The COVID-19 pandemic had a significantly impact on the healthcare system, particularly healthcare providers such as nurses, who were obliged to adhere to strict procedures and had to manage their time effectively during their shifts. The aim of this study was to to identify internal strengths, weaknesses, threats, and challenges in palliative care management during the COVID-19 pandemic, from the perspective of nurses. Methods. A cross-sectional research was conducted on a 100% sample of nurses working in hospital-based palliative care in two clinics in Belgrade (47 respondents in total). The research instrument was a Questionnaire on Employee Satisfaction. The survey was conducted during September and October 2022, according to ethical standards and principles. Results. The research revealed that nurses' job satisfaction was moderate (3.43), influenced by adequate financial compensation and the implementation of measures to prevent and control COVID- 19 infection. The dimensions of management significantly related to respondents' satisfaction were motivation and adequacy of hygienic conditions and measures to control COVID-19 infection (?2 = 62.83; p = 0.004** and ?2 = 36.42; p = 0.006**, respectively). Conclusion. Nurses who experienced stress at work in regular conditions tended to react the same way in different conditions, such as those related to COVID-19 infection. The toughest issues for management is recognizing the importance of nurses? job by the social community. {"title":"Neurophysiological confirmation of phrenic nerve affection in patient with dyspnea and herpes zoster","authors":"V. These lesions are usually unilateral and for most clinicians, the clinical manifestation of herpes zoster in the cervical or thoracic region and diaphragmatic paralysis on the same side is sufficient for making a diagnosis of segmental herpes zoster phrenic nerve palsy. We report a patient, who had a classic clinical picture, in which we confirmed phrenic nerve affection on nerve conduction study. Case report. A 58-year-old female patient, presenting with shortness of breath. The patient had a herpetic rash of her right shoulder two and a half months earlier. The elevation of the right hemidiaphragm was seen on chest X-ray imaging. Asymmetry was evident on nerve conduction study of the phrenic nerve: prolonged latency and reduced amplitude of her right phrenic nerve. The patient was treated by acyclovir, pregabalin, and B complex vitamins. After 6 months the motor deficit was completely reduced. Conclusion. Nerve conduction study of the phrenic nerve is useful in making the definitive diagnosis. One of possible manifestations of primary hyperparathyroidism (PHPT) is the appearance of a benign bone tumor. We hereby present a case of a young woman whose first clinical manifestation of PHPT was a brown tumor of the mandible. Case report. A female patient was hospitalized in the Clinic of Endocrinology, Diabetes and Metabolic Disease due to the problems occurring in the form of nausea, exhaustion, feeling of suffocation, dysphagia, pain in the right ear and the right half of the cheeks, who was under suspicion of primary hyperparathyroidism (PHPT). Initial laboratory findings pointed out the high levels of parathyroid hormone (PTH) and Ca2+ ions, low levels of vitamin D and increased parameters of bone metabolism with signs of osteopenia. Cone beam computed tomography (CBCT) revealed a presence of bilateral radiolucent lesions of the mandible. Scintigraphy showed a retrosternal hot focus consistent with parathyroid adenoma. Parathyroidectomy was performed with following normalization of PTH values, bone metabolism parameters, and normalization of the Ca2+ ion values. Four months after parathyroidectomy, there was a complete resolution of clinical and radiological findings of initial tumefaction of the mandible. Conclusion. Brown tumors are rare first clinical manifestation of PHPT. Because of their histological similarities with other giant-cell lesions (GCL), definitive diagnosis is sometimes difficult, and is based on correlation with radiological and laboratory findings. {"title":"Retrospective analysis of hospitalization rates in patients with schizophrenia 12 months before and 12 months after the switch on once-monthly long-acting injectable paliperidone palmitate","authors":"S. There is no available published data about use of long-acting injectable paliperidone palmitate in schizophrenia patients in Republic of Serbia. The aim of this study was to assess hospitalization rates before and after the switch on once monthly longacting injectable paliperidone palmitate in schizophrenia patients, as well as their compliance with this drug. Methods. This was retrospective cross-sectional study which was evaluating hospitalization rates in the period of 12 months before and 12 months after the switch on once-monthly long-acting injectable paliperidone palmitate in 113 schizophrenia patients. The age of enrolled patients was between 18 and 66 years. Results. Average age of enrolled patients was 38.36 ?11.62 years. Among them, 77 (68.1%) were male, and 36 (31.9% ) were female. Out of total number of 113 patients who were treated with once-monthly injectable paliperidone palmitate, 78 (69.03%) was on monotherapy, while 35 (30.97%) had one additional oral antipsychotic (risperidone, olanzapine, aripiprazole or clozapine). Out of total number of 113 patients, 68 (60.18%) were not hospitalized in 12 months period before the switch on once-monthly long-acting injectable paliperidone palmitate, while 45 (39.82%) were hospitalized in the same period. Considering the fact that in the period after the switch on once-monthly injectable paliperidone palmitate 8 patients out of total