Its exact prevalence is unknown and it is detected by way of an urinary measurement of creatine. ARC is defined as a creatinine clearance greater than 130 ml / min / 1.72 m2. Pathological mechanisms are not clearly defined and are probably multifactorial. For patients with confirmed ARC, dosing modifications of all renally purified medicinal products should be taken into account. The use of conventional doses can lead to the failure of the therapy with worse results for the patient and a higher economic cost for the healthcare system. It hypothesizes the existence of many points in the way of channels, where the vital energy (Qi) flows. These points have therapeutic actions and many of them can be used in the anaesthetic time to improve the perioperative patient management. Materials and methods: A bibliographic research was conducted to identify the evidence based of acupuncture benefits during the anaesthetic time and the physiological mechanisms implicated in these actions. Results and discussion: The main areas where acupuncture showed evidence based benefits during the anaesthetic time were anxiety,sedation, hypnosis, analgesic requirements and nausea and vomiting. Almost all these actions are mediated by the nervous system and chemical substances such as endogenous opioids and some neurotransmitters. Despite the great knowledge it has been reached in this review on the basic mechanisms that explain actions of acupuncture during anaesthesia, more studies are needed to allow us to know in detail and precision the physiological mechanisms and the structures involved. This growing population is overweight, and they still have some of the co-morbidities of obesity, such as hypertension, ischemic heart disease, pulmonary hypertension, sleep apnea, iron deficiency anemia, hyperglycemia, among other pathologies. They should be considered as high anesthetic risk and therefore, should be thoroughly evaluated. If more than one surgery is planned, a safe operative plan must be defined. The anesthetic management is adjusted to the physical condition of the patient, the anatomical and physiological changes, the psychological condition, as well as the surgical plan. Anemia is a frequent complication of obesity and bariatric procedures and should be compensated with appropriate anticipation. Pre-anesthetic medications may include benzodiazepines, alpha-2 agonists, anti-emetics, antibiotics, and pre-emptive analgesics. Regional anesthesia should be used whenever possible, especially subarachnoid blockade, since it has few side effects. General anesthesia should be left as the last option and can be combined with regional techniques. It is prudent to use conscious sedation for facial and neck surgery, maintaining strict control, especially with respect to the airway management. The necessity of prescribing a sedative drug in premedication was determined individually and was guided by the obtained data of the Kerdo vegetative index. The results of the study: Analysis of the depth of sedation showed that 5 minutes after sedation in 7 children (8.4%), the degree of sedation was at the RO level, and in 67 children (80.7%) at the RI level, in 9 children (10.8%) at R2 level. After 10 minutes, 72.3% of the children began to experience drowsiness, but at the same time they were in contact, performed simple commands, opened their eyes to treatment, which corresponded to the R3 degree of sedation. The remaining 27.7% of the children did not experience drowsiness, while they were calm and contact (sedation degree R2). At the 20th minute of the study, 84.3% of the children reached R3 sedation. And 15.7% of children at the time of separation from their parents corresponded to degree R2.Thus, in the light of the above information, each anesthetist should carefully plan all aspects of the preoperative introduction of each individual child, including premedication means in this plan, if there is a clear need for them. The point of view of some doctors \"applying\" standard premedication is extremely erroneous, since there are no two identical patients.
