Prevention of recurrences of atrial fibrillation, slowing the ventricular rate during atrial fibrillation, and the acute management of atrioventricular junctional reentrant supraventricular tachycardia (paroxysmal supraventricular tachycardia) often require treatment with antiarrhythmic drugs. These drugs comprise a pharmacodynamically and pharmacokinetically heterogeneous group of agents whose individual properties determine correct use, contraindications and side effects. Stabilisation of sinus rhythm can be achieved with class IA and class IC sodium channel blocking drugs as well as with the class III agents amiodarone or sotalol. Verapamil, diltiazem, cardioselective beta-adrenoceptor-blocking drugs or cardiac glycosides can be used to slow the ventricular rate during atrial fibrillation. Rapid termination of paroxysmal supraventricular tachycardia is achieved with i.v. administration of adenosine, verapamil, ajmaline, diltiazem, propafenone, or flecainide. {"title":"[Non-pharmacologic treatment of pre-excitation syndromes].","authors":"D Pfeiffer, J Tebbenjohanns, W Jung","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Paroxysmal tachyarrhythmias due to accessory atrioventricular pathways are usually not completely suppressible with antiarrhythmic drug therapy. These patients can be treated by implantable antitachycardia pacemakers or surgical or catheter ablation of accessory pathways. The antitachycardia pacemaker offers a symptomatic treatment, which is indicated in patients with less often arrhythmias, without atrial fibrillation and with haemodynamically stable tachycardias. The surgical interruption of accessory pathways depends on thoracotomy and often the cardiopulmonary bypass. This therapy is a curative treatment, which is indicated in patients with further cardiac disease needing surgical intervention. Catheter ablation of accessory pathways with radiofrequency energy is the modern treatment. They make very high demands on the physician in respect of differential diagnosis and clinical assessment. Atrial tachycardias, atrial flutter and atrial fibrillation are mainly associated with an underlying cardiac disease. The isolated forms of atrial flutter and fibrillation without any detectable cardiac disease are a diagnostic challenge and are associated with a relatively high rate of complications. {"title":"[Despite use of iodized salt in individual food preparation there remains a 1st degree iodine deficiency in Germany].","authors":"R Hampel, K Rönsch","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Due to the fact that the use of iodated table salt is not compulsory in Germany and entirely at the discretion of the individual consumer, there is hardly any effective prevention of endemic iodine deficiency struma in this country. The good results achieved in the former GDR with iodine prophylaxis dropped to the FRG level after the unification. Using iodated salt when preparing individual food dishes is not enough to counteract iodine deficiency. We proved this in our study which is communicated below. An iodine deficiency of the first degree remains. The M-mode and two-dimensional echocardiographic findings are presented, and the patient's clinical progress is described. The HDL fraction only showed a reduction of 10%. These dextran effects were dose-dependent. During the post-treatment period of 20 days a continuous increase in the lipid parameters to the preinfusion values was demonstrated. {"title":"[Illness-specific knowledge, coping with illness and metabolic control in insulin-injecting diabetic patients in a random city population].","authors":"U A Müller, G Berlet, I S Ross, H Klinger","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>A knowledge test about diabetes using a standardized interview was assessed in 186 (81%) of 229 insulin treated patients below the age of 60 who are registered and treated in the diabetes care unit Jena. 90% of these patients were on conventional insulin therapy. The frequency of errors was positive correlated to glycosylated haemoglobin, age and body-mass-index, it was lower in type 1 than in type 2 diabetic patients and decreased with a better education level. Deficiencies of knowledge were detected in the following areas: hyperglycaemic coma, consequence of acetone and obesitas. Knowledge regarding late complications was on a high level. 20% of type 1 and 43% of type 2 diabetic patients did not know their daily carbohydrate units. But the metabolic control in these patients was not different from the patients who know their carbohydrates exactly. Therefore, we repeated the investigation in 63 patients one year after the first examination. In 6 patients (10%) hyperthyroidism had developed. Euthyroid autonomy was diagnosed in 11 patients (17%). A prolonged normalisation period of the pituitary-thyroid regulation after having discontinued the thyroid hormone drugs was found in 7 patients (11%). In 40% of the patients the TRH-test remained negative and in 22% of the patients the TSH-response became positive without any known reason.
