Haki, Hakan Demirci","doi":"10.46310/tjim.988602","DOIUrl":"https://doi.org/10.46310/tjim.988602","url":null,"abstract":"Background To examine the relationship between an institutionally prepared standardized patient evaluation form and the Edmonton Frail Scale (EFS) in patients receiving home health care. \u0000Material and Methods Our prospective, observational study included 200 patients over the age of 18 who requested home health care, regardless of gender. The EFS and institutional data collection forms were applied consecutively on the same day to all patients included in the study. \u0000Results Among the 200 individuals recruited for the study, 59% were female and 41% were male; the overall average age was 80 years. According to the EFS results, 4.5% of the patients were classified as non-frail, 6% were vulnerable, and 89.5% had varying degrees of frailty (mild, moderate and severe). The gender, education level, age of onset of diabetes, presence of hypertension, smoking and body mass index of the patients were examined, and the effects of these variables on time elapsed for switching to insulin therapy were analyzed in three phases. \u0000Results Three hundred fifteen patients (117 males, 198 females) were enrolled in the study. The mean time elapsed for switching from oral antidiabetic therapy to insulin therapy was 9.93±6.67 years. The effects of education level, age at the onset of diabetes, presence of hypertension, and body mass index on time elapsed for switching to insulin therapy were found to be statistically significant (p0.05). The time elapsed for switching to insulin therapy shortened as the education level, the age at the onset of diabetes, and body mass index level increased. Occult breast cancer (OBC) is a rare diagnosis in which physical examination, imaging methods, and even surgical procedures are insufficient to put on a primary tumoral site in the breasts. This definition leads to only 0.3-1% of all breast cancer cases. The diagnosis of OBC is usually obtained with an axillary lymph node biopsy. A 52-year-old female presented with right arm weakness and a neck lump. On the physical examination, multiple masses were palpated at the right axillary, right supraclavicular, and anterior cervical areas. Mammography, breast ultrasonography, breast magnetic resonance imaging, and 18-fluorodeoxyglucose positron emission tomography (18-FDG PET) scan could not show a suspicious tumoral area as the primary origin. 18-FDG PET scan put on the massive tumoral burden at multiple bones and lymph nodes, but there were no lesions to suspect as the tumoral origin. Finally, the supraclavicular lymph node biopsy result has revealed the diagnosis; hormone receptor-positive and C-erbB2 positive occult breast cancer. Cervical lymph node metastasis is also a scarce condition for breast cancer. Several reports indicated that PCOS patients have lower vitamin D status compared to healthy subjects. In our study we aimed to investigate whether vitamin D deficiency has effect on the pathogenesis of insulin resistance in PCOS. Material and Methods Fourty eight patients with PCOS and 24 healthy controls were included in the study. Following the physical examination and anthropometric measurements of the patients and healthy subjects, glycemic control data, lipid values, parathormone, vitamin D status and hormonal parameters were studied. Results In our study, vitamin D levels were significantly lower in PCOS patients (19.7±26.9 ng/mL) compared with controls (31.9±35 ng/mL, p<0.01). Vitamin D levels were found to be lower in the obese PCOS group compared to those with non-obese, but not significant. Statistically significant inverse correlation was found between vitamin D levels and body mass index (BMI).
