Boughey","doi":"10.1158/1538-7445.SABCS18-ES7-1","DOIUrl":"https://doi.org/10.1158/1538-7445.SABCS18-ES7-1","url":null,"abstract":"Neoadjuvant chemotherapy (NAC) is known to decrease the extent of disease in the breast and increase rates of breast conservation. In addition, NAC also can reduce the likelihood of nodal positivity and hence decrease need for axillary node dissection and its associated morbidities. Three prospective clinical trials have assessed the false negative rate (FNR) of SLN after NAC for patients with clinically node-positive disease at presentation. The American College of Surgeons Oncology Group (ACOSOG) Z1071 study reported a false negative rate (FNR) of SLN surgery of 12.6% in patients with cN1 disease with 2 or more SLNs resected. The FNR was lower at 10.6% when dual tracer technique was utilized. Additional analysis showed that when a clip was placed in the positive node at diagnosis and the clipped node was resected as one of the SLNs, the FNR was 6.8%. The Canadian study (SN FNAC - sentinel node following neoadjuvant chemotherapy) reported a FNR of 13.3% when defining SLN with isolated tumor cells (ITC) as negative and 8.4% when including ITC in the definition of a positive SLN. The SENTINA study from Europe reported an overall FNR of 14.2%, however when excluding patients with only a single SLN removed the FNR was 9.8%. Further work with preoperative localization of the clipped node with a seed and resection of the localized clipped node along with the sentinel nodes (termed targeted axillary dissection) has been shown to have a FNR of 2.4%. Surgeons are incorporating SLN after NAC into their clinical practice for patients with a good response to NAC and thus use of SLN surgery after NAC for patients with node-positive breast cancer is increasing. There are multiple methods that can decrease the FNR of the procedure in this setting. These include use of dual tracer for SLN identification, resection of the initial biopsy-proven positive node, resection of at least 2 SLNs and use of immunohistochemical staining of the SLNs. There are several different techniques to assist with ensuring resection of the initially biopsy-proven positive lymph node which include varying methods to mark the node at diagnosis and ways to identify the node at time of SLN surgery. The node can be marked at the time of percutaneous lymph node biopsy (or at a subsequent visit prior to initiation of NAC) with a clip, a radioactive seed, or tattooed with ink. At the time of surgery, if ink or a radioactive seed was placed, the node can be identified using these techniques at the time of axillary surgery. If a clip was placed at diagnosis, this can undergo preoperative localization, with a radioactive seed or wire, to maximize likelihood of identifying the clipped node during surgery. For patients with biopsy-proven node-positive breast cancer, SLN surgery after NAC allows assessment of residual nodal disease and can enable patients who have their axillary disease eradicated by NAC to avoid ALND. Young women are more likely to develop more aggressive subtypes of breast cancer. Emerging data suggest the effect of age on breast cancer recurrence and death varies by tumor subtype, with young age particularly prognostic in women with ER-positive disease. Greater understanding of the biologic and genetic underpinnings of cancers that arise in younger women, potential differences in tumor and host responses including disease presenting in very premenopausal women or recently pregnant women, is critical to develop novel treatments to improve outcomes. There is also mounting evidence that access to care and behavioral differences leading to suboptimal treatment contribute to disparities in young women, including relatively poor adherence to adjuvant hormonal therapy. Prevention, detection and management of non-adherence, with attention to unique issues facing young women including desire for future fertility and pregnancy, has been the subject of recent research for this vulnerable population who, unsurprisingly, are at increased risk of psychosocial distress compared with older breast cancer survivors. Decision-support tools, and innovative, acceptable, scalable interventions to address anxiety and distress are under investigation. Finally, given that most young women will live for many decades after a diagnosis of breast cancer, it is imperative that we follow this population, in particular, over time and study their long-term, late effects in survivorship. While local approaches such as surgery and radiotherapy can be effective, they do not address extracranial disease, and can be associated with short- and long-term toxicity. As some subsets of patients live longer after a diagnosis of brain metastasis, there is increasing interest in systemic approaches in lieu of repetitive rounds of local therapy. To date, no systemic therapy has gained an indication for the specific treatment of breast cancer brain metastases; however, a number of regimens have been associated