{"title":"The Italian story of HEMS: the HEMS Association","authors":"","doi":"10.53767/rp.2021.01.01.en","DOIUrl":"https://doi.org/10.53767/rp.2021.01.01.en","url":null,"abstract":"Mountain HEMS Association was born, with a notarial deed of incorporation, in April 2008 following the initiative of 7 autonomous Region /Provinces of the Alps and Apennines: in 2005, Dr. Enrico Visetti, Anesthetist Doctor of the Aosta hospital, organized a helicopter rescue training course aimed at new doctors to be included in the local helicopter rescue. In December 2006, access to the course was offered to all the other operational bases in the Alps. The invitation was extended to the various figures operating within the Service, therefore doctors, nurses, and subsequently the CNSAS Helicopter Rescue Technicians: with the participation of new external experiences, the course objective was also modified. In 2007, 90% of the participants in the training course were Doctors and Nurses who already had experience and actively operated in the helicopter rescue in the bases of origin: the participants themselves had to bring their own ideas and experiences and simultaneously cover the role of speakers. Until now, the main objective, concentrated in the training addressed to new doctors, became, thanks to the support of the various experiences, an exchange of opinions and a training ground for discussion between the various actors. The technical mountaineering part was managed thanks to the support of the mountain rescue, always present in the external activities alongside the staff. In the theoretical part, various lines of common behavior were discussed to be adopted in the interventions of Helicopter Rescue, while in the context of the scientific discussion, the total lack of a database of helicopter rescue services in which to obtain useful information was recognized. L’Italia ha mostrato un incremento sostanziale di richieste di soccorso per patologie respiratorie e il personale infermieristico impiegato sui mezzi di soccorso è stato travolto dalla situazione con conseguenti ripercussioni psicofisiche quali depressione e Post Traumatic Stress Disease (PTSD). Lo studio mira a valutare l’impatto dell’emergenza pandemica sugli infermieri del Sistema di Emergenza Territoriale (SET). Metodo Analisi di depressione e PTSD mediante “Screening Questionnaire for Disaster Mental Health”. Il questionario è stato distribuito tra gli infermieri italiani afferenti al SET con campionamento non probabilistico tra il 1° dicembre 2020 e il 31 gennaio 2021. Risultati Hanno partecipato allo studio 441 infermieri italiani con età media 43,28 anni (DS ± 9,38), l’esperienza lavorativa media nel SET è 11,68 anni (DS ± 7,98). Il 6,12% dei partecipanti lavora presso una Centrale Operativa (CO), il 72,34% è impegnato nel soccorso territoriale e il 21,54% opera in entrambi i setting. Il 17,01% del campione è ad alto rischio per PTSD e il 15,65% per depressione. Discussione Il campione è costituito da infermieri generalmente esperti nel soccorso territoriale. Il rischio di PTSD e depressione rientra nei range individuati in letteratura. L’analisi per setting operativo ha evidenziato un maggior rischio di sviluppare PTSD per gli infermieri che svolgono attività di CO, con valori quasi doppi rispetto a chi opera solo sui mezzi di soccorso. Il personale con poca esperienza nel SET risulta soggetto a maggiori esiti per le 2 sintomatologie. Conclusione Il personale inesperto e quello impegnato nelle CO è risultato maggiormente vittima di PTSD e depressione. È essenziale attuare piani per permettere al personale del SET di superare tali situazioni di criticità e prevenirle. On 23 December 2020, at 6:44 pm, the parents of P, a two-year-old boy, called 118 because the little boy had lost consciousness following the ingestion of a small piece of mozzarella cheese. The dispatcher quickly identified the target of the call and conducted the health interview. From the mother’s information, the child appeared unconscious, with cyanotic lips and no respiratory activity. The nurse recorded the event on the operating system as K3 Advanced Red Blue, i.e., a respiratory pathology in the home was identified, and the code of maximum severity was assigned, which requires the simultaneous dispatch of an ambulance and self-medication, so the nurse in charge at 18:47 sent a BLSD vehicle and the ALS vehicle of territorial competence.