number of 113, were discontinued from treatment due to adverse event or their own decision, analysis of the hospitalization rate after the switch was performed for remaining 105 patients, and the results showed that 9 (8.57%) were hospitalized in the period of 12 months after the switch, while 96 (91.43%) were not hospitalized in the same period. Conclusion. Our results show high compliance in treatment with once-monthly injectable paliperidone palmitate and positive impact of treatment with this drug on low hospitalization rate in 12 month period in patients with schizophrenia. The cause of discolored gingiva (DG) is the presence of metal in the gingival tissue. DG is a relatively common case, and is caused by deposition of amalgam material in the gingival tissue during dental application or oral surgery. The aim of this study was to determine the significance of cast post presence for appearance of DG of abutment teeth (AT) in relation to the type of fixed dental prosthesis (FDP) alloy. Methods. The study included 327 subjects with FDP. A total of 1585 AT were examined. Subjects were classified into four groups depending on the type of alloy of which their FDPs were fabricated of (silver-palladium, gold, nickel-chromium alloy or a combination of them). The presence of a cast post was confirmed by X-ray or by inspection of the dental chart. The data were collected within one year. Results. DG was observed in more than one third (37.7%) of the subjects of which 57.3% had a cast post . In the case of AT, discoloration occured in 14.6% of them. Cast post was present in 92.7% of AT with DG. Conclusion. DG occured in all groups of subjects regardless of the type of alloy from which the FDP was fabricated. Johanson-Blizzard syndrome is a very rare genetic disorder caused by a mutation of the ubiquitin protein ligase E3 component N-recognin 1 (UBR1). Clinical diagnosis is based on the pathognomonic combination of congenital exocrine pancreatic insufficiency with facial dysmorphology (nasal wing hypo/aplasia, and oligodontia of permanent teeth). Diagnosis is confirmed by genetic screening of the UBR1 gene. The purpose of this case report is to emphasize that nucleotide variants in the UBR1 gene that are described as benign or unclassified should be considered as genetic causes in patients with clinical characteristics of Johanson-Blizzard syndrome. Case report. In this report, we present an 8-month old child who was admitted to our hospital due to poor weight gain and loose stools. On admission, the signs of protein-energy malnutrition, with facial dysmorphological signs and other anomalies, were observed. He had hypotonia and convergent strabismus. A laboratory examination confirmed exocrine pancreatic insufficiency and hypothyroidism. Genetic testing confirmed two single nucleotide variants in ubiquitin protein ligase E3 component N-Recognin 1, chromosome 15q15.2: NM_174916.3:c.4700+12A>G (intron 42), and NM_174916.3 UBR1:c.862-18C>T (intron 07). A pancreatic enzyme replacement therapy with liposoluble vitamin supplementation and adequate nutrition was conducted. Conclusion. Recognition of clinical features of Johanson-Blizzard syndrome and genetic confirmation is very important, especially in patients with idiopathic pancreatic insufficiency. Apical periodontitis (AP) is an acute or chronic inflammation of the periradicular tissue, usually caused by the presence of microbial irritants in the root canal system. The aim of the present study was to radiographically investigate the AP prevalence in root-filled teeth in adults from an urban area of Vojvodina. Methods. Randomly selected digital ortopantomographs of 616 subjects attending the Clinic of Dentistry of Vojvodina, Serbia, were examined. Periapical status, root canal filling (RCF) quality, type of coronal restorations, and their relationships were evaluated. The chi-square test was used for statistical analysis. Results. The total number of endodontically treated teeth was 965, 44.2% of them received an adequate RCF score, and 34.9% had AP, which was diagnosed in 57.4% of inadequately scored root-filled teeth. Posterior teeth had significantly worse RCF quality than anterior teeth (p < 0.001). The direct logistic regression results indicate that a technically well-performed RCF reduced the risk of AP almost 21-fold. The prevalence of AP in our cohort was not influenced by the type of restoration (direct or indirect), but statistically significantly, the highest prevalence of AP was found in rootfilled teeth without any coronal restoration. Conclusion. Increasingly evidence showed that high-sensitivity C-reactive protein (hs-CRP) is a reliable biomarker in patients with hypertension. While the relationship between hypertension and age is well established, the connection between hs-CRP and age remains unclear. The aim of the study was to determine a relationship between hs-CRP and age in middle-aged people. Methods. This crosssectional survey was conducted in Shanghai, China, and included data from 1,677 healthy male participants aged 18 to 50 years and 1,127 healthy female participants aged 19 to 49 years, who were recruited during routine health examinations. The hs-CRP, body mass index (BMI), systolic blood pressure (SBP), diastolic blood pressure (DBP) were recorded. Results. The participants were first separated into four age quartile groups, and an increase in BMI, SBP, and DBP was observed, but not in hs-CRP. Next, the participants were divided into four hs-CRP quartile groups and an increase in BMI, SBP, and DBP was noted, but not in age. Finally, using Pearson correlation, positive correlations were found between hs-CRP, BMI, SBP, and DBP, but no correlation was seen between age and hs-CRP. Conclusion.
