The goal as stated in the book is to provide a guide for staff physicians, residents, and advanced practitioners on the indications and applications of nuclear medicine for diagnosis, staging, and therapy of malignancy. After an opening guide on how to use the book, there is an initial chapter providing a general description of 18F-FDG PET and PET/CT molecular imaging, including how 18F-FDG is taken up in cells, indications for examinations, patient preparation, how the tests are performed and reported, pitfalls, and a couple of case studies. Most of the remaining chapters are organized by cancer type, with additional chapters on pediatric cancers and the comparative costs of diagnostic procedures. Appendix 1 covers radiation and its effects, imaging devices in nuclear medicine, and molecular imaging. Appendix 2 covers radionuclide pharmaceuticals for therapies. As stated in the opening guide, each chapter is subdivided into the following sections: epidemiology, advances in nuclear imaging, staging and prognosis, diagnostic nuclear medicine scans and primary clinical indications, and radiopharmaceuticals. A description of the procedure, information on patient preparation, explanations of the resulting report, and an examination of the treatment and overall impact of nuclear medicine imaging on patient management follows. Remaining sections address what is on the horizon for diagnosis or therapy for this cancer. Finally, case presentations are used to illustrate applications of nuclear imaging for some of the more common malignancies. Relevant references are offered at the end of each chapter for the reader who wishes to delve deeper. Chapter 2 covers lung cancer, including subdivisions for small cell, non–small cell, bronchoalveolar carcinoma, carcinoid, and mesothelioma. In the “on the horizon” section, the authors discuss apoptosis, tumor angiogenesis, and receptor blocking strategies that may someday be in the nuclear arsenal. Chapter 3 contains a great deal of information on breast cancer, including how therapies are chosen using PET/CT and sentinel lymph node biopsies. In chapter 4, the focus is prostate cancer, including coverage of 111In-capromab pendetide (ProstaScint; EUSA Pharma [USA], Inc.) and bone scanning as well as prognosis and staging. Chapter 5, on colon cancer, includes information about chemotherapy options and side effects. It explains why some patients need to have a colostomy whereas others are spared that procedure. Chapter 6 covers the 2 major classifications of lymphoma— Hodgkin disease and non-Hodgkin lymphoma—and how these classifications are subdivided into several others. This chapter also covers therapy with 131I-tositumomab (Bexxar; GlaxoSmithKline) and ibritumomab tiuxetan (Zevalin; Spectrum Pharmaceutical, Inc.). It is quite informative and leaves no aspect of nuclear cardiology behind. The authors are virtually a who’s who of experts in the nuclear cardiology field, and even though the price tag is a bit high, it is certainly worth the money. The 211-page, 16-chapter soft-cover study guide, published by the SNM in 2010, is designed to help technologists who want to enhance their career by earning the esteemed credentials NCT to put after their name and on their resume. It includes every aspect of the on-demand NMTCB examination: instrumentation/procedures/processing, anatomy/ physiology/pathology, radiopharmaceuticals/interventional drugs, nonpharmacologic (exercise) stress testing, and patient care. It also includes a comprehensive color atlas, a total of 14 continuing education units that are optional but affordable ($14.21 per credit), and a 140-question mock examination with the answers to boot! Not to mention that every chapter ends with follow-up questions to make sure the reader absorbs the information being presented. The guide begins by introducing the reader to basic cardiac anatomy and physiology. It discusses the heart chambers; electrophysiology; coronary artery distribution; heart valves, including the great vessels; cardiac diseases; and normal and abnormal physiologic responses to stress. The second chapter is an excellent complement to the first chapter as it segues from anatomy and physiology to the most basic of cardiac tests, the electrocardiogram. The electrocardiography chapter discusses the leads and which wall of the heart each lead records, the QRS complex, 8 steps to interpreting the electrocardiogram, and how to recognize the different rhythms of the heart and their origin, and the chapter finishes with the different disorders and disruptions of normal conduction. Because many programs do not focus on electrocardiograms and interpretation, this chapter really does make comprehension of electrocardiograms easier for even the most experienced technologist. The third chapter moves from the simplest of cardiac tests, the electrocardiogram, to other cardiac examinations that do not fall under the myocardial perfusion umbrella. These examinations include equilibrium radionuclide angiocardiography or multigated acquisitions, the first-pass study, myocardial infarction, myocardial viability, and bidirectional cardiac shunt studies. The book is divided into 5 sections, with multiple chapters in each. Section 1, “Myocardial Perfusion Imaging,” has several chapters dedicated to the review of cardiac anatomy, radiopharmaceuticals used in cardiac SPECT, and protocols for exercise and pharmacologic stress testing and includes some detail on acquiring and processing images. Section 2 includes 2 brief chapters, the first discussing imaging of acute chest pain and the second discussing radiopharmaceuticals used in myocardial infarction–avid imaging. Section 2 also provides examples of myocardial infarction scans showing normal and abnormal findings. Section 3 reviews dynamic cardiac imaging, including indications, methods, red blood cell labeling, and resting gated acquisitions, and concludes with an overview of first-pass imaging. There is also a chapter on gated image acquisition and processing of gated blood-pool SPECT. This chapter could have been longer and more detailed but otherwise is well written. Section 4 discusses cardiac PET. The overall content is good, but the discussion does not include cardiac sarcoidosis, and more detail on insulin loading for patients undergoing 18F-FDG viability studies would significantly enhance this section. The image displays are excellent and easy to understand. Section 5 is worth the read. The chapter on electrocardiography is well written, and the examples of electrocardiograms are excellent for someone just learning or as a review. The summary of drugs provided in the chapter on the cardiovascular system is a helpful overview for anyone studying for an examination. New to this second edition are a well-written chapter on radiation exposure from medical diagnostic imaging and a nice chapter on cardiovascular drugs that were not included in the first edition. Overall, this is an excellent reference book for nuclear medicine students, cardiology fellows, radiology residents, and technologists sitting for the nuclear cardiology technologist or NMTCB examination.
