Ciuche","doi":"10.55453/rjmm.2023.126.2.13","DOIUrl":"https://doi.org/10.55453/rjmm.2023.126.2.13","url":null,"abstract":"\" We aim to introduce a 2022 update on CHD (carcinoid heart disease) considering a multidisciplinary perspective. This is a narrative mini-review. We searched English, full-length papers (PubMed). Inclusion criteria: original articles regardless of the level of statistical significance. We identified 44 papers and manually selected those with CHD, as follows: 8 original studies, 1 case series (N=9 patients), 16 case reports (N=1 patient/paper), and a total of 1,030 patients on published studies. The most remarkable results are the longest period of enrolment of 3-4 decades; CHD ratio among carcinoid syndrome of 37%; 30-day mortality post-cardiac surgery of 12%; median survival in CHD from 1.3 to 3.9 years (more than 2 years if valve intervention is provided); most useful prognostic markers: 5HIIA, NT-pro-BNP, but, potentially, chromogranin A. The specific protective role of somatostatin analogs against developing CHD is yet to be determined. CHD in the absence of carcinoid syndrome/liver metastasis may be related to ovarian NEN (neuroendocrine neoplasia) with a better outcome if prompt intervention. Remarkably, 3 guidelines are released regarding CHD on behalf of ENETS (European Neuroendocrine Tumor Society), and ESC (European Society of Cardiology). The high ability of MMPs in the degradation of extracellular proteins is an important event in many normal biological as well as pathologic processes, including tumor progression, tissue repair, wound healing, developmental morphogenesis, and inflammatory diseases. This review is focused on the description of the modulation mechanism of MMPs via oncoviruses. Up to now, a large number of oncogenic viruses have been detected and recognized as etiologic factors of multiple human cancers. Actually, by expression of several viral oncoproteins of tumorigenic viruses with the simple genetic system, we are able to deregulate or modulate the pathways of cell transformation procedure in malignancy. In brief, upregulated expression of MMPs genes by direct (through DNA binding) , as well as indirect interactions (by activating PI-3K/AKT, ERK/MAPK, IFN/JAK/STAT, JNK, NF-κB signaling pathways and/or immune response) with tumorigenic viruses, have a noticeable role in the proliferation of cells, as well as angiogenesis, mobility, EMT, invasiveness, and metastatic features. Aim: The present study aimed to determine the relationship between mental health, life satisfaction, and job satisfaction of nurses in Ilam city in 2022. The present correlational study was carried out on 160 nurses working in educational hospitals of Ilam city and was selected by stratified random sampling in 2022. A four-part questionnaire was used: the first part included a question on demographic information. Minnesota Satisfaction Questionnaire (MSQ) (1977), Diener et al.'s Satisfaction with Life Scale (SWLS) (1985), and Derogatis et al.'s Symptom Checklist-90 (SCL-9) (1973) were included in the second, third and the fourth parts, respectively. Resultsshowed no significant relationship between the total score of mental health and the total score of job satisfaction. There was a significant relationship between the total job satisfaction score and life satisfaction score (r=0.267, p=0.001). There was also an inverse and significant relationship between the total mental health score and the life satisfaction score (r=0.294, p=0.001). The collected data was analyzed using descriptive tests (frequency, mean percentage, and standard deviation) and non-parametric Spearman and Kruskal-Walli’s correlation tests in SPSS ver. 22. Pvalue<0.05 was considered as the significance level in all tests. Job satisfaction is effective in creating motivation, efficiency, responsibility, and social accountability in the personnel of any organization. Job dissatisfaction can lead to complications such as job burnout, illness, absenteeism, the job left, leaving many nursing jobs, and the reluctance of the young workforce to attend the wards. Special attention is now given to identifying the mechanisms and stages of disease development. From studies that have highlighted the importance of venous hypertension as an essential element in the development of the disease, to this day it has come up to demonstrating the fundamental role of the venous endothelium and glycocalyx in triggering and progressing of chronic venous disease. Biochemical, immunohistochemical, and functional investigations have allowed the evaluation of changes that occur in cellular metabolism and evidence of some immune-vascular factors responsible for vascular alterations in chronic venous disease: specialized receptors, adhesion molecules, cytokines, matrix and metalloproteinases. Although extensive research on these progressive neurodegenerative disorders has been conducted, pathogenetic treatments with long-term significant benefits are yet controversial. From a clinical perspective, there is an acute need to find therapeutic strategies that could delay cognitive impairment in patients diagnosed with Alzheimer’s disease (AD), vascular dementia (VaD), Lewy body dementia (LBD), etc. Also, slowing the transition from mild cognitive impairment (MCI) to clinically significant AD is another important clinical aspect, with a major impact on the patient’s daily functioning, quality of life, and caregivers’ burden. Acetylcholinesterase inhibitors (AChEI) are still the first line of treatment in AD patients, and they are also administered in the case of VaD or Parkinson’s dementia. Various nootropics have been studied in this population, as add-on agents. Highly purified animal tissue extracts (HPATE) are administered in patients with neurocognitive disorders due to their neurotrophic properties, but many questions remain unanswered regarding their pharmacodynamic characteristics. These extracts may be added to AChEI to enhance their pro-cognitive effect, but evidence