{"title":"Real-time microscope-integrated optical coherence tomography-guided stromal fluid drainage in corneal hydrops for early resolution of corneal edema","authors":"Pranita Sahay, Rinky Agarwal, P. Maharana","doi":"10.4103/DLJO.DLJO_122_23","DOIUrl":"https://doi.org/10.4103/DLJO.DLJO_122_23","url":null,"abstract":"Aim: This study aims to describe a novel technique of microscope-integrated optical coherence tomography (MiOCT)-guided stromal fluid drainage in corneal hydrops with multiple stromal fluid clefts for rapid resolution. Materials and Methods: Eight patients of keratoconus with corneal hydrops with multiple stromal fluid clefts underwent this procedure. Medical management was initiated in all cases at presentation and intervention was performed only in cases with poor response to medical therapy at 3 weeks. The surgical technique comprised of MiOCT-guided localization of the stromal fluid clefts followed by multiple venting incisions into the stromal fluid clefts with micro-vitreoretinal blade and simultaneous air tamponade in the anterior chamber. Corneal massage with Merocel sponge further aided in draining and imbibing fluid through the incision site. Time taken for resolution of corneal edema, requirement for repeat procedure and any complication arising out of this procedure was noted. Results: Eight patients of acute corneal hydrops (5–30 years) underwent this procedure. The primary diagnosis was bilateral keratoconus for all. Majority (n = 7/8) of the cases were resolved in 3 weeks and all were resolved in 4 weeks. None of the cases required a repeat procedure. No intraoperative or postoperative complication was noted in any case. Conclusion: MiOCT-guided drainage of stromal fluid pockets in cases of corneal hydrops is a safe and effective technique. Real-time monitoring with MiOCT allows for fluid clefts location, instrument depth judgment, and complete drainage of fluid clefts. Reactive arthritis is a rheumatoid factor-seronegative, human leukocyte antigen B27-linked arthritis. It most commonly strikes individuals aged 20–40 years of age and is more common in men than in women. This inflammatory arthritis manifests several days to weeks after a gastrointestinal or genitourinary infection and is thought to be caused by an autoimmune response involving cross-reactivity of bacterial antigens with joint tissues. Genetic studies, though logistically difficult to offer all patients, have highlighted the hereditary nature of certain glaucoma subtypes, paving the way for improved risk assessment, and early intervention strategies for affected individuals. The integration of genome-wide association studies, next-generation sequencing techniques, and advanced bioinformatics has accelerated the discovery of novel genetic markers associated with glaucoma. The article follows the subsequent landmark developments in the field of retinal detachment surgery in the form of external buckles, cryotherapy, laser photocoagulation, operating microscopes with inverter, wide-angle viewing systems, vitrectomy, vitreous substitutes, and endolaser that have been deliberated on. It was done in the left eye (LE) of a Marfan's syndrome patient with bilateral congenital subluxated lenses who developed corneal decompensation after he underwent cataract surgery elsewhere with anterior chamber intraocular lenses 13 years ago. Materials and Methods: An observational, cross-sectional study was done on patients with POAG presenting to a tertiary eye center in Northern India. The structural assessment was based on peripapillary retinal nerve fibre layer (RNFL) thickness which was measured using spectral domain optical coherence tomography (SD-OCT) and functional assessment was visual field (VF) assessment which was done using the Humphrey Field analyzer model 740i. Patients were divided into groups according to Hodapp, Parrish, and Anderson's criteria. Results: Fifty eyes (26 patients) with POAG were included in the study. There were 26 (52%) right eyes and 24 (48%) left eyes. Patients' average age in the early group was 50.90 ± 6.14 years and the moderate group was 54.40 ± 7.51 years. The RNFL thickness was compared among early and moderate cases of POAG. In early cases, RNFL thickness was found to be 106.33 ± 7.09, and in moderate cases, it was 78.93 ± 26.2. The VF indices were compared among early and moderate cases. In early glaucoma, the mean deviation (MD) was found to be −2.82 ± 0.73, and in moderate glaucoma, it was found to be −11.49 ± 8.26. This correlation between structural and functional changes was found to be statistically significant for superior, inferior, average RNFL thickness, and VF MD. Nasal and temporal RNFL and MD were moderately correlated. Conclusion: In our study, we correlated RNFL thickness with VF indices. This correlation between structural and functional changes was found to be statistically significant for superior, inferior, average RNFL thickness, and VF indices.
