The wide-angle view provided by the endoscope enables transcanal access to the tympanic cavity and its otherwise difficult-toreach extensions. The attic, sinus tympani, facial recess, and hypotympanum are the primary sites of disease and surgical failure to cure. This report is a summary of the author’s 15 years of experience with the use of transcanal operative endoscopy as the primary approach to the management of cholesteatoma. During the last 15 years, our laboratory has focused on elucidating the molecular pathology of otosclerosis with the long-term goal of developing better forms of therapy. These studies have brought us to the point where we are ready to consider and develop new and innovative treatment strategies. Glomus tympanicum and glomus jugulare lesions are associated with the temporal bone. The usual presenting symptom is pulsatile tinnitus followed by hearing loss. There is no controversy with the management of glomus tympanicums as they are mainly treated with surgical removal.
