Aim: The aim of this study is to describe and evaluate the admissions to the University Psychiatric Emergency Unit of University Hospital Ghent (UPSIE) and the Mobile Crisis Team (MCT) of the Gent-Eeklo-Vlaamse Ardennen region during the COVID-19-pandemic. Method: A retrospective observational study was conducted, analyzing data from UPSIE and MCT from 2018 to 2021. We compared the clinical and socio-demographic characteristics of admission in 2020 and 2021 with those in 2018 and 2019. Results: During the COVID-19 pandemic, we observed a decrease in admissions at UPSIE. There was an increase in suicidality and requests for involuntary hospitalization in 2020 and 2021. The proportion of female admissions increased during the COVID-19 pandemic. As for MCT, we observed an increase in admissions in 2021, with a rise in the proportion of anxiety disorders in 2020 and mood disorders in 2021. Conclusion: We found changes in the number and nature of admissions to UPSIE and MCT during the COVID-19 pandemic. Vanwalleghem","doi":"10.47671/tvg.79.23.048","DOIUrl":"https://doi.org/10.47671/tvg.79.23.048","url":null,"abstract":"Rare neutrophilic dermatosis with significant hematologic implications A patient was seen in the dermatology department because of a solitary lesion on the right forearm. This lesion had an annular aspect with a raised, bright erythematous, inflammatory border of confluent papules with an oedematous aspect. Previously, similar lesions had developed, regressing spontaneously after a few weeks. Histological examination showed a marked oedema and a dens dermal predominant neutrophilic infiltrate with leucocytoclasia without vasculitis. Based on the clinical picture, the chronic recurrent course and the histology, the diagnosis of chronic recurrent annular neutrophilic dermatosis (CRAND) was made. Since CRAND belongs to the neutrophilic skin disorders, a comprehensive blood test was performed, showing a monoclonal B cell population. In consultation with a fellow haematologist, additional CT scans of the thorax and abdomen were performed. These showed neither adenopathies nor hepatosplenomegaly, upon which the diagnosis of monoclonal B cell lymphocytosis was made. This condition increases the risk of developing chronic lymphatic leukaemia. This case report thus shows a rare neutrophilic dermatosis that differs from Sweet syndrome given the absence of fever, systemic symptoms or neutrophilia in the blood. This case is specifically relevant because of the additional finding of a monoclonal B cell lymphocytosis, increasing the risk of developing chronic lymphatic leukaemia. In a changing healthcare landscape and the hospital of the future, a crucial question remains how to maintain a good quality of care for patients in a healthcare system under pressure. Enhanced recovery after surgery (ERAS) may provide the answer to this question and focuses on a multimodal, multidisciplinary patient-centered care pathway. The pathophysiological core of ERAS consists of reducing the surgical stress response and the associated inflammation to limit the degree of organ dysfunction and to enhance recovery. ERAS programs include (but are not limited to) evidence-based guidelines regarding a reduced fasting time and allowing the intake of carbohydrate-rich drinks up to 2 hours prior to surgery, minimally invasive surgery, multimodal opioid-sparing analgesia, normovolemia, normothermia, the avoidance or early removal of drains, and the early restart of nutrition and mobilization. With an increased compliance to ERAS elements, the outcome becomes better. Ample evidence exists that ERAS programs lead to a reduced length of stay, less complications, less readmissions, less reoperations and a reduced mortality. Additionally, ERAS programs are cost-saving. She has been treated for 8 months with pembrolizumab for a stage IVa non-small-cell adenocarcinoma of the right lung. A blood test reveals eosinophilia. The MRI of the left forearm shows oedematous thickening of the superficial and deep intermuscular fascia, consistent with fasciitis. On the biopsy, fascial thickening and accumulation of lymphocytes is observed. The diagnosis of eosinophilic fasciitis is made, presumably triggered by pembrolizumab. After multidisciplinary consultation at the Belgian Multidisciplinary Immunotoxicity Board (BITOX), the treatment with pembrolizumab is discontinued. Despite the low incidence (0.2/10,000 patients), early recognition can prevent severe complications, such as paralysis or death. The case of a 50-year-old woman with diffuse pain (including acute lower back pain), a subfebrile temperature and initially no neurological deficits is described. This case demonstrates the importance of early diagnosis. Further attention is paid to the link between a high erythrocyte sedimentation rate (ESR), an elevated C-reactive protein (CRP) and extreme back pain. An MRI confirms the diagnosis. Genetic expanded carrier screening (ECS) can determine this risk and allows couples planning a pregnancy to make informed reproductive choices. The Belgian genetic centers developed the ‘Belgian Genetic Expanded Carrier Screening’ (BeGECS), an ECS for 1,248 genes. The center for medical genetics (CMGG) of the Ghent University Hospital already received samples from over 350 couples. This article discusses the results of the first 250 analyses. Of the 250 couples, 70% consulted in the context of a preimplantation genetic testing (PGT) trajectory. These couples had an already known carriership in 1 or both partners of an autosomal dominant, autosomal recessive and/or X-linked disorder. These risks are of course not included in the results of the BeGECS analyses. The BeGECS analyses showed that 15 couples (6%) were at risk of having a child with an autosomal recessive disorder and 5 (2%) of having a child with a severe phenotype. In 7 couples (2.8%), 1 of the partners was carrier of an X-linked disorder. In 26% of those screened, individual carriership was identified for 1 of the highly frequent autosomal recessive disorders. Couples who are aware of this risk, prior to pregnancy, can make autonomous informed reproductive choices.