with efficacy in the central nervous system (CNS) in non-randomized studies. For patients with HER2-positive breast cancer, the combination of lapatinib and capecitabine has been associated with CNS response rates of 66% in the upfront setting, and 18-38% in patients whose disease has progressed after prior radiotherapy. Several investigational HER2-targeted tyrosine kinase inhibitors have shown promise. The combination of tucatinib, capecitabine and trastuzumab was associated with a CNS response rate of 42% in a small number of patients with brain metastases treated on the phase 1 trial. Notably, patients with brain metastases (either stable or progressive) are allowed on the current tucatinib registration trial (NCT02614794). The combination of neratinib and capecitabine led to a CNS response rate = 49% in a prospective phase 2 trial. Finally, trastuzumab-emtansine has been reported to lead to CNS regressions in up to 40% of patients, based upon case series from several institutions. For patients with ER-positive breast cancer, endocrine therapies such as tamoxifen and aromatase inhibitors have been reported to induce CNS responses in case reports and small case series. Recently, there has been interest in exploring the role of CDK4/6 inhibitors in patients with brain metastases. Preliminary results of the JPBO study support potential CNS efficacy of abemaciclib in patients with ER+ breast cancer; final results are awaited. To date, no commercially available targeted agents have demonstrated efficacy against brain metastases from triple-negative breast cancer. Trials of immunotherapy approaches in breast cancer have almost completely excluded patients with active/progressive brain metastases and thus, the safety/efficacy profile of immunotherapy in this setting is unknown, though promising data in melanoma and lung cancer would support its exploration in patients with brain metastases from breast cancer. Despite the fact that most chemotherapeutic agents do not cross the intact blood brain barrier, it appears empirically that the blood-tumor barrier can be sufficiently disrupted to allow for CNS efficacy in the case of established brain metastases. As the field has evolved, it has become apparent that there is not a “one size fits all” definition for EMT, and that the process is instead plastic, with epithelial and mesenchymal states being on the far left and right ends of a continuum where cells can exist in various intermediate states. How do these different intermediate states contribute to metastasis, if they do at all? And do these EMT cancer cells work in isolation, or do they form part of a cooperative network of heterogeneous tumor cells, together facilitating the metastatic process? In this talk, I will discuss the controversy around whether and how EMT is involved in the metastatic process, and the significant progress that we and other labs across the country have made in understanding its role in, and contribution to, the metastatic cascade. Among patients with HER2-positive disease, HER2-targeted therapy has been associated with substantial improvements in overall survival in the adjuvant setting, a high rate of pathologic complete response (pCR) after neoadjuvant therapy, and improvements in survival in patients with metastatic disease. While much of these improvements in outcome is due to more effective therapies, improvements in patient selection as a result of more accurate HER2 testing has also contributed to the better prognosis of patients with this disease. Data from clinical trials examining the benefit of trastuzumab in the adjuvant setting and published in the early 2000s (e.g., NSABP B31; NCCTG 9831) demonstrated that up to 25% of community-based assays reported as HER2-positive by immunohistochemistry (IHC) and up to 15% reported as HER2-positive by fluorescence in situ hybridization (FISH) could not be confirmed upon central laboratory testing. To address this high frequency of false-positive results and to improve the accuracy of HER2 testing, the American Society of Clinical Oncology (ASCO) and the College of American Pathologists (CAP) in 2007 published guideline recommendations for HER2 testing. These guidelines were subsequently updated in 2013 and, more recently, in 2018. Since the implementation of these guidelines, the accuracy of HER2 testing has improved. In fact, in current clinical practice, when HER2 IHC and FISH testing are performed according to the ASCO-CAP guidelines, approximately 95% of breast cancers can ultimately be categorized as either unequivocally HER2-positive or HER2-negative. In the remainder of the cases, combined IHC and FISH assay results provide a less straightforward picture of the HER2 status and, in turn, create uncertainty regarding the likelihood of benefit from HER2-targeted therapy. These include cases which on dual-probe FISH testing show: 1) HER2/CEP17 ratio >2.0 and an average HER2 copy number 6.0 signals/cell (polysomy); and 3) HER2/CEP17 ratio 4.0 and Citation Format: Schnitt S.
