This system was developed following an object-oriented approach, and adopting the event calculus (EC) as a temporal ontology. EC is a general theory of time and change based on the concepts of event and property: an event happens at a time point and initiates/terminates one (or more) properties, while a property holds during a time interval. Examples of abstract properties in our domain are parameter state (normal, high, low), and parameter trend (increasing, decreasing and stationary). The system was tested by evaluating time-stamped sequences of numeric echocardiographic parameters (e.g. ventricular diameters, Doppler flow velocities, valve-lumen areas, etc.) in different clinical conditions (e.g. myocardial infarction, valve disease, etc.). Ripoli","doi":"10.1109/CIC.1997.647932","DOIUrl":"https://doi.org/10.1109/CIC.1997.647932","url":null,"abstract":"In order to evaluate the factor of cardiovascular risk in hypertensive patients, the most common measured physiological parameter is the arterial blood pressure (BP), often leaving out of considerations other important cardiovascular parameters, such as mechanical arterial compliance. To better evaluate the inherent variability of blood pressure, with respect to the control of cardiovascular system and situational determinants, a computerised system has been developed. By means of a PC, equipped with a 12 bits A/D board, BP, heart rate patient voice and posture and external temperature have been acquired. The systolic, diastolic, and mean values of BP and the heart rate have been continuously, beat-to-beat, acquired with a Finapres, while ad hoc developed sensors have been used to acquire the other parameters. The signals, acquired in the different detected situations, have been processed by means of a software developed in LabView environment. A linear regression of systolic blood pressure and RR interval in agreement with the Karemaker equation, is carried out, evaluating the baroceptorial system sensitivity. The developed system may be usefully exploited in the investigation of the baroceptorial reflex and the of control mechanism of cardiovascular system. A significant decrease from 0.10 to 0.087 Hz of the centre frequency of the low frequency component of the power spectrum was observed with tilt. In order to better understand the nature of this frequency shift, a multivariate model of the effects of respiration and blood pressure on heart rate was applied to data from one individual. ECG leads I, avF and V1 were recorded during pacing in 23 patients, and QRS complexes aligned by (i) peak voltage, (ii) dot product, (ii) QRS fiducial, (iv) pacing artifact. TWA was computed as the T-wave power spectral sum at 0.5 cycles/beat. Fifteen patients had inducible ventricular tachycardia (VT) and 8 were non-inducible. From the latter group, 20/24 leads failed to exhibit TWA and were used to simulate alternans via a 1% augmentation of alternate STU segments. TWA was subsequently retrieved from 46%, 100%, 74% and 92% of simulations, with TWA ratios of 2.98, 5.02, 3.68, 4.49 with each alignment. Similar magnitude and sensitivity trends were seen in the VT group. The model features an anatomically accurate geometry, an intramural rotating anisotropy, and a physiologically accurate computational implementation of propagated excitation. The authors' findings agreed with those of experimental studies in a complete transient loss of one of the positive areas when the necrosis was located subepicardially, and in a transient gap in the expanding positive areas when the necrosis was located intramurally and subendocardially. This study investigated the value of an anatomically accurate model of the human ventricular myocardium in simulating BSPMs that can be used as templates for discriminating among septal preexcitation sites. Preexcitation sequences were initiated at 12 sites at different regions of the basal septum; for each sequence, the authors calculated BSPMs at 2-ms increments within the first 40 ms after the onset of preexcitation. Viable and non-viable tissue are determined from polar displays by comparison with normal subjects. The LV is isolated on the blood pool studies by assignment of regions of interest on beating images in the short-, vertical long- and horizontal long-axes. LV size is then determined from the total number of voxels in the LV cavity. The geometry of the LV is assessed by a sphericity index representing the ratio of the measured LV volume to the volume of a sphere with diameter equal to the long-axis dimension of the actual ventricle. Especially, the usefulness of the DWT to the determination of the QRS-duration (QRSD), which is the main parameter of the time-domain. Late potential analysis, was under investigation. Using DWT processing, the classification performance (leaving-one-out simulation) based on QRS duration increased by approximately 10% in accuracy (in each detail signal) as compared to the standard VLP analysis (51 healthy volunteers vs. 44 patients with coronary artery disease). This was due to an increase of sensitivity. A three-dimensional model of the coronary arterial tree is reconstructed from a professional model using Coordinate Measuring Machine (CMM) probing and the coronary anatomy is represented as a topologic hierarchy structure. Each segment of the vessel is associated only with its relative segments; i.e. parents and children. By doing so, the boundary checking of the catheter/guidewire in/outside the vessel is performed upon an optimized group of a small number segments and, therefore, has dramatically reduced the time for contact detection and subsequently the computation for catheter navigation.
