The NHS has a target for net zero emissions by 2040, with improved waste recycling. Literature demonstrates need for greater engagement with recycling: one audit found over 40% of all hospital waste is recyclable. Despite this, recycling is not commonplace in healthcare settings. <h3>Methods</h3> We started our project with a GEMBA walk, evaluating the patient journey through the department and identified areas we could make greener. We then surveyed staff, asking what was the most important and sustainable project, identifying recycling. We began our quality improvement project by gathering information about commonly recyclable items, before educating staff during a department-wide teaching session. Following this, we introduced educational posters to reinforce knowledge and prompt staff to recycle. After each intervention, we weighed the recycling and assessed understanding with a quiz. <h3>Results</h3> There was an increase in knowledge of recycling amongst staff; 84% correctly identified recyclable items compared to 47% before the teaching. So far, we have observed no significant increase in weight of the recycling bins with weekly averages of 2kg to 2.1kg. <h3>Discussion</h3> There was an increase in recycling knowledge despite the recycling weight being unchanged. At the beginning of our project we observed non-recyclable items being disposed of in the recycling. Therefore, a static weight may reflect positive change in practice. Findings demonstrated barriers to recycling included lack of bins. Our next intervention will be increasing the recycling bin availability, to make changes more accessible and sustainable. We observed wide variations in recycling information printed on medical packaging, and believe this is something that should be systemically changed. CD8 Encephalitis (CD8E) is a newly recognised condition in the setting of Human Immunodeficiency Virus (HIV) infection which can present acutely with classical radiological findings and brain parenchymal infiltration of CD8+ T-cells. This review attempted to clarify symptomatology and determinants of this condition. <h3>Methods</h3> A literature review was conducted, utilising PubMed and Google Scholar databases, reviewing papers published between 2006–2022. Eighteen papers, totalling 57 patients, were identified and analysed. Statistical analysis was undertaken using Chi-squared and Wilcoxon rank-sum tests, with p&lt;0.05 deemed significant. <h3>Results</h3> Among the cohort, a female (61%) and Black African (70%) preponderance was identified. Headache was commoner in females (p=0.006) and in those who died (p=0.02). There was no statistically significant association between baseline CD4 count (p=0.079) and viral load (p=0.72) with mortality. Overall, 77% of patients had classical radiological findings of gadolinium-enhancing punctate and perivascular white matter lesions, with many requiring a brain biopsy to confirm the diagnosis. CSF analysis typically showed lymphocytic pleocytosis, demonstrating CD8+ predominance in 90% of where the lymphocyte subset was tested. Corticosteroids were commonly prescribed (64%) and demonstrated mortality benefit, with an overall survival in this group of 71% (p=0.0008). In those who died, the median survival was 5.5 months. <h3>Discussion</h3> Amongst symptoms, headache appeared more common in females and was significantly associated with death. CD8E is likely under-diagnosed, possibly due to overlapping features with other illnesses and lack of physician experience. Corticosteroids demonstrated a mortality benefit, but more studies should determine their optimal dosing, duration, and use of steroid-sparing agents. {"title":"O11 Surveillance of Mpox cases attending sexual health services in England (SOMASS): design, implementation and initial findings from the SOMASS data collection tool, 2022","authors":"Hannah Charles, Mateo Prochazka, Judith Murray, Suneeta Soni, Lewis Haddow, Katie Beets, Victoria Pilkington, Nicola Low, Sophie Candfield, Rachael Jones, Tanya Bleiker, Claire Dewsnap, Matt Phillips, David Phillips","doi":"10.1136/sextrans-bashh-2023.11","DOIUrl":"https://doi.org/10.1136/sextrans-bashh-2023.11","url":null,"abstract":"<h3>Introduction</h3> To understand the clinical presentation, severity and epidemiology of individuals with Mpox attending sexual health services (SHSs) in England, we developed a tool for Surveillance of Mpox Cases Attending Sexual Health Services (SOMASS). <h3>Methods</h3> Experts from the UK Health Security Agency (UKHSA), British Association of Sexual Health and HIV and the British Association of Dermatologists formed a multi-disciplinary working group in May 2022 and co-developed a web-based data collection tool. The UKHSA hosted the tool, which was accessible from July 2022. SHS clinicians abstracted information from clinical records into SOMASS following consultations with patients suspected to have Mpox. <h3>Results</h3> As of 17th November 2022, 276 SOMASS responses were submitted from 31 SHSs in England, representing 8% of all confirmed Mpox cases in England as of 14th November 2022. Of these, 274 were subsequently laboratory-confirmed Mpox positive; in two cases the laboratory result was unknown but assumed to be positive, and therefore these individuals were retained for analysis. Where recorded, most (245/261; 94%) people identified as gay, bisexual or men who have sex with men (GBMSM). For 46% (127/276), dermatological lesions were the initial symptom. Lesions were mostly asymmetrical and polymorphic, predominately affecting genital area and perianal areas. Among GBMSM, we found an association between receptive anal intercourse and proctitis (27/115; 24% vs. 7/130; 5%; p<0.0001), and perianal lesions as the primary lesion site (46/115; 40% vs. 25/130; 19%; p=0.0003). <h3>Discussion</h3> SOMASS demonstrates multi-disciplinary, responsive, and agile working between front-line sexual health practitioners, clinical professional bodies and the national health protection agency. The data collection tool improved surveillance, strengthened the knowledge base and supported clinical care during the Mpox outbreak. All initially presented as heterosexual, 15 were male, majority were married, aged 40–70 and from a similar area. We discuss the vital input sexual health (SH) had in identifying the outbreak and liaising with partner agencies. We also review the outreach and difficulties supporting people who may not feel able to disclose risk factors (RF) especially to non SH staff 2 cases of HBV/syphilis coinfection were diagnosed in the hospital in December 2020 with no identified source of infection. When the men attended SH both disclosed male sexual contacts. Concern of a cluster was raised by SH. 9/12 passed and SH were advised of 5 further cases. SH linked with Public Health and an Incident Management Team was created which identified weakness in pathways between services to realise an outbreak. Extensive collaborative work was done to improve communication between departments and training on sexual history taking. All cases were then referred to SH, of those that attended, 5 men newly disclosed male sexual contacts, 2 were wives of cases, 7 still had no identifiable infection source Genotyping was possible for 12, 10 were type A2. This is the most common strain and associated with MSM Several collaborative outreach events were arranged, >80 contacts were made but no new cases were seen and none disclosed RF. Communications were made to general public but due to the concern of inadvertently “outing” people or creating stigma in small communities it was decided to not focus them at MSM. This outbreak highlights SH’s vital importance in supporting patients disclose and reduce infection spread.
