Recent technological advances have made virtual surgical planning and threedimensional printing incorporating computer-aided design and computer-aided modeling possible, thereby shortening the ischemic time of free flap and enabling more sophisticated reconstruction. We report a successful case of osteocutaneous fibular free flap using this technique.","PeriodicalId":22016,"journal":{"name":"Soonchunhyang Medical Science","volume":"70 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-06-29","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"80293478","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

We first present the case of a 38-year-old woman who was diagnosed with appendiceal mucocele associated with appendiceal mucosal prolapse. The lesion was first found by colonoscopy and treated surgically. On examining the specimen, it revealed a dilated lumen containing mucinous fluid at the proximal portion, a polypoid mass at the mid portion, and a slightly dilated distal portion filled with mucoid material. But there was no histologic evidence of polyp. We report the first case of mucosal prolapse of the appendix resulting in mucocele with a review of the concepts of prolapse.","PeriodicalId":22016,"journal":{"name":"Soonchunhyang Medical Science","volume":"1 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2022-12-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"77527867","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Complications such as catheter-related infection, granuloma, and migration may occur, so continuous management is required. In this case, ITP insertion was performed due to central cord syndrome that occurred after a traffic accident. T-L spine computed tomography was performed due to persistent pain exacerbation 22 months after insertion, and epidural granuloma was confirmed. After that, granuloma removal and ITP revision surgery were performed, and pain control has been good so far. The purpose of this case was to investigate the risk factors and evaluation methods for catheter-related granuloma.","PeriodicalId":22016,"journal":{"name":"Soonchunhyang Medical Science","volume":"20 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2022-12-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"90624605","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Patients with inguinal endometriosis usually exhibit cyclic inguinal pain with a cyclic change in the size of the inguinal mass. It is more often found on the right side and commonly accompanies concomitant endometriosis lesions on pelvic organs or peritoneum. We report a case of inguinal endometriosis without any of the usual characteristics. A 48-year-old with non-severe dysmenorrhea and pelvic pain presented a left inguinal mass, palpable only when standing. Under the impression of direct inguinal hernia, laparoscopic herniorrhaphy was performed. Intraoperative laparoscopic findings revealed no other endometriosis lesion on the pelvic organ or in the abdominal cavity, and the histopathologic report confirmed it was endometriosis. Thorough inspection and excision of endometriosis lesions in the pelvic cavity are crucial for treatment. Therefore, an appropriate surgical plan following accurate preoperative diagnosis is important. If the intraoperative evaluation of endometriosis was not enough, postoperative gynecologic assessment is strongly recommended.","PeriodicalId":22016,"journal":{"name":"Soonchunhyang Medical Science","volume":"36 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2022-12-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"82437018","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Various complications due to the port system for intra-arterial chemotherapy have been reported so far, but no case has been reported in which a coil for port catheter fixation was extruded into the gastrointestinal tract lumen. We report a case of a patient who developed persistent gastrointestinal bleeding after a coil for port catheter fixation was extruded into the gastrointestinal tract lumen.","PeriodicalId":22016,"journal":{"name":"Soonchunhyang Medical Science","volume":"40 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2022-12-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"81523620","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Balloon-occluded retrograde transvenous obliteration has traditionally been applied in the treatment of gastric varix. Recently, plug-assisted retrograde transvenous obliteration (PARTO) has been widely and popularly applied for the treatment of gastric varix. There have been no reports of cases in which PARTO was performed in patients with both gastrorenal shunt and gastrocaval shunt. We report a case in which PARTO was performed on a patient with gastric varix who had both a gastrorenal shunt and a gastrocaval shunt.","PeriodicalId":22016,"journal":{"name":"Soonchunhyang Medical Science","volume":"122 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2022-12-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"82599423","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The most common site of spinal synovial cyst is the lumbar spine. If conservative management is failed, the operation must be considered. So far, microscopic cyst removal has been considered a treatment of choice. Recently, biportal endoscopic surgery is now becoming increasingly common in spine surgery and has the advantage of minimizing damage to related structures such as muscle, bone, and nerve. There was a case in which the synovial cyst of the L4–5 level was successfully removed through a biportal endoscopic surgery, so we would like to report it with a literature review.","PeriodicalId":22016,"journal":{"name":"Soonchunhyang Medical Science","volume":"69 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2022-12-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"73350065","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

It is a benign variant but its radiologic findings trick clinicians into concern about the tumor being malignant. A 42-year-old patient with a history of dysmenorrhea and adenomyosis presented a 60 mm-sized mass in the Douglas pouch with irregular echogenicity. The patient went under the first surgery for tumor removal, and its histopathologic diagnosis was endometriosis. We started daily dienogest medication to suppress tumor recurrence, but after 10 months we decided to stop the medication due to the side effects. Four months after the cessation, a new tumor recurred and after few months of observation, we performed the second surgery of tumor removal and total hysterectomy because of adenomyosis. The tumor was located in the retroperitoneal space in the Douglas pouch with severe adhesions. After the second surgery, the tumor marker Cancer antigen-125 level was normalized and the histopathologic result was endometriosis, which we concluded as polypoid endometriosis.","PeriodicalId":22016,"journal":{"name":"Soonchunhyang Medical Science","volume":"394 2 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2022-12-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"73147260","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Jaundice is a very visual clinical sign that both patients and physicians can easily recognize. It warrants further investigation regarding the cause of jaundice because jaundice is always a pathological sign regardless of its seriousness. Clinical manifestations of obstructive jaundice can be understood in terms of bile formation and secretion, and the anatomy of the biliary tree. Obstruction of a critical part of the biliary tree such as the common bile duct can cause jaundice, and investigating the etiology of jaundice is the first step of managing this condition. Choledocholithiasis, pancreatic ductal adenocarcinoma, cholangiocarcinoma, and biliary atresia are some of the common medical conditions that cause obstructive jaundice. Whenever jaundice is observed by a physician, early diagnosis and prompt treatment should be performed as early as possible for better outcomes.","PeriodicalId":22016,"journal":{"name":"Soonchunhyang Medical Science","volume":"18 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2022-12-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"73089300","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Some few cases and studies have been reported about the role of fluorine-18-fluorodeoxyglucose positron emission tomography/computed tomography (18FFDG PET/CT) in diagnosing MF-LCT. This is a case of a 55-year-old man who had a left neck mass for 4–5 months. He underwent excisional biopsies on the skin and soft tissue. The histopathologic report showed MF-LCT. The 18F-FDG PET/CT findings revealed hypermetabolic lesions in the skin, cervical lymph node, and visceral organ. We introduce a rare case of MF-LCT involving both cutaneous and extracutaneous lesions.","PeriodicalId":22016,"journal":{"name":"Soonchunhyang Medical Science","volume":"14 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2022-12-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"90174186","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
