The population of chronic condition sufferers in the UK is considerable and teleconsultation use has, in recent years, been explored. COVID‐19 has created an additional pressure on health services to use teleconsultation. Diabetes mellitus affects approximately four million people in the UK. These lesions appeared shortly after initiation of subcutaneous premix human insulin injection. On examination, two well‐defined annular erythematous plaques of size around 10cm x 5cm with a rim of trailing scale over the margin were seen over the anterior aspect of bilateral thighs. He underwent assessment for diabetic complications screening and other relevant investigations. Histopathological examination of the lesion revealed uninvolved epidermis along with classical coat sleeve appearance of perivascular lymphocytic infiltration and dermal oedema in the upper dermis, which was suggestive of a diagnosis of erythema annulare centrifugum (EAC). However, the effects of exercise on glucose concentration are complex, and despite advances in supportive technologies and insulins, exercise remains a challenge for people living with T1D. Despite the challenges, Team Novo Nordisk (TNN) has demonstrated that with planning and determination, people with T1D can successfully pursue endurance sports. TNN has developed, refined, and adapted evidence‐based scientific strategies for managing nutrition and glycaemia in elite‐level athletes with T1D participating in ultra‐endurance events. Advances in technologies, such as continuous glucose monitoring, have facilitated non‐invasive and comprehensive observation of the TNN riders, who are supported by a multidisciplinary team of doctors, coaches, researchers, and nutritionists to formulate individualised data‐driven strategies. This article highlights the current learnings and data‐informed strategies being employed to assist management of diabetes in a professional endurance sport environment, almost 10 years after the publication of ‘Lessons from the professionals: diabetes and pro cycling’ (2013). In recent years, TNN has placed a high level of emphasis on research to improve safety and athletic performance while also striving to make these insights widely available to benefit the broader community. Many factors impact on a person's ability to manage their diabetes, and consequently on their mental health and wellbeing. These include everyday life pressures: work, celebrations, holidays and major life events, such as loss of a loved one or a job. Diabetes‐specific stresses include diabetes distress and burnout, fear of hypos and complications, eating disorders and denial, with an increased risk of anxiety and depression. Despite an established evidence base in other areas of health care, and emerging evidence that engaging in Expert by Experience activity can beneficially impact people living with diabetes, this important area of practice is still very much neglected in routine diabetes care and the number of people who are able to access this opportunity is small. This paper, written by a team of people who have either acted as, or supported people to become, Experts by Experience, offers practical guidance to diabetes teams to assist them in supporting individuals to engage in this powerful activity while maintaining psychological health and healthy boundaries.
