Apart from orbital pathology, paranasal sinus mucocele could be a cause of progressive, unilateral, and gradual onset of proptosis. Herein, we discuss a case of sudden onset of non axial unilateral proptosis with diminution of vision. CECT showed heterogeneous enhancing mass lesions causing expansion and destruction of roof of the left orbit and inferomedial displacement of eyeball. {"title":"Simple Technique For The Management Of A Cataract Surgery Wound Site Scleral Abscess and Endophthalmitis","authors":"M. Kaur, T. Simple adjunctive measures enhance the success of conservative management for such cases. Method: Seventy-year-old female was diagnosed as scleral abscess with acute post operative endophthalmitis after one week of an uneventful scleral incision cataract surgery. The patient was managed with surgical wound debridement adjunctive to topical, intravitreal and systemic antibiotics. Results: Satisfactory final best corrected visual acuity of 20/40 was achieved at the last follow-up at 20 weeks. Conclusion: Post cataract surgery scleral abscess and endophthalmitis pose a significant management challenge. {"title":"Bilateral Electric Shock Induced Cataract","authors":"A. Chandak, A. The severity of the complications depends on the voltage and site of passage of electric shock. The probability of ocular involvement rises more if the injury occurs at the scalp or face. Electric shock induced cataract is usually bilateral. The exact pathogenesis of cataract development is unknown. Direct coagulation of lens proteins and the osmotic changes following damage to the subcapsular epithelium are thought to be responsible. Other ocular complications like conjunctival hyperemia, interstitial corneal opacities, uveitis, miosis, spasm of accommodation etc may occur. Some rare complications like optic nerve coagulation, necrosis of retina, choroid and optic atrophy have been reported. Macular oedema may lead to the development of macular cysts or holes. Paresis of extraocular muscles have been frequently observed. We report two rare cases of high voltage electric injury in two young male patients resulting in bilateral cataract. No other ocular complications were encounterd. Entry and exit wounds were present in both the patients. Fundoscopy, Ultrasound B – scan and OCT were done to evaluate the posterior segment which were normal . Patients underwent manual small incision cataract surgery (MSICS) with posterior chamber intra ocular lens implantation with good post operative visual recovery. A 17 years old male presented to the outpatient department with chief complaints of blurring of vision in the left eye for 13 days, with some improvement in last 3 days. The patient had consulted elsewhere 10 days back and was noted to have a visual acuity of 6/60 in the left eye and was diagnosed to have an Optic disc pit with serous retinal detachment involving the macula. He was advised pars plana vitrectomy with gas tamponade but he preferred to wait for some time. The patient presented to us with improvement of the visual acuity to 6/9 in the left eye. Detailed fundus examination showed a large disc with a pit at the temporal disc margin with serous detachment seen temporal to the fovea. The patient was advised against any intervention and asked for regular follow up. Total unilateral limbal stem cell deficiency was managed by performing single stage simple limbal stem cell transplantation (SLET) combined with large graft penetrating keratoplasty, cataract surgery and posterior chamber intra-ocular lens implantation. She regained 20/80 vision at 3 months. The ocular surface was stable and the cornea was clear. Further visual gain was limited due to anterior capsular opacification and amblyopia. Treatment of unilateral corneal blindness following ocular surface burns, once considered incurable, gives rewarding results.
