She presented to the emergency department with facial and neck pain, difficulty in swallowing, and a feeling of tightness and pressure in the chest. Computed tomography revealed free air foci extending from the right mandibular region to the mediastinum. Pneumomediastinum related to dental root canal treatment was considered and the patient was admitted to the thoracic surgery department and oxygen therapy and prophylactic antibiotics were initiated. After 3 days of observation, the emphysema resolved, the symptoms disappeared, and the inflammatory markers remained within normal limits. The patient was discharged in stable condition.","PeriodicalId":196245,"journal":{"name":"Yemen Journal of Medicine","volume":"62 9","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-12-16","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"138967164","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

This review offers an in-depth analysis of the complex relationship between epithelial failure, mucosal inflammation, and microbial dynamics, explaining their combined role in the pathophysiology of chronic sinusitis. We address the important role of the epithelial barrier and the effects of its disturbance on immune responses. This review focuses on the mucosal inflammation found in chronic sinusitis cases, highlighting the cytokines and cells that contribute to the ongoing inflammatory response. We provide an in-depth overview of the sinus microbiome’s role in both health and disease states, looking at the effects of bacterial, viral, and fungal colonization on sinus inflammation and their relationship with the epithelial barrier. We also discuss the role of mucociliary clearance and the consequences of its disruption on ongoing inflammation. Each of these factors has a relationship with the others, and together they contribute to chronic inflammation. Current treatment methods are somewhat effective but have limitations. These treatments include methods to improve epithelial function, reduce inflammation, use antibiotics and antifungals, introduce probiotics, and improve mucociliary function. As we learn more, we look at potential new treatments and the need for a complete approach that covers all areas. In short, understanding these areas is key to improving patient care and encouraging more research.","PeriodicalId":196245,"journal":{"name":"Yemen Journal of Medicine","volume":"68 11","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-12-16","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"138967595","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Pre-eclampsia, a globally significant pregnancy complication associated with maternal mortality and adverse outcomes, is on the rise, driven by factors such as obesity, advanced maternal age, and the increasing prevalence of underlying medical conditions. This review explores recent advancements in managing these conditions, extending the scope to encompass cases of hypertension during pregnancy and up to 12 weeks postpartum. Notably, chronic hypertension is now incorporated within the HDP definition, with blood pressure (BP) assessment as a cornerstone in diagnosis. Emphasizing the importance of out-of-office BP measurements, including ambulatory and home-based monitoring, is underscored for pregnant and non-pregnant women. Recent insights into preeclampsia and other HDP have paved the way for enhanced preventive measures, screening protocols, and management strategies. With emerging early detection and intervention tools, there is significant potential for reshaping clinical practices.","PeriodicalId":196245,"journal":{"name":"Yemen Journal of Medicine","volume":"30 4","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-12-16","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"138967882","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The main cause of CPE is disruption of the thoracic duct caused by traumatic or non-traumatic causes. High triglyceride, cholesterol, and/or chylomicron levels confirm the diagnosis. Recognizing the chyle leak site and underlying etiology is critical to the management of this clinical condition. There is no specific consensus on chylothorax treatment. Abdominal-pelvic ultrasound finding was consistent with an ovarian tumor, and bilateral oophorectomy and total hysterectomy were performed. The ovarian mass removed was white and contained a viscous fluid. It weighed 7 kg and was oval in shape with a length of 57 cm on the long axis and 48 cm on the short axis. Histopathological analysis of sections from the removed mass and samples of aspirated cyst fluid confirmed the diagnosis of mucinous cystadenoma of the right ovary. Postoperatively, the patient remained in the hospital for 10 days, was discharged in good condition, and was required to visit our outpatient clinic every 6 months. At her past visit, 1 year after discharge, the patient was in good condition and no mass was detected on abdominal-pelvic ultrasound.","PeriodicalId":196245,"journal":{"name":"Yemen Journal of Medicine","volume":"51 41","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-12-16","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"138995597","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

This study was conducted to determine the relationship between pregnant women’s healthy lifestyle behaviors and distress during the COVID-19 pandemic. Materials and Methods: The data were collected between April and October 2021 through face-to-face interviews with pregnant women over 18. The sample of the study consisted of 209 pregnant women. The Tilburg pregnancy distress scale and the healthy living behaviors scale were used to collect data. Descriptive statistics, parametric tests, and regression analysis methods were used to analyze the data in the study. Results: As a result of the study, the mean Tilburg Distress Scale score of pregnant women was 19.91±9.15. The total psychological distress score of 26.8% of the pregnant women was observed to be higher than the cutoff point. The mean score of the healthy living behaviors scale of pregnant women was 120.71±17.04. It was observed that the Tilburg Distress Scale total score and the spousal participation sub-dimension score affected pregnant women’s healthy living behaviors scale total score (values p<0.001 and p<0.005, respectively). It was determined that the total score of the Tilburg Distress Scale was 0.34 times effective on the quality of life during pregnancy, and the spousal participation sub-dimension was −2.16 times effective. Conclusion: This study concluded that pregnant women experienced psychological distress during the COVID-19 period and that this situation affected the quality of life of pregnant women.","PeriodicalId":196245,"journal":{"name":"Yemen Journal of Medicine","volume":"59 11","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-12-16","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"138967010","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
