{"title":"Intravenous thrombolytic therapies in the management of unknown onset and wakeup strokes: A scoping review","authors":"Saima Nazish","doi":"10.56310/pjns.v17i02.184","DOIUrl":"https://doi.org/10.56310/pjns.v17i02.184","url":null,"abstract":"Background : The efficacy and safety of thrombolytic therapies for the management of unknown time onset stroke (UTOS) and wake-up stroke (WUS) are in a debate.  \u0000Objective: The objective of this scoping review is to map the existing research literature to answer the research question: What is the efficacy and safety of intravenous thrombolysis in the management of unknown time onset stroke?  \u0000Data Sources:  The literature search was conducted using the search terms {(Intravenous thrombolysis)} AND {(Unknown time onset stroke)} in pub med and Intravenous thrombolysis *, Unknown time onset stroke *in Cochrane. The review was limited to articles written in the English language. Databases searched included MEDLINE, PubMed and Cochrane electronic databases complemented with a manual search. \u0000Study Selection and Data Extraction: 45 articles of potential relevance were selected. Out of them 14 were containing relevant information direct to the question. The analysis was conducted by charting descriptive numerical data and by content analysis of the narrative representations \u0000 Results:  From 14 relevant papers, 2 Retrospective observational, 2 case control, 1 pilot study, and 5 recent randomized control trials were included for critical appraisal and review writing. \u0000Conclusions:  The penumbral mismatch in UTOS is a cogent method to recognize the patients for thrombolytic therapy without significant risk, but the appropriate imaging criteria to maximize the efficacy and minimize bleeding complications of thrombolytic treatment still has to be fully defined. His complete blood counts showed elevated eosinophil count. Nerve conduction showed neuropathy. Diagnosed Churg Strauss syndrome due to presence of asthma, eosinophilia, neuropathy and pulmonary infiltrates. It is a treatable disorder if diagnosed earlier in life. The objective was to identify PKU patient(s) in the intellectually disabled children. \u0000Methods: Blood samples (n=100) were collected from intellectually disabled children from Faisalabad, Pakistan. Screening was performed on plasma samples through HPLC, and DNA samples were examined for mutational analysis of PAH through direct PCR and SSCP analyses. \u0000Results: 85% consanguinity rate, the average BMI (16.15 kg/m2) and head circumference (50.21 cm) was observed. Moreover, through biochemical and genetic analyses, not a single PKU patient was identified. \u0000Conclusion: Based on just one previous report and our small dataset it is concluded that either mutations are not common in the hotspot regions or chances of occurrence of PKU might be rare in Pakistan. Pirani, Rabeeka Aftab, Tania Nadeem","doi":"10.56310/pjns.v17i02.192","DOIUrl":"https://doi.org/10.56310/pjns.v17i02.192","url":null,"abstract":"OBJECTIVES \u0000To determine the frequency of depression in caregivers of stroke patients at a tertiary care teaching hospital in Karachi Pakistan. \u0000 METHODS \u0000A cross-sectional study was conducted among caregivers of stroke, who were recruited from the Aga Khan University Hospital Karachi Pakistan, between January 2018 to October 2018. Data was collected through the Urdu Hamilton rating scale for depression (HAM-D-U) and it was analyzed using descriptive and inferential statistics. \u0000 RESULTS \u0000A total of 136 caregivers participated in the study. Among them, 44.1 % were male and 55.9% were female. The mean age of caregivers was 45.38 ± 10.33 years. Using the HAM-D-U scale, depression was present in 64 (47.1%) caregivers. Out of them (n=64), 46 (33.8%) caregivers had mild depression whereas 18 (13.2%) had moderate depression. Depression was found to have a significant relationship with the age of the patient (p=0.002), education level of caregiver (p= 0.012), employment status of caregiver (p=0.012), being a sole caregiver (p=0.039), and monthly family income (p=0.016). \u0000 CONCLUSIONS \u0000Caring for patients with neurological disorders is highly challenging and demanding. The need for this role may arise unexpectedly in one’s life; leaving little space for adjustment and coping. Depression itself can be very paralyzing for the sufferer especially if moderate to severe in intensity. Depression in stroke caregivers can be missed as patients are the center of a clinical visit. Due to a lack of knowledge and awareness among the Common public. The objectives of this study were to determine common myths and misperception about epilepsy by analysing a comprehensive panel of variables in our local population. Methods: We conducted a face-to-face questionnaire interview survey in three different shopping malls 1000 people were randomly enrolled in this cross-sectional study at Quetta, Baluchistan, during the period of twenty-one august 2021 to 30 august 2021. \u0000Results: A total of 1000 people were interviewed among them 220(22%) were male and 780(78%) were female. People of all age group were included in the study. Out of thousand people 419 (41.9%) says epilepsy is brain disease, 488 (48.8%) responds its ghost attack, 23(2.3%) believes its psychosis. 980(98%) respondents had heard about epilepsy. The aspects of knowledge tested were on causes, types, and management of epilepsy. Forty-nine (4.9%) people believes that cause of epilepsy is head trauma, 488 (48.8%) said its Ghost attack, 67 (6.7%) said it’s because of evil eye, and 293 (29.3%) peoples believes that epilepsy is caused by Black magic. 928 (92.8%) who believed on domestic stunts such as sniffing shoes is effective in epilepsy. This result was much surprising because in this era of modern medicine and technology, still superstitious believes prevail among large population \u0000Conclusions: This study revealed a lack of awareness about epilepsy in Baluchistan, as well as the prevalence of both negative and favourable attitudes about Epilepsy.
