Transanal total mesorectal excision (TaTME), which is a new approach in rectum surgery, is the resection of the rectum with the perianal approach with the help of endoscopic instruments. Although there are specially designed tools for this approach, it can also be done with the help of other options. Our patient, who we prepared for the operation with the diagnosis of rectal cancer, was operated with a standard technique using a manually prepared sponge-port during the Coronavirus disease-19 pandemic period. After resection, the specimen was taken out transrectally, coloanal anastomosis was performed with 2/0 vicryl one by one, and the operation was completed with a protective ileostomy. Operation time was 245 minutes. The patient who did not develop postoperative complications was discharged on the 5th day. Although key for diagnosis,\u0000clinical symptoms tend to be weak and atypical in the elderly.\u0000Therefore, patients present late to health institutions. Hence,\u0000prognosis and complication rates are worse in the elderly. Here, we\u0000aimed to reveal the role of basic laboratory parameters in diagnosing AA\u0000and determining disease severity. Methods: 143 elderly patients who\u0000underwent appendectomy were retrospectively analyzed. The patients were\u0000divided into three groups based on AA severity as Group I: negative\u0000appendectomy (n=15); Group IIa: uncomplicated appendicitis (n=79); Group\u0000IIb: complicated appendicitis (n=49). Results: We found no difference\u0000between the groups for age, sex, or comorbid diseases\u0000(p>0.05). As the time of admission to the hospital\u0000increases, the severity of the disease increases. Group IIb had higher\u0000length of stay and complication rates (p<.0.05) Conclusions:\u0000Preoperative WBC, neutrophil, NLR, MPV, CRP, and direct and total\u0000bilirubin levels can be used as biomarkers to determine AA diagnosis in\u0000the elderly.
