Erdem","doi":"10.5606/e-cvsi.2021.1120","DOIUrl":"https://doi.org/10.5606/e-cvsi.2021.1120","url":null,"abstract":"Objectives: In this study, we present the late results of patients who underwent carotid body tumor (CBT) resection and compared postoperative complication rates according to the Shamblin classification. Patients and methods: Between April 2012 and April 2019, a total of 55 consecutive patients (20 males, 35 females; mean age: 56.2±1.8 years; range, 38 to 62 years) who were operated for CBTs were retrospectively analyzed. The patients were classified according to the Shamblin classification. Demographic and clinical characteristics of the patients, postoperative complications, total amount of drainage, and length of hospital and intensive care unit stay were recorded. Results: Carotid body tumors were bilateral in five patients, while they involved only the right side in 23 and left side in 27 patients. The CBTs were Shamblin type 1 in 16, type 2 in 31, and type 3 in eight patients. Intraoperative vascular reconstruction was required in eight patients. Early postoperative morbidities were transient ischemic attack (TIA) in three, cranial involvement in nine, middle cerebral artery occlusion in one, dysphonia in six, dysphagia in six, and hemiparesthesia in one patient. Permanent neurological damage (hoarseness 10.9%, ptosis 45%, internal carotid artery 1.81%) was seen in 18.2% of the cases. The length of stay in the hospital and intensive care unit, total amount of drainage, rates of postoperative cranial nerve involvement, permanent neurological damage, disease recurrence, postoperative TIA, and dysphagia were found to be significantly increased in patients in the Shamblin type 3 (p<0.005). Diagnosis and treatment are challenging. Aortoenteric fistulas can occur either as a complication of aortic aneurysm or, more commonly, secondary to previous aortic reconstructive surgery. Conventional treatment options are graft excision and extra-anatomic bypass or in situ graft replacement. This treatment is associated with high morbidity and mortality rates. Endovascular repair is an emerging therapeutic option. He was under treatment for varicose veins due to femoral and saphenous insufficiency for 10 years. Edema, redness, and cyanosis were present in his toes on physical examination. According to the pulse examination, organ meridians with reduced energy were applied to the body acupuncture needles (Liv-3, St-41, Kid-6, St-25,24, Sp-9). The ear was stabbed in the ankle, Shen men’s point. Diet for weight control of the leg was suggested. Thanks to the most effective acupuncture points for edema resolution, the measurements yielded a 4-cm decrease in the tibia plateau and 3-cm decrease in the ankle circumference. The patient’s pain and cyanosis decreased. Local acupuncture was applied around the contracture as the patient’s complaints diminished. A four-month-old girl, who was born at 25 weeks and treated in the neonatal intensive care unit for one month due to respiratory distress, was admitted to our clinic with complaints of swelling and bruising on the right arm. Doppler ultrasound revealed an arteriovenous fistula located between the basilic vein and brachial artery, which was surgically treated. Unlike adults, fistulas during infancy may lead to a variety of morbidity, including heart failure, if left untreated. Patients and methods: This cross-sectional study included a total of 153 patients (121 males, 32 females; mean age 58.7±10.9 years; range, 18 to 80 years) who underwent open heart surgery in our cardiovascular surgery clinic and readmitted with SSI within 30 days after surgery between September 2014 and December 2014. Risk factors which were related to the patient, surgery, and hospital stay were evaluated. Results: Of the patients, 73.8% had coronary artery bypass grafting (CABG). A total of 22 patients (14.4%) were readmitted to hospital after discharge with SSI. Half of them had saphenous infections (superficial infection n=8, deep infection n=3), while the other half had sternal infections (superficial infection n=7, deep infection n=4). Nine of these patients were rehospitalized. The mean length of hospital stay was 14.2±3.8 (range, 10 to 20) days. The risk factors for SSI development included patient, surgery, and hospitalization and hospital setting. Conclusion: Our study results show that several risk factors may play a role in the development of SSIs and rehospitalization. They may present with different manifestations. A 30-year-old male patient was admitted with self-inf licted left parasternal stab wound in the second intercostal space. Computed tomography revealed pericardial and pleural effusion. Echocardiography and aortography showed severe aortic regurgitation and aorto-right ventricular communication mimicking a congenital sinus Valsalva aneurysm rupture. The patient was operated and aortic leaf let repair with a pericardial patch and closure of the communication on both aortic and right ventricular sides were performed.
