Current surgical techniques designed to remove the synechiae early on are quite cumbersome. We describe a simple technique of anterior synechiolysis, performed 1 to 4 weeks postoperatively, using a bent 26-gauge needle, that proved safe and effective in seven cases. The only complication was a hyphema in one case. {"title":"Optimum timing of postoperative adjustment in a rabbit model of adjustable suture strabismus surgery.","authors":"J M Holmes, A M Townshend","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Strabismus surgeons disagree on the optimum timing of the postoperative adjustment in adjustable suture surgery. We compared the peak force necessary for adjustment at various postoperative times using a rabbit model. Twenty-four eyes of 12 rabbits underwent a superior rectus recession of 5 mm using a standard adjustable suture technique. The eyes were randomized to adjustment at 15 minutes, and at 6, 24, and 48 hours postoperatively. The peak force required to advance the muscle 3 mm was measured using a strain gauge. During each adjustment, the peak force was noted to be that required for initial disinsertion of muscle from sclera. The mean peak force required was 14 +/- 5 g at 15 minutes, 37 +/- 10 g at 6 hours, 60 +/- 40 g at 24 hours, and 103 +/- 52 g at 48 hours. There was a significant relationship between the time of adjustment and the peak force required for adjustment (F = 7.8, P = 0.001). Post-hoc analysis showed that the peak force required at 48 hours was greater than that required at 24 hours (P = 0.04), 6 hours (P = 0.003), and at 15 minutes (P = 0.0002). The force required for adjustment at 24 hours and beyond was greater than the maximal force generated by an extraocular muscle. Matsushita","doi":"10.3928/1542-8877-19950501-12","DOIUrl":"https://doi.org/10.3928/1542-8877-19950501-12","url":null,"abstract":"We retrospectively examined the effects of laser suture lysis on the long-term results of initial trabeculectomies in 130 consecutive eyes that had an intraocular pressure greater than 21 mm Hg during the first 4 postoperative weeks. After the high intraocular pressure was noted, 46 eyes underwent laser suture lysis; the other 84 did not. The 2-year cumulative probability of success in the laser suture lysis group was .84 and, in the control group, .82 (P > .05). Intraocular pressures in the two groups 1 and 2 years after trabeculectomy were similar. {"title":"Ocular motility defects in patients with the Krupin valve implant.","authors":"J W Frank, T W Perkins, B J Kushner","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>To determine the prevalence of ocular motility defects following placement of a Krupin valve with disk and adjunctive mitomycin-C in glaucoma patients, a retrospective case series of all patients receiving a Krupin valve with disk and intraoperative mitomycin-C in a university-based referral practice was conducted. Each of the seven consecutive patients undergoing placement of a Krupin valve with disk in one eye because of uncontrolled glaucoma, developed significant primary position heterotropia or limitation of ocular rotations. One patient had significant postoperative diplopia. We conclude that the Krupin valve with disk used with intraoperative mitomycin-C can be associated with the development of ocular motility defects.
