The time to attend the screening programme, conducted in Keelung, Taiwan, within 10 years was assessed by innovativeness (innovators, early adaptors, early majority, late majority and laggard) using data from 79,303 residents, with the information on metabolic syndrome accrued from routine adult health check-ups. The median time of adopting the programme and the relative rates of early adoption by metabolic syndrome and its severity score were estimated. The results show that the estimated times to adopt the programme ranged from 3 months for innovators to 10 years for the laggard. The rate of early adoption was 34% higher for participants without metabolic syndrome than for those with the disease, and the gradient relationship of disease severity was noted. The adjusted median time to adopt innovativeness was 0.82 years earlier for participants who were disease-free than those with the disease. Meanwhile, the adjusted median time was wider by up to 2.25 years for those with severe disease. A lack of appropriate access and engagement with public health nursing (PHN) service contributes to major health disparities for CALD mothers.</p><p><strong>Aim: </strong>To describe and synthesize the available literature on the factors that promote or hinder CALD mothers' access to and engagement with PHN services and in particular sustained nurse home-visiting (SNHV) programs. This paper identifies gaps and new areas of needed research inquiry to support best provision of PHN for CALD mothers.</p><p><strong>Method: </strong>A narrative review of the literature was conducted. Databases searched included PubMed, CINAHL, MEDLINE, Cochrane, and Scopus with year of publication between 2000 and 2020.</p><p><strong>Results: </strong>Of 463 total articles, 12 met inclusion criteria. The key barriers and facilitators identified were grouped in three major themes. These include mother-related issues, experiences with interpreters, and program and program delivery related issues.</p><p><strong>Discussion: </strong>The review demonstrates that CALD mothers with limited English proficiency (LEP) are experiencing multiple challenges in accessing and engaging with PHN and SNHV services. This article shares the story of how the PHI Wheel was created, disseminated, implemented by public health nurses (PHNs) and educators across the globe, and updated with new evidence published in the second edition of Public Health Interventions: Applications for Public Health Nursing in 2019. Evidence on the relevance of PHI Wheel interventions for public health practice in cultural and international settings supports the model's value in explaining PHN practice. This article highlights the experiences of various countries with the PHI Wheel including Canada, Ireland, New Zealand, Norway, Sweden, the United Kingdom, and the United States. California mandates an HEP for every child in foster care.</p><p><strong>Design: </strong>Utilizing an online survey, PHNs describe their agency's use of the HEP, reported how they obtain health and education records for the HEP.</p><p><strong>Results: </strong>Social workers, PHNs and mental health providers were main HEP users, less so for foster parents and youth who age out of foster care. The HEP was used at medical and dental visits. PHNs reported little to no participation in updating educators. Recruitment was conducted through email. Interviews were conducted in Winter 2020. Data were analyzed using thematic analysis.</p><p><strong>Sample: </strong>A total of 12 participants were recruited to participate in this study. Six participants identify as Black and six identify as Indigenous. All participants identify as women.</p><p><strong>Measurements: </strong>Qualitative interviews were conducted using a semi-structured interview guide to elicit a breadth of information.</p><p><strong>Results: </strong>Key themes included training experiences, training improvements and ''bridging the gap''. The training validated participants' experiences of birth and began to address the exclusion of Black and Indigenous people from birth work. However, participants expressed concerns about not being adequately positioned for sustained participation in birth work.</p><p><strong>Conclusions: </strong>Participants expressed receiving great value from the training programs. These trainings, which were fully subsidized, removed a financial barrier. However, these trainings do not address the exclusion of Black and Indigenous people from perinatal work or the lack or sustainable support systems for Black and Indigenous communities. This article provides examples aimed at helping and inspiring others who might be interested in conducting a complex intervention study. The intervention focused on counseling about food and feeding practices provided by public health nurses in a sample of child health centers in Norway 2015-2018. Aspects of food and nutrition are central to public health nurses' counseling at the child health center, and they experience challenges when counseling on these themes. This article offers an approach to handling the complexity of public health nursing interventions on counseling among families. We examined complexities in DR-TB care transition between community- and hospital-based care.