Projection moiré interferometry was employed to determine the out-of plane displacement produced by laser exposure, and in-situ optical microscopy of reference markers on the eye was used to measure in-plane shrinkage.\u0000\u0000\u0000RESULTS\u0000Measurements based on moiré interferometry allow a three dimensional view of shape changes in the eye surface as laser treatment proceeds. Out-of-plane displacement reaches up to 1.5 mm with a single laser spot exposure. In-plane shrinkage reached a maximum of around 30%, which is similar to that reported by Sasoh et al (Ophthalmic Surg Lasers. 1998;29:410) for a Tm:YAG laser.\u0000\u0000\u0000CONCLUSION\u0000The moiré technique is found to be suitable for quantifying the effects of CO2 laser scleral shrinkage and buckling. Ten to 15 minutes later, the retinas were irradiated with a dye laser (spot size, 2 mm; 10-60 mW; 10-60 seconds; fluences 3.2 to 95.5 J/cm2) at two wavelengths: yellow (568 nm) and red (647 nm). Fluorescein angiography was performed 2 and 24 hours later. Parallel controls were irradiated without a photosensitizer. Histology was performed immediately after the 24-hour angiogram. observed 24 hours after PDT in eyes subjected to yellow laser irradiation at 20 mW for 60 seconds (fluence, 38.2 J/cm2) and at higher powers for lesser durations: 30 mW for 40 seconds (fluence, 38.2 J/cm2), 40 mW for 20 seconds (fluence, 25.4 J/cm2), and 50 and 60 mW for 10 seconds (fluences, 15.9 and 19.1 J/cm2, respectively). Light microscopy showed occlusion of the choriocapillaris in all eyes with angiographic responses. We identified upper lid masses and a limitation of supraduction in the left eye of a patient who was associated with extrusion and fragmentation of the MIRAgel episcleral exoplant 11 years after scleral buckling surgery. The material could not be removed completely because of fragmentation. Nguyen","doi":"10.3928/1542-8877-20020501-03","DOIUrl":"https://doi.org/10.3928/1542-8877-20020501-03","url":null,"abstract":"OBJECTIVE\u0000To present a method for remodeling oversized blebs using the THC:YAG laser and describe its use in treating hypotony and bleb leaks.\u0000\u0000\u0000METHODS\u0000Retrospective medical record review of 13 patients who underwent holmium laser bleb revision.\u0000\u0000\u0000RESULTS\u0000Thirteen eyes of 13 patients were analyzed. All the patients had an oversized bleb and 2 patients presented an associated late-onset leaking bleb. Mean preoperative IOP was 4.1 mm Hg. THC:YAG laser energy was applied to the bleb to remodel and/or close the leak. After a mean follow up of 17.3 months, the mean IOP was 7.84 mm Hg +/- 2.3. Postsurgery complications included hyphema and the inadvertent rupture of a conjunctival microcyst. The mean follow up was 18.3 months.\u0000\u0000\u0000RESULTS\u0000An incomplete posterior vitreous separation was noted in 10 eyes (66.7%). An epiretinal membrane was present in 4 eyes (26.7%), and in 3 eyes, a retinal fold along the arcade was noticed. Significant visual improvement occurred in all eyes; visual acuity of 20/40 or better was achieved in 14 eyes (93.3%). Only 3 eyes developed lens opacity after surgery.\u0000\u0000\u0000CONCLUSION\u0000Vitrectomy in Terson syndrome provides excellent visual recovery. We hypothesized that hyperbaric oxygen (HBO) therapy improves the visual field (VF) in these patients.\u0000\u0000\u0000MATERIALS AND METHODS\u0000Vitrectomy for MH was performed on 73 eyes from 1994 to 1997. TVF defect was detected in 19 eyes and, of that 19, 12 patients were followed. Seven patients were treated with HBO therapy and 5 were controls. HBO was performed for approximately 110 minutes a day with 100% oxygen inhalation and a maximum of 2.8 atmospheric pressure. This continued for 20 days. The preoperative VF determined by kinetic perimetry was considered to be 100%, and the VF following HBO therapy was compared with that standard.\u0000\u0000\u0000RESULTS\u0000We detected VF defect (postoperative VF area average 71.9+/-12.8% of the preoperative VF). In all 5 patients who had no HBO therapy, TVF defects remained, while the TVF recovered remarkably in all patients treated with HBO therapy. The VF recovered to 81.7+/-16.7% of the preoperative VF after 3 days of HBO, and to 91.6+/-15.8% months after HBO therapy.\u0000\u0000\u0000CONCLUSION\u0000We speculated that the cause of TVF defect is likely to be chorioretinal circulation disturbance during surgery, and that HBO activates the retinal cells and improves VF.
