{"title":"Vasculitis asociadas a los anticuerpos anticitoplasma de neutrófilos (ANCA): de la A a la Z","authors":"","doi":"10.1016/j.piel.2023.12.015","DOIUrl":"10.1016/j.piel.2023.12.015","url":null,"abstract":"<div><p>Antineutrophil cytoplasmic antibodies associated vasculitis include microscopic polyangiitis, granulomatosis with polyangiitis, and eosinophilic granulomatosis with polyangiitis. The pathogenicity of ANCA involves neutrophil activation, NET generation, and complement activation. A common feature of ANCA-associated vasculitides is the presence of necrotizing inflammation and fibrinoid necrosis in the walls of small to medium-sized blood vessels.</p><p>Microscopic polyangiitis primarily affects small blood vessels and is associated with glomerulonephritis. Granulomatosis with polyangiitis involves necrotizing granulomas, affecting the upper and lower respiratory tract, kidneys, and skin, often presenting with purpura. Eosinophilic granulomatosis with polyangiitis, predominantly eosinophilic, affects multiple organs, including the respiratory, gastrointestinal, renal, and cutaneous systems, featuring palpable purpura and characteristic ulcers.</p><p>Multidisciplinary treatment involves glucocorticoids, immunosuppressants, and monoclonal antibodies. Treatment duration depends on the disease phase, with aggressive therapy for active phases and maintenance therapy for remission. Close monitoring is essential. This study describes a series of procedures, types of closure, their recurrence and complications.</p></div><div><h3>Materials and methods</h3><p>We conducted a retrospective observational study of 33 surgical procedures in 28 patients diagnosed with hidradenitis suppurativa performed from January 2017 to March 2023.</p></div><div><h3>Results</h3><p>84.8% of the procedures were performed on women. Recurrence in infiltration and drainage was 57.14% (<em>n</em> <!-->=<!--> <!-->12), 33.33% with deroofing (<em>n</em> <!-->=<!--> <!-->1) and 11.11% with local resection (<em>n</em> <!-->=<!--> <!-->1). Recurrence in primary closure was 16.66% (<em>n</em> <!-->=<!--> <!-->1) and 12.5% in closure by secondary intention (<em>n</em> <!-->=<!--> <!-->1). Patients with severe disease had less recurrence, followed by moderate and mild disease.</p></div><div><h3>Conclusión</h3><p>We highlight primary closure with the highest rate of complications and local resection as the procedure with the lowest recurrence. This disease has an impact on the psychosocial aspect of the affected individuals. The aim of this article was to determine the clinical-epidemiological characteristics of AGA in adult patients in a tertiary hospital in Trujillo during 6 months.</div></div><div><h3>Materials and methods</h3><div>Prospective descriptive observational study. Data collection forms of patients older than 18 years with a diagnosis of AGA were used to obtain information.</div></div><div><h3>Results</h3><div>Of a total of 40 patients with AGA, 55% were male and 45% female, mainly aged 31–49 years and with a family history of AGA (72.5%). The typical grade IIIv pattern prevailed in males whose most frequent age range was 31–49 years. The diffuse grade I pattern (25%) predominated in females, who were < 70 years old.