to support the superior efficacity of this association versus AChEI monotherapy is mainly derived from low-to-medium quality clinical trials. In conclusion, HPATE may be a useful add-on to first-line pro-cognitive agents in AD and VaD, but larger trials with better methodology are needed. The present study is a cross-sectional study that was performed on 120 patients (60 cases in the sphincter maintenance group and 60 cases in the permanent colostomy group) who underwent surgery from February 2017 to November 2016. Quality of life assessment was provided with QLQ-C30 and QLQ-CR29 questionnaires. Finally, the collected data were analyzed using SPSS23 software. According to the results of the QLQ-C30 questionnaire, there was no statistically significant difference in patients' quality of life in terms of performance in the study groups (P˃0.05). Symptoms such as nausea, pain, shortness of breath, sleep disturbance, loss of appetite, constipation and economic problems in the study groups were not statistically significant (P˃0.05). But the mean symptoms of fatigue (P=0.038) and diarrhea (P=0.037) were significantly higher in the sphincter maintenance group than the permanent colostomy group. The mean score of patients' quality of life was 53.43 ± 8.40 in the sphincter retention group and 52.23 ± 11.45 in the permanent colostomy group (P = 0.556). According to the results, the quality of life in the two surgical methods of sphincter preservation and permanent colostomy was not statistically significant. The purpose of this systematic review is to evaluate the efficacy of physiotherapy, specifically chest physiotherapy, on different cytokines in Covid-19 and non-covid-19 patients. The main complications and symptoms of this virus are as follows; a dry cough, fever, and progressive dyspnea. Quickly, the coronavirus, which is named SARS-CoV-21, has spread worldwide, causing severe lung inflammation, respiratory distress syndrome, cardiac and renal injury, especially in comorbidities patients. Approximately 96% of the cases experience mild respiratory symptoms; some progress to pneumonia, respiratory insufficiency, acute respiratory distress syndrome, and multiorgan failure. The overall mortality rate per number of diagnosed cases is 4.6%; it can range from 0.2% to 15% according to age and health problems. Combination MeSH and text terms were used to perform the search strategy. Interventions in RCTs and clinical trials with or without comparison were assessed. Six studies met the inclusion criteria. It develops primarily in the retroperitoneal cavity, the genital tract and extremities. Leiomyosarcoma can also affect the bone, either as a primary tumor or as metastasis. Primary osseus leiomyosarcoma is a rare finding in patients with radiologically identified osteolytic lesions having an incidence of <1% of total primary bone tumors.Due to its low incidence patients with primary osseous leiomyosarcoma are never diagnosed before immunohistochemical analysis and case management is difficult because of the lack of specific indications pertaining to primary osseus leiomyosarcoma. Data on age specific incidence is scarce but there seems to be a tendency towards a higher incidence with advanced age. Recently, infants hypothesized that serum. NT-pro BNP levels might be a good predictor of septicemia severity and response to treatment and prognosis in these patients. This study investigated the relationship between serum natriuretic type B peptide level and neonatal sepsis. Materials and Method: In this case-control study, all full-term and pre-term neonates admitted to children’s Hospital, TabrizIran(2021), with sepsis and a septicemia diagnosis were confirmed after obtaining consent. The study was deliberately entered by the patient's parents or legal guardian. Complete Blood Count with Differential (CBC/Diff ), C - reactive protein (CRP) , Bacterial Culture (B/C) , Urine Analyze(U/A),Urine Culture (U/C) ,NT-pro BNP were checked and finally compared with laboratory results of the same number of infants who did not have clinical and laboratory symptoms of sepsis. After collecting samples, laboratory results, including serum levels of NT-pro BNP, were compared in case and control groups. Results: One hundred patients were studied in two groups. Thirty-eight patients (38%) were boys, and 62 patients (62%) were girls. The mean age of the patients was 7.58±7.46 days. The mean weight of the studied patients was 2811.80±620.33 grams, with a median of 2855 grams. The most common clinical symptom observed in patients in the case group was fever (100%) followed by Poor feeding (84%). In the control group, all patients had jaundice. Neonates with sepsis had significantly higher initial pro-BNP values than the control group (10023.80 vs. 2247.20; p=0.001). The NT-pro BNP level cut-off point in predicting the final treatment status and mortality of neonates with sepsis was 9583 pg/ml with 97.7% sensitivity and 93.6% specificity. In this study, by reviewing the authoritative books of Traditional Persian Medicine, we are looking for medicinal plants so that they can be used in the treatment of glaucoma or glaucoma. the purpose of this study is not to express the complete cure of glaucoma, but this article seeks to classify plants that from the perspective of Traditional Persian Medicine can be used as complementary medicine alongside conventional medical treatments. Methods and Materials: This is a summative qualitative content analysis that focused on Traditional Persian Medicine (TPM) texts from 2th to 13th AH centuries. Literature was searched during centuries 4th to 13th AH, and after described of glaucoma, extracting and classifying plants. Results: The scholars of TPM have opinions in the field of recognizing, describing and treating eye diseases and defining practical words. In almost all books of general Persian PM, the diagnosis and treatment of eye diseases have been discussed. Glaucoma in Traditional Persian Medicine is called “black water”.