</p><p><strong>Design: </strong>We conducted a case study using medical record data. Four patients were purposively selected to illustrate intersectional complexities in DR-TB care transition involving PHN.</p><p><strong>Results: </strong>Case A (HIV negative male) received PHN care at a community-based facility 124 km from Cape Town. Cases B, C, and D (males living with HIV) received PHN community-based care, averaging 25 km from the hospital. Treatment failed in cases A, B, and C; they subsequently died. Case D was cured. All cases were granted leave of absence at least once while hospitalized. None returned when expected mainly due to lack of transport funds. PHN played critical roles regarding patients' return by conducting home visits, interacting with relatives, and assisting emergency officers to transport patients back to the hospital. PHN supported relatives to endure protracted patient hospitalizations.</p><p><strong>Conclusion: </strong>The role of PHN in continuity of DR-TB care in low-middle income countries is unambiguous. PHN are key partners in the DR-TB care cascade, namely facilitating retention in care between hospital and community-based care. The purpose of this paper is to examine the healthcare needs and difficulties experienced by foreign residents in Japan by level of their Japanese language fluency.</p><p><strong>Design: </strong>A quantitative, descriptive design was used.</p><p><strong>Sample: </strong>We surveyed foreign residents in the greater Tokyo area (N = 209).</p><p><strong>Measurements: </strong>The research team created the survey questions and the contents of the survey include foreign residents' experiences during their visits or stays at medical and public health facilities in Japan.</p><p><strong>Results: </strong>More than 90% of the participants or their families visited medical facilities in Japan regardless of Japanese language fluency; however, those with less Japanese language fluency experienced statistically significant uneasiness or inconvenience concerning communication compared to those with native Japanese language fluency (p = .000).</p><p><strong>Conclusions: </strong>Nurses in Japan may benefit from additional training related to use of interpreters as well as education about diversity and cultural humility. We sought to identify the extent to which there were variations in home visiting care in response to the program schedule and families' risk factors.</p><p><strong>Design and sample: </strong>Data collected within the right@home program, a randomized controlled trial (RCT) for a sustained nurse home visiting intervention in Australia, were analyzed. A total of 352 women comprised the intervention arm of the trial.</p><p><strong>Measurements: </strong>Visit content in the home visiting program, sociodemographic data, and families' risk factors were used for analysis.</p><p><strong>Results: </strong>Our results confirmed that the majority of women received scheduled content on time or within an acceptable timeframe, except for the sleeping program. Women with identified risks were significantly more likely to receive content related to those risks than women without those risks (smoking: Odds Ratio [OR] = 15.39 [95%CI 3.7-64.7], mental health: OR = 15.04 [1.8-124.0], domestic violence: OR = 4.07 [2.0-8.3], and drugs and alcohol: OR = 1.81 [1.1-3.0]).</p><p><strong>Conclusions: </strong>The right@home program had high compliance with the scheduled content. Capacity development in responding to mothers with the risk of domestic violence and drugs and alcohol is recommended. {"title":"Mothers susceptible to child maltreatment and requiring public health nurses' continuous support.","authors":"Kyoko Yoshioka-Maeda,&nbsp;Hitoshi Fujii","doi":"10.1111/phn.13004","DOIUrl":"https://doi.org/10.1111/phn.13004","url":null,"abstract":"<p><strong>Objective: </strong>This study aimed to identify the characteristics of mothers who are at a high risk for future child maltreatment and therefore, require continuous support from public health nurses (PHNs) to prevent child maltreatment and those who do not require it.</p><p><strong>Design and samples: </strong>This retrospective cross-sectional study included women who were registered in 2018 for being at risk for future child maltreatment and required help with childrearing in City A within the Tokyo metropolis.</p><p><strong>Measurements: </strong>Public health nurses registered data pertaining to each mother, including demographic data, family-related variables, assessment by PHNs, use of maternal and child healthcare services, and continuity of support from PHNs.</p><p><strong>Results: </strong>Of the 206 cases, the discontinued group-comprising those who no longer needed PHNs' support-included 83 cases (40.3%) and the continuing group-comprising those needed continuing support-included 123 cases (59.7%). Logistic regression analysis showed that those with marital conflicts, maternal mental instability, lack of parenting skills, and not moving out of the city were more likely to require continuous PHNs' support.</p><p><strong>Conclusions: </strong>By identifying these factors, PHNs can identify new mothers who need continuous support and provide more concentrated care for them.
